Goiter is an enlargement of the thyroid gland that often produces a noticeable swelling in the front of the neck. This enlargement can be caused by iodine deficiency, inability of the body to use iodine correctly, or a variety of thyroid disorders, including infection , tumors, and autoimmune disease. Some environmental pollutants, heavy metal poisonings, and certain drugs can also contribute to goiter formation. hypothyroidism and the symptoms include fatigue, weight gain, heavy menstrual bleeding in women, dry skin and hair, as well as goiter. Causes There are different kinds of goiters. A simple goiter usually occurs when the thyroid gland is not able to produce enough thyroid hormone to meet the body's requirements. The thyroid gland compensates by enlarging, which usually overcomes mild deficiencies of thyroid hormone.
A simple goiter may be classified as either an endemic (colloid) goiter or a sporadic (nontoxic) goiter. Endemic goiters occur within groups of people living in geographical areas with iodine-depleted soil, usually regions away from the sea coast. People in these communities might not get enough iodine in their diet. (Iodine is vital to the formation of thyroid hormone.) The modern use of iodized table salt in the U.S. prevents this deficiency; however, it is still common in central Asia and central Africa. In most cases of sporadic goiter the cause is unknown. Occasionally, certain medications such as lithium or aminoglutethimide can cause a nontoxic goiter.

A goitre (or goiter) ( Latin struma ) is a swelling in the neck due to an enlarged thyroid gland . The most common cause for goitre in the world is iodine deficiency. Other causes are: Iodine is necessary for the synthesis of the thyroid hormones, triiodothyronine and thyroxine (T3 and T4). When iodine is not available these hormones cannot be made. In response to low thyroid hormones, the pituitary gland releases thyroid stimulating hormone (TSH). Thyroid stimulating hormone acts to try and increase synthesis of T3 and T4, but also causes the thyroid gland to grow in size as a type of compensation. Goitre was previously common in many areas that were deficient in iodine in the soil . The condition now is practically absent in affluent nations, where table salt is supplemented with iodine.

Goiter
Goiter refers to an enlarged thyroid gland. It can occur in both hypothyroidism, as thyroid cells enlarge in their efforts to trap more iodine, and in hyperthyroid as thyroid cells elongate and swell due to to their vigourous assembly line production of thyroid hormone.
There are approximately 25 different types of goiter, depending on the type of pattern created when cells take up iodine and by characteristic changes in thyroid tissue. Goiters may also be nodular, having hot or cold nodules, or they may be vascular, with changes present in the veins and arteries that supply the thyroid gland. In Graves' disease, iodine distribution is even although the uptake of iodine is increased. For these reasons, goiter in Graves' disease is called "diffuse toxic goiter."
Why do some goiters continue to grow? In hypothyroidism due to iodine deficiency, goiters continue to enlarge as the cells try to pull in more iodine. In autoimmune thyroid disorders, including Graves' disease and Hashimoto's thyroiditis, thyroid growth immunoglobulins (TGI) are the primary cause of goiter enlargement. TGI stimulate thyroid cell growth, whereas thyroid stimulating immunoglobulins (TSI) cause excess thyroid hormone production.

Positive Deviance to Fight Goiter
The Indonesia PD Network, comprised of six International NGOs and their counterparts, published their 4th bulletin, with articles in both English and Bahasa Indonesian focusing on on-going PD informed-projects addressing goiter and iodine deficiency, among others. This is a story from Cikurutug Cikurutug lies in the mountains of West Java in the District of Cianjur. The last 15 kilometers can only be accessed by dirt paths either using a motorcycle or hiking. Though remote, the hamlet has an elementary school and 10 warungs for 207 families with a total of 740 people. A year ago, this community contacted the district health office (DHO) to get help. Goiter is a common problem in the village, and the results from the thyroid palpitation of all the school children were alarming - 17% had grade 1 goiter. The PD Network organized a Positive Deviance study. In use of iodised salt, they found some interesting findings:
1) Many of the PD families put a small dish of salt on the table for each meal, as instructed by the Prophet Mohammad – “begin a meal by dipping your finger in the salt dish and a prayer of thanks”.
