Hypothyroidism

What is hypothyroidism?
Hypothyroidism refers to any state in which thyroid hormone production is below normal. There are many disorders that result in hypothyroidism. These disorders may directly or indirectly involve the thyroid gland. Because thyroid hormone affects growth, development, and many cellular processes, inadequate thyroid hormone has widespread consequences for the body.
This article will focus specifically on hypothyroidism in adults. What are thyroid hormones?
Thyroid hormones are produced by the thyroid gland. This gland is located in the lower part of the neck, below the Adam's apple. The gland wraps around the windpipe (trachea) and has a shape that is similar to a butterfly - formed by two wings (lobes) and attached by a middle part (isthmus).
The thyroid gland uses iodine (mostly available from the diet in foods such as seafood, bread, and salt) to produce thyroid hormones. The two most important thyroid hormones are thyroxine (T4) and triiodothyronine (T3), which account for 99.9% and 0.1% of thyroid hormones present in the blood respectively. However, the hormone with the most biological activity is T3. Once released from the thyroid gland into the blood, a large amount of T4 is converted into T3 - the active hormone that affects the metabolism of cells.

Goiter
Definition
Goiter refers to any visible enlargement of the thyroid gland. Description
The thyroid gland sits astride the trachea (windpipe) and is shaped like a butterfly. It makes thyroxin, a hormone that regulates the metabolic activity of the body, rather like the gas pedal on a car. Too much thyroxin increases the metabolism, causing weight loss, temperature elevation, nervousness, and irritability. Too little thyroxin slows the metabolism down, deepens the voice, causes weight gain and water retention, and retards growth and mental development in children. Both conditions also alter hair and skin growth, bowel function, and menstrual flow. Curiously, the thyroid gland is often enlarged whether it is making too much hormone, too little, or sometimes even when it is functioning normally. The thyroid is controlled by the pituitary gland, which secretes thyroid stimulating hormone (TSH) in response to the amount of thyroxin it finds in the blood. TSH increases the amount of thyroxin secreted by the thyroid and also causes the thyroid gland to grow.
  • Hyperthyroid goiterIf the amount of stimulating hormone is excessive, the thyroid will both enlarge and secrete too much thyroxin. The result

Goiter
Goiter refers to enlargement of your thyroid gland. If the whole gland is enlarged it's unlikely to be due to cancer, which usually starts as a lump within the gland. A general enlargement probably means that your thyroid is becoming overactive (hyperthyroidism) or slowing down (hypothyroidism). Related Articles Goiter usually begins as swelling in the lower front of your neck and is a sign that you should visit your doctor for a thyroid check-up to see if yours is malfunctioning. Most goiter caused by a change in thyroid function will be smooth and non-tender except for situations in which the gland has become mildly inflamed (sub-acute thyroiditis). Sudden appearance of a lump in your thyroid A sudden painful swelling can signal a hemorrhage into a nodule or fluid-filled thyroid cyst. Since it may interfere with breathing or swallowing, it's a good idea to call your doctor immediately for advice that's best for you in your particular situation.

Goiter
Goiter and Iodine Deficiency
One fifth of the world's population (1 billion people) have diets that are deficient in iodine. People in these areas commonly have thyroid enlargement (goiter), and often thyroid nodules as well. Related Articles In European countries and many other developed countries iodine intake has only risen to normal levels in recent years. Here in America, many in the population have thyroid enlargement and nodules as a result of iodine deficiency in the past. In the United States, dietary iodine deficiency was eliminated early in the last century with the addition of iodine to salt and its presence in milk and bread products. And even though dietary iodine is dropping in some parts of the country to borderline and even slightly low levels, goiters here are not due to a lack of iodine. Therefore if you have an enlargement of your thyroid gland and have not moved to the United States from a country with iodine deficiency, you need to look for another cause for your problem.

If your doctor has diagnosed you as having a thyroid nodule, a thyroid lump or an enlargement of the thyroid known as goiter, you'll want to know more about the diagnosis and treatment process, the relationship between nodules to thyroid cancer, and followup.
    What is a Thyroid Nodule?
Thyroid nodules are very common. A nodule is a swelling or lump, which can be a solid or liquid filled cyst or mass. Most are benign, but a small percentage can be cancerous. So you should always have a nodule evaluated by your physician as soon as you notice it.
Symptoms of a nodule can be varied. Some people have hyperthyroidism symptoms such as palpitations, insomnia, weight loss, anxiety, and tremors and others have hypothyroidism symptoms weight gain, fatigue, depression. Some will cycle back and forth between hyperthyroid and hypothyroid symptoms. Some people mainly have difficulty swallowing, a feeling of fullness, pain or pressure in the neck, a hoarse voice, or neck tenderness. And finally, many people have nodules wiht no obvious symptoms related to thyroid dysfunction at all.
If you have a thyroid nodule, don't be very worried that it is cancerous. Only 5% of nodules are cancerous, and most forms of thyroid cancers are highly treatable and curable.

    Goiter
    Definition
A goiter is an enlargement of the thyroid gland. It is not cancer.
Causes There are different kinds of goiters. A simple goiter usually occurs when the thyroid gland is not able to produce enough thyroid hormone to meet the body's needs. The thyroid gland makes up for this lack by enlarging, which usually overcomes mild deficiencies of thyroid hormone. A simple goiter may be classified as either an endemic (colloid) goiter or a sporadic (nontoxic) goiter. Endemic goiters occur within groups of people living in geographical areas with iodine-depleted soil, usually regions away from the sea coast. People in these communities might not get enough iodine in their diet (iodine is vital to the formation of thyroid hormone). The modern use of iodized table salt in the U.S. prevents this deficiency. However, inadequate iodine is still common in central Asia and central Africa. In most cases of sporadic goiter the cause is unknown. Occasionally, certain medications such as lithium or aminoglutethimide can cause a nontoxic goiter.

    Definition
Toxic nodular goiter involves an enlarged thyroid gland that contains a small rounded mass or masses called nodules, which produce too much thyroid hormone.
    Alternative Names
Toxic adenoma; Toxic multinodular goiter; Plummer's disease
    Causes
Toxic nodular goiter arises from a long-standing simple goiter and occurs most often in the elderly. Symptoms are those of hyperthyroidism , but the protruding eyeballs seen in Graves' disease do not occur. Risk factors include being female and over 60 years old. This disorder is never seen in children.
