    Suspicion of somatoform disorder in undiagnosed tabes dorsalis

    Loyola University Medical Center, Hines, Illinois. We present a case of tabes dorsalis, which consisted of intermittent, sharp pains and diffuse neurological abnormalities, and was initially considered to be a somatoform disorder. The unusual behavioural presentations of neurosyphilis may lead to premature psychiatric diagnoses. It is thus important that psychiatric consultants be aware of the myriad manifestations of the disease.

    Tabes dorsalis: Slowly progressive degeneration of the spinal cord that occurs in the tertiary (third) phase of syphilis a decade or more after originally contracting the infection. Among the terrible features of tabes dorsalis are lancinating lightning-like pain, ataxia (wobbliness), deterioration of the nerves to the eyes (the optic nerves) leading to blindness, urinary incontinence, loss of the sense of position, and degeneration of the joints (Charcot's joints). Tabes is the Latin word for decay. The term tabes dorsalis was devised in 1836 when the cause of the condition was thought to be wastage of the dorsal (posterior) columns of the spinal cord, well before it was recognized as part of late syphilis. Douglas Moray Cooper Lamb Argyll Robertson was born in 1837 in Edinburgh, where his father, John Argyll Robertson, was a general surgeon, with a special interest in surgery of the eye. He received his early education in Edinburgh, and then studied medicine at St. Andrews, from where he graduated in 1857, the year of his father’s death. He returned to Edinburgh to work as a house surgeon at the Royal Infirmary before departing for Berlin to study under the leading ophthalmologist of the day, Albrecht von Graefe (1828-1870). In this he was following on the work of his father who had been a lecturer in surgery at Edinburgh in the extra-mural school and had confined himself chiefly to ophthalmic surgery. During the 1850s, the invention of the ophthalmoscope by Hermann Ludwig Ferdinand von Helmholtz (1821-1894) in 1851, and the new operations such as iridectomy introduced by von Graefe in 1856 ensured that ophthalmology developed into a specialty of its own right.

    Peer Review Status: Internally Peer Reviewed Syphilis, the only important spirochaetal infection of the brain, is caused by the organism Treponema pallidum . Infection of the CNS occurs in the context of tertiary syphilis one or more years following primary infection. Classically, there are three recognized types of CNS syphilis: meningovascular, parenchymatous, and tabes dorsalis. Because the parenchymatous form merely represents cortical extension of meningovascular syphilis, both types will be considered together. Parenchymatous syphilis was formerly called general paresis of the insane. This gross photo shows the findings typical of meningovascular-parenchymatous syphilis - thickening of the meninges at the dorsum of the brain and generalized cerebral atrophy. The microscopic findings in meningovascular-parenchymatous syphilis consist of meningeal fibrosis, cortical atrophy, proliferation of microglial cells, and iron deposition throughout the cortex. Iron deposits are brought out on this slide through the use of an iron-hematoxylin stain which stains the iron blue. The demonstration of the spirochaetal organisms is extemely difficult in the CNS regardless of the form of involvement. The only hope of demonstrating the organisms is with the use of special stains.     The high-profile tour did not conceal that the author was ill. She spent part of her three months in the United States on intravenous infusions. At this time of her life, three years before her death, she weighed less than eighty pounds and ate almost nothing. Her hosts offered oysters, grapes, and champagnefoods she could keep downbut even these she ignored in favor of entertaining the assembled guests. Through a variety of hints, confidences, and declarations, she told intimates that she suffered from syphilis. Karen Blixen does not mention her illness in her lyrical memoir Out of Africa . Her book offers her impressions of Africa and the friends she made therenot insights into her private life. Denys Finch Hatton is a friend, not her lover, in her story. Karen Blixen went to Africa to marry her second cousin Bror Blixen and gain the title of Baroness. Her husband was generous with his affections, and his infidelities resulted in her syphilis in their first year of marriage. Bror Blixen recovered without medications, but Karen Blixen's more serious symptoms required that she take mercury and arsenicthe only medicines used to treat the disease at that time.
