He spoke to several of his contemporaries about this project, but the book never became more than a collection of brief notes, which were collected and published posthumously as "La Doulou" (Provencal for "pain"). This short book, translated here by Julian Barnes, consists of "fifty or so pages of notes on his symptoms and sufferings, his fears and reflections, and on the strange social life of patients at shower-bath and spa." (p. xiii) The notes are generally in chronological order, beginning with short comments like "Torture walking back from the baths via the Champs-Elysées" (p. 4) and "Also from that time onwards pins and needles in the feet, burning feelings, hypersensitivity." (p. 6) In a long section toward the end Daudet comments on his experience at Lamalou, a thermal spa that he visited annually from 1885 to 1893. "I've passed the stage where illness brings any advantage or helps you understand things; also the stage where it sours your life, puts a harshness in your voice, makes every cogwheel shriek." (p. 65)

Pathogenesis
Pathology
In the early stages, a lymphocytic and mononuclear infiltrate is seen in the meninges. These inflammatory reactions may involve the cranial nerves and can cause degeneration of the axons. When the inflammation involves small meningeal vessels, proliferation of the endothelial lining occurs, resulting in vascular compromise and infarction of brain and spinal cord tissue. Similar findings are seen in tabes dorsalis where the inflammation of meninges and blood vessels is followed by degeneration of the posterior roots and posterior fiber columns of the spinal cord and, sometimes changes are seen in the cranial nerves. This explains the multiple symptoms seen in this disease.
The pathological hallmark of the disease is inflammation of the meninges and the nerve roots. The lower spinal cord roots are most commonly involved. There is atrophy and loss of myelinated fibers in the posterior columns, secondary to root pathology. In late untreated cases the inflammation spreads to the anterior roots and atrophy may then be seen in the distribution of the affected roots.
Clinical Features
The disease is sometimes called progressive locomotor ataxia. The varied clinical features represent the extent of involvement of the brain, cranial nerves and spinal cord. The initial symptoms consist of diplopia due to paralysis of the 3rd, 4th, or 6th cranial nerves, irregular pupils, paresthesia and hyperesthesia. Pupillary abnormalities are present in 90% of patients. The characteristic pupillary abnormality, which is seen 50 percent of the time, is the

He received this honor when he was not yet 42 years old. Indeed, Kipling's career is remarkable for its precocious success. His collection of verse Departmental Ditties was published when he was 20 years old. When he first went to England in 1889, he was already a well-known writer. In his short story Love-o'-Women, published in 1893 in the collection Many Inventions, Kipling gives a clinically accurate description of tabes dorsalis and what is probably the only literary description of Romberg's test. Love-o'-Women was published when Kipling was 27, a year after he had married Caroline Balestier, an American woman, and moved to Dummerston, Vermont. Kipling remained in Vermont for 4 years, during which time he wrote The Jungle Book, The Second Jungle Book, Captains Courageous, and The Seven Seas.

In the AIDS population up to 13% of patients are positive to cerebrospinal fluid (CSF) Veneral Disease Research Laboratory (VDRL) tests. Without therapy, 510% of patients develop clinical evidence of neurosyphilis. The CNS manifestations of syphilis can be broadly classified into meningeal and parenchymal. Meningeal manifestations range from the acute and more subtle and chronic forms of meningitis and meningoencephalitis up to the formation of circumscribed masses. These gummas are composed, similarly to those in all other sites in the body, of granulation tissue surrounded by mononuclear epithelial and fibroblastic cells and, intracranially, usually located over the cerebral convexities, adherent to both dura and brain parenchyma. Parenchymal manifestations include two types of vasculitis, respectively Heubner's and Nissl's endarteritis, the former affecting large and medium-sized arteries with resultant irregular luminal narrowing and ectasia, the latter primarily involving small vessels in which a luminal narrowing occurs as a consequence of intense proliferation of endothelial and adventitial cells. Vascular neurosyphilis may present with focal neurological deficits as a consequence of arterial occlusion. For so-called meningovascular syphilis the usual interval of infection to symptom onset in the general population is 5 to 10 years.
