Glaucoma: A group of disorders characterized by progressive damage to the eye at least partly due to intraocular pressure.
Incidence
Glaucoma is the second most common cause of blindness in the USA. About 2 million Americans have glaucoma, but roughly half are unaware of it. Although glaucoma is generally considered a disorder of the elderly, it can occur in any age group. Of those with ocular hypertension, only about 1% per year will develop glaucoma.
Pathogenesis
Glaucoma can be described according to the mechanism of outflow obstruction as either open-angle or closed-angle (angle-closure) glaucoma. Alternatively, classification can be based on etiology as primary or secondary (see Table 100-1
Diagnosis
A comprehensive ophthalmic examination is essential for diagnosis and prompt treatment. Examination includes visualization of the angle by a special prism or contact lens (gonioscopy), IOP measurement, visual field examination and, most importantly, examination of the optic disks. A screening test for glaucoma based only on IOP has low sensitivity, specificity, and positive predictive value. Patients who are at risk for glaucoma should be referred to an ophthalmologist for a comprehensive ophthalmic examination. Visualization of the anterior chamber angle allows differentiation of angle-closure from open-angle glaucoma. However, because the overlying sclera and limbus are opaque, the anterior chamber angle can only be visualized using gonioscopy.

Overview Glaucoma is a disease caused by increased intraocular pressure (IOP) resulting either from a malformation or malfunction of the eye’s drainage structures . Left untreated, an elevated IOP causes irreversible damage the optic nerve and retinal fibers resulting in a progressive, permanent loss of vision. However, early detection and treatment can slow, or even halt the progression of the disease. What causes glaucoma? The eye constantly produces aqueous , the clear fluid that fills the anterior chamber (the space between the cornea and iris ). The aqueous filters out of the anterior chamber through a complex drainage system. The delicate balance between the production and drainage of aqueous determines the eye’s intraocular pressure (IOP). Most people’s IOPs fall between 8 and 21. However, some eyes can tolerate higher pressures than others. That’s why it may be normal for one person to have a higher pressure than another. Common types of glaucoma Open Angle Open angle (also called chronic open angle or primary open angle) is the most common type of glaucoma. With this type, even though the anterior structures of the eye appear normal, aqueous fluid builds within the anterior chamber, causing the IOP to become elevated. This may occur secondary to inflammation with secondary fibrotic changes. Another common cause of secondary angle closure glaucoma is neovascularization of the iris surface or rubeosis iridis. Histopathologically, the iris can be seen adherent to the posterior surface of the cornea, thus closing off the trabecular meshwork (#21985, #21987). In cases of chronic inflammation, the angle is closed off by a proliferation of fibrous tissue. Symptoms to look for are increased pressure, need for new glasses, loss of side vision and blank spots in your vision. Laser surgery helps fluid drain out of the eye. Although your eye care professional may suggest laser surgery at any time, it is often done after trying treatment with medicines. In many cases, you will need to keep taking glaucoma drugs even after laser surgery. Laser surgery is performed in a doctor's office or eye clinic. Before the surgery, your doctor will apply drops to numb the eye. As you sit facing the laser machine, your surgeon will hold a special lens to your eye. A high-energy beam of light is aimed at the lens and reflected onto the meshwork inside your eye. You may see flashes of bright green or red light. The laser makes 50-100 evenly spaced burns. These burns stretch the drainage holes in the meshwork. This helps to open the holes and lets fluid drain better through them. Your surgeon will check your eye pressure shortly afterward. He or she may also give you some drops to take home for any soreness or swelling inside the eye. You will need to make several follow-up visits to have your pressure monitored.

Web posted at: 6:39 p.m. EDT (2239 GMT) AUGUSTA, Georgia (CNN) A new study that says smoking marijuana is a hazy and impractical way to treat glaucoma is the latest twist to the medical marijuana debate. The study, by ophthalmologist Keith Green at the Medical College of Georgia, found the medical benefits of smoking marijuana are slight and relief is temporary. "Glaucoma is a 24-hour-a-day disease, 365 days-a-year disease and you cannot get away from it," he said. The battle over legalizing marijuana for medical purposes has been smoldering for over 30 years. But the issue has been recently fueled by voters in five states Alaska, Arizona, Oregon, Nevada and Washington who passed laws legalizing the drug to ease the symptoms of certain diseases such as glaucoma, AIDS and cancer. Glaucoma is a chronic eye disease that can lead to blindness if left untreated. It is caused by an abnormal increase of fluid pressure in the eye which leads to the gradual loss of vision.
