This nerve disorder should be suspected in all patients with type 2 diabetes and in patients who have had type 1 diabetes for more than five years. In some instances, patients with diabetic neuropathy have few complaints, but their physical examination reveals mild to moderately severe sensory loss. Idiopathic neuropathy has been found to precede the onset of type 2 diabetes or to occur as an early finding in the disease. Classification of Diabetic Neuropathy The primary types of diabetic neuropathy are sensorimotor and autonomic ( Table 1 ). A patient may have only one type of neuropathy or might develop different combinations of neuropathies.

How does diabetes affect my body? Diabetes makes your blood sugar level higher than normal. High blood sugar levels can damage the nerves in your body. When diabetes damages the nerves, it's called diabetic neuropathy. What is diabetic neuropathy? Diabetic neuropathy makes your nerves less effective, so they can't carry messages to the brain and other parts of your body. Diabetic neuropathy can affect the following:
  • Your ability to feel sensation in different parts of your body (especially your feet). The ability of your heart to keep up with the needs of your body. The ability of your intestines to digest food.

    Central Nervous System People with diabetes commonly develop temporary or permanent damage to nerve tissue. Nerve injuries are caused by decreased blood flow and high blood-sugar levels , and are more likely to develop if blood-glucose levels are poorly controlled. Some diabetics will not develop nerve damage, while others may develop this condition relatively early. On average, the onset of symptoms occurs 10 to 20 years after diabetes has been diagnosed.     Diabetic neuropathy almost always starts in the feet because these are the longest nerves in the body. They are also fed by the longest blood vessels in the body. The peripheral neuropathy that is characteristic of diabetes is found only in the presence of high blood sugar levels. Neuropathy represents the most common complication of diabetes. Neuropathy appears to be caused by damage to small blood vessels. Nerves depend on multiple tiny vessels that carry nutrients and oxygen to keep each and every segment of these very long nerves intact. Damage to one small segment can result in loss of feeling, pain or burning sensations that bother the feel and legs.

    A neuropathy is a condition in which the peripheral nerves (the nerves in your body, aside from your spinal cord and brain) are damaged or not working correctly. There are hundreds of different types of neuropathies and many different ways to categorize them, including by the type of nerve damaged, the causes of the nerve damage, or the pattern of nerve damage.
    A polyneuropathy is a neuropathy pattern, whereby the nerve damage initially starts in both feet and may progress to involve the feet, calves, and fingers/hands. Another word for this pattern is a Stocking and Glove Neuropathy . Many patients with polyneuropathy may not even have any symptoms; in this case the diagnosis is made by a physical examination or a laboratory test (electromyography (EMG) and nerve conduction velocity test (NCV)). Some patients with polyneuropathy have only numbness, "tingling," and/or "pins and needles." Less often, some unlucky patients with polyneuropathy experience pain.

    Symptoms of neuropathy include numbness and sometimes pain in the hands, feet, or legs. Nerve damage caused by diabetes can also lead to problems with internal organs such as the digestive tract, heart, and sexual organs, causing indigestion, diarrhea or constipation, dizziness, bladder infections, and impotence. In some cases, neuropathy can flare up suddenly, causing weakness and weight loss. Depression may follow. While some treatments are available, a great deal of research is still needed to understand how diabetes affects the nerves and to find more effective treatments for this complication. Can Diabetic Neuropathy Be Prevented?
    People with diabetes can develop nerve problems at any time. Significant clinical neuropathy can develop within the first 10 years after diagnosis of diabetes and the risk of developing neuropathy increases the longer a person has diabetes. Some recent studies have reported that: 60 percent of patients with diabetes have some form of neuropathy, but in most cases (30 to 40 percent), there are no symptoms. 30 to 40 percent of patients with diabetes have symptoms suggesting neuropathy, compared with 10 percent of people without diabetes. Diabetic neuropathy appears to be more common in smokers, people over 40 years of age, and those who have had problems controlling their blood glucose levels. The symptoms of diabetic neuropathy vary. Numbness and tingling in feet are often the first sign. Some people notice no symptoms, while others are severely disabled. Neuropathy may cause both pain and insensitivity to pain in the same person. Often, symptoms are slight at first, and since most nerve damage occurs over a period of years, mild cases may go unnoticed for a long time. In some people, mainly those afflicted by focal neuropathy, the onset of pain may be sudden and severe.

    Diabetic neuropathy is a common complication of diabetes mellitus in which nerves are damaged as a result of hyperglycemia (high blood sugar levels).

    Causes
    People with diabetes commonly develop temporary or permanent damage to nerve tissue. Nerve injuries are caused by decreased blood flow and high blood-sugar levels , and are more likely to develop if blood-glucose levels are poorly controlled. Some diabetics will not develop nerve damage, while others may develop this condition relatively early. On average, the onset of symptoms occurs 10 to 20 years after diabetes has been diagnosed. Approximately 50% of people with diabetes will eventually develop nerve damage. Peripheral nerve injuries may affect cranial nerves or nerves from the spinal column and their branches.
