Extractions: Plastic Surgery Other conditions in which craniofacial anomalies may occur: Apert's Syndrome - The head usually appears short in the front and back and may be pointed at the top. Other characteristics may include webbed fingers and toes, widely spaced and protruding eyes, and dental crowding. Carpenter Syndrome - This syndrome typically includes traits such as abnormally short fingers, webbed toes, extra toes, underdeveloped jaw, highly arched palate, widely spaced eyes, and/or low-set, deformed ears. Half of patients with Carpenter syndrome also have heart defects. Cleft Palate / Cleft Lip - The incomplete closure of the lip and/or the roof of the mouth results in this defect. Crouzon's Syndrome - Characterized by abnormalities in the skull and facial bones, this syndrome often causes the skull to be short in the front and the back. Flat cheek bones and a flat nose are also typical of this disorder.

Virtanen, T. Korhonen, J. Fagerholm, J. Viljanto. Pediatrics 1999;103;791-795. Early cranioplasty for scaphocephaly has become routine in most countries. In addition to normalizing the shape of the skull, it has been found to decrease intracranial hypertension. Whether corrective surgery benefits the child's cognitive outcome has been inadequately documented. Eighteen children whose sagittal suture showed premature fusion at birth or soon thereafter were operated on at age 1 week to 7 months. All patients healed without complications and were followed up at regular intervals. At the age of 7.8 and 16.3 years, they were examined to clarify their neurocognitive development and to compare the results with their age-and gender-matched healthy controls. Originally, scaphocephalic children, although operated on, had mild deficiencies in auditory short-term memory and language development when examined with the general comprehension, similarities, and digital scan subscales of the Wechsler Intelligence Scale for Children-Revised. In all other respects, their developmental outcome was equal to that of the controls.

Extractions: Professor Claes Lauritzen in Sweden has pioneered the technique of using a spring for craniofacial reshaping. He has performed over 120 cases using springs. Here in Wellington and Christchurch we have recently performed the first 6 cases in the southern hemisphere. Springs have significant advantages over traditional craniofacial techniques in that there is minimal blood loss, and much faster recovery. However the shape change occurs slowly over several months and it is only effective if performed quite early in life. A second procedure is required to remove the spring.

Extractions: Cephalic disorders are congenital conditions that stem from damage to, or abnormal development of, the budding nervous system . Cephalic is a term that means " head " or "head end of the body." Congenital means the disorder is present at, and usually before, birth Cephalic disorders are not necessarily caused by a single factor, but may be influenced by hereditary or genetic conditions, nutritional deficiencies, or by environmental exposures during pregnancy , such as medication taken by the mother, maternal infection, or exposure to radiation (such disorders are more common in areas of the former Soviet Union affected by nuclear waste disposal problems, such as the area around the Mayak plant in Chelyabinsk Russia .) Some cephalic disorders occur when the cranial sutures (the fibrous joints that connect the bones of the skull ) join prematurely. Most cephalic disorders are caused by a disturbance that occurs very early in the development of the fetal nervous system.

Extractions: A series of 50 consecutive patients with scaphocephaly treated by a new operation are reported, and the results are reviewed in detail. The treatment consists of a broad sagittal craniectomy plus extensive morcellation of both parietal bones and, occasionally, the squamous portion of the occipital bone, without dislodging the bony fragments from their dural attachments. The procedure can be done relatively rapidly (median operative time = 55 min) and with minimal loss of blood (median transfusion = 85 ml). The results are analyzed by several criteria and are considered to be excellent.
