Extractions: BELFAST, NORTHERN IRELAND. Systemic lupus erythematosus (SLE) is a chronic inflammatory disease. It can manifest itself via a photosensitive facial rash, fatigue, anorexia, weight loss, and night sweats and can progress to life-threatening involvement of the heart, lungs, kidneys or central nervous system. Flare-ups of SLE are typically followed by periods of clinical remission. Fish oils and copper have both been found useful in the treatment of other inflammatory diseases, so researchers at the University of Ulster decided to see if supplementation with one or both of these would help alleviate SLE symptoms. Their clinical trial involved 52 SLE patients who were randomly assigned to receive 3 grams/day of fish oil providing 540 mg/day of EPA (eicosapentaenoic acid) and 360 mg/day of DHA (docosahexaenoic acid), 3 mg/day of copper in the form of a copper di-glycinate amino acid complex, both fish oil and copper, or a placebo. The study lasted 24 weeks and participants were assessed at baseline, 6, 12 and 24 weeks. The researchers found that disease activity at 24 weeks, as measured by the SLAM-R score, was significantly less in the groups that had supplemented with fish oil than in the placebo and copper only groups. They conclude that supplementation with fish oil may be effective in favourably modifying the symptomatic disease activity in SLE.

It affects more than 2 million Americans of which 200,000 are juveniles. RA's onset usually occurs between the ages of 20 and 50, but is known to affect older adults too. Women are affected three times as often as men. RA is an immune disorder termed seropositive. This means in many patients with RA the rheumatoid factor (M class) can be identified in their blood (serum). Not everyone with who tests positive for the rheumatoid factor develops the disease. Researchers indicate the disease cannot be inherited but the susceptibility toward development could be, although this has not been proven. The actual cause is unknown. Joints are lined in a membrane called the synovium. In rheumatoid arthritis, the white blood cells, whose normal duty is to provide immunity, turn against the synovial membrane causing inflammation. In turn, inflammation triggers the release of chemicals causing the synovial membrane to thicken. Destructive enzymes in the synovial fluid create a granulated membrane (pannus) that covers the normal joint surface. Over time, these chemicals and enzymes can destroy joint cartilage, bone, tendons, and ligaments. As these structures are compromised, the joint loses it shape and alignment.

How is RA diagnosed? Only your doctor can tell you for sure that you have RA, but the most common methods of diagnosis include an analysis of symptoms, your medical history, lab tests, a physical exam, x-rays, and possibly a test for the rheumatoid factor. The rheumatoid factor is a test that detects an antibody that is found in roughly 80% of people with RA. After 10 years, 50% of people with rheumatoid arthritis in employment no longer work, most losing their jobs in the first 12 months after diagnosis.1 Excessive amounts of the pro-inflammatory cytokines, tumour necrosis factor, TNF interleukin-1 (IL-1), and interleukin-6 (IL-6), mediate most of the pathogenic features of rheumatoid arthritis. Infliximab, the chimeric antibody to tumour necrosis factor , and etanercept, a fusion protein p75 tumour necrosis factor receptor immunoglobulin, have been shown to be very effective in reducing the chronic symptoms and signs of rheumatoid arthritis in patients who fail to respond to conventional treatment with disease modifying drugs.2 Both these molecules produce response rates which are at least as high as those seen with other treatments given for milder disease. Importantly, these drugs have been shown to be effective in patients who were thought to be resistant to all treatment. Before these new drugs such patients were left to deteriorate, resulting in cachexic individuals with destroyed joints, a picture all too familiar to physicians.

Objectives: To perform a systematic review on the effectiveness of balneotherapy for rheumatoid arthritis. Search strategy: Using the Cochrane search strategy, studies were found by screening: 1) The MEDLINE CD-ROM database from 1966 to June 2002 and 2) the database from the Cochrane 'Rehabilitation and Related Therapies' Field, the Pedro database up to June 2002.
