Patient Information A patient information handout on screening for prostate cancer , written by the authors of this article, is provided on page 1541. Another handout on management options for prostate cancer , also written by the authors, is provided on page 1545. P rostate cancer is second only to lung cancer as the leading cause of cancer deaths in American men. In 1997, approximately 209,900 new cases of prostate cancer were diagnosed, and more than 41,800 deaths were attributed to this malignancy. At present, chemotherapy and immunotherapy cannot cure prostate cancer once it has spread beyond the gland. Therefore, curative treatment for localized tumors may be the best hope of lowering the mortality rate for prostate cancer. According to this viewpoint, the primary focus of prostate cancer management should be the detection and aggressive treatment of tumors while they are still confined to the prostate.
Extractions: AAFP Home Page Journals Vol. 58/No. 2 (August, 1998) ... Patient Information The American Cancer Society and other national medical organizations emphasize the need for routine screening for prostate cancer in men over the age of 50. The serum prostate-specific antigen (PSA) assay is the test most commonly recommended for the purpose of screening. However, when PSA screening is examined critically from the standpoint of the principles of screening, evidence from prospective studies to support the routine use of PSA testing is lacking. Data suggest that screening often detects what may be indolent, nonaggressive prostate cancer. The treatment of such a cancer with radiation or radical prostatectomy can result in significant morbidity, including urinary incontinence and impotence, without a proven decrease in mortality. Evidence from randomized clinical trials in support of routine PSA screening is urgently needed. T he purpose of screening tests is to identify disease in asymptomatic persons at a stage when treatment will alter the natural history of the condition. There is now a widespread call to incorporate routine prostate-specific antigen (PSA) screening for prostate cancer into the care of men over age 50. However, little prospective evidence shows that the benefits of such testing outweigh the risks. The ideal way to assess the efficacy of screening is with prospective randomized clinical trials; unfortunately, no such trial of prostate cancer screening has been conducted.

Extractions: defaultStatus="All Seekers"; This web site, Allseekers.com, evolved from an earlier site, Sophrony.com, which was first made available on the Internet early in 2000. In Sophrony.com, we attempted to show how Guy Leekley had been applying the teachings of the Tao Te Ching to his own inner struggles with alcohol abuse and other obsessions. He believed that over many years, hormone imbalances in his body brought on by these extremes had contributed to a diagnosis of prostate cancer at the end of 1990. We tried to show in Sophrony.com how during that decade from 1990-2000, as Guy was working through his translation of the Tao Te Ching, its teachings helped him to deal with his various addictions, obsessions and delusions. This spiritual text provided guidance for living in a manner that has helped to keep the cancer in remission. With the publication of his Tao Te Ching A New Version for All Seekers , Guy feels that now it is time to retire Sophrony.com and use Allseekers.com to show how we can all learn from the teachings of the Tao Te Ching We coined the term Sophrony because there did not seem to be a word in English that clearly expresses the view of how we should live that one finds in the Tao Te Ching . The closest that Guy could find is the Greek word, sophrosyne, an ideal of the ancient Greeks that has no equivalent in English. It meant the opposite of arrogance and insolence, learning to live gracefully within our natural limitations, and following the inner laws of harmony and proportion.

All RSS Feeds Reuters Wednesday, July 6, 2005; Page A03 A screening test for prostate cancer taken by millions of men every year is not terribly accurate, and even the best result does not ensure that a man is cancer-free, experts said yesterday. Researchers found that the standard prostate-specific antigen, or PSA, test produces many false positives and false negatives meaning some men who think they do not have cancer actually do, while others may undergo uncomfortable biopsies only to learn there is no tumor after all. "Patients have assumed, 'My PSA is below 4. It's normal. I have no risk,' " lead author Ian Thompson of the University of Texas Health Science Center at San Antonio said at a news conference. Usually, a biopsy is not recommended unless the PSA level exceeds 4 nanograms per milliliter.
