     Extractions: The World Health Organization recommended that a pertussis incidence of <1 case per 100,000 population be achieved in Europe by 2000. Available data indicate that this goal has generally not been achieved, and the incidence is actually rising in some countries. Understanding the reasons for this increased incidence may lead to better global control of pertussis. In the majority of countries where pertussis is a notifiable disease, a case-based national surveillance system is in place. However, different case definitions, methods of diagnosis and reporting and surveillance systems make direct intercountry comparisons difficult, and pertussis is not a statutory notifiable disease in every country. Nevertheless the general consensus is that reported incidences are probably considerably lower than the actual incidence of pertussis; underreporting is common. Prolonged cough may be the only clinical feature in adolescents or adults, who may present for diagnosis late (precluding laboratory confirmation) or not at all. When they do present, their condition is often misdiagnosed because, in part, clinicians continue to perceive pertussis as a childhood disease.

It is spread by droplets and direct contact and is highly contagious. Each year, about 286 000 people, almost all young infants, are killed by the disease worldwide, and at least 45 million suffer from the illness with prolonged and exhausting bouts of coughing that may continue for up to 3 months. Some children also suffer seizures and neurological damage. In recent years there have been reported increases in the incidence of pertussis, including in several industrialized countries. Immunity to infection is not permanent, even after infection with B. pertussis. This partly explains the number of cases observed in adolescents and adults in some countries. Vaccines based on whole killed B. pertussis bacteria (known as "whole-cell vaccines") have been used effectively and safely for several decades to prevent infection.

Index Pertussis is a respiratory infection caused by bacteria called Bordetella pertussis. It is also called whooping cough because of the characteristic sound of the cough it causes. The illness usually begins with a runny nose, mild cough, and pink eyes that last about a week. Then an increasingly severe cough develops that can last 2 to 4 weeks. The cough usually comes in spasms and ends with a high-pitched whoop. Often the coughing causes a child to vomit or his or her face to turn red or blue. During the coughing phase, children with whooping cough do not have a fever. In infants, whooping cough is a very serious illness and may require them to be hospitalized. What is the treatment?
