In these diseases, inflammatory cells surround, invade and destroy normal muscle fibers as if they were defective or foreign to the body. In rare cases, inflammation of the heart, lungs, intestines and skin can occur. Types of myositis include polymyositis, in which many muscles become inflamed, and dermatomyositis, in which the muscles and skin become inflamed. Symptoms All forms of myositis involve chronic muscle inflammation. This almost always results in muscle weakness, frequent falling, swollen feet and legs and muscular and joint pain. Early signs of the condition include: The muscle and skin inflammation associated with dermatomyositis can cause a distinctive, patchy rash on the eyelids, cheeks, nose, back or upper chest, elbows, knees and knuckles. Sometimes hardened bumps develop under the skin. The rash is often followed by muscle weakness. Causes and Risk Factors The cause of myositis is unknown. These diseases can affect people of any age. Most children who get the disease are between five and 15 years old, and most adults are 30 to 60 years old. Women are more likely to get the myositis than men.

Myositis is an inflammatory muscle disease that can damage muscles and other organs, resulting in significant disability. Children or adults with polymyositis or dermatomyositis or a related condition with onset of these diseases during childhood may be evaluated under this study. Healthy children or adults will also be enrolled as "controls," for comparison of test results. All patients will undergo a complete history (including completing some questionnaires) and physical examination, review of medical records, and blood and urine tests. Patients may then choose to participate in an additional 3- to 5-day evaluation, which will include some or all of the following diagnostic, treatment or research procedures: 1. Standardized muscle strength testing, range of motion of joints and walking (gait) analysis by a physiotherapist; completion of a questionnaire regarding ability to perform daily tasks

Patients may become unable to perform daily living activities and be confined to wheelchairs. s-IBM is thought to be an autoimmune disease, in which the body's own immune system attacks healthy muscles. This study will explore the causes of the muscle tissue inflammation that is responsible for destruction of muscle fibers and weakness in this disease. Information from the study may help in the development of an effective treatment for this disease. Patients with s-IBM may be eligible for this study. Those who are unable to travel or who have severe cardiovascular, renal or other end-stage organ disease will be excluded. Candidates will be screened for eligibility with a medical history and physical and neurological examinations.

Perhaps he will not open his mouth at all. In time, the muscles around the head (particularly the temple region) hollow out giving the dog a thin faced look. What is happening? There are plenty of possibilities but here is one that must be ruled out. What is “Trismus?” In short, trismus is the inability to open the mouth. Regardless of whether or not the act is painful to attempt, the mouth simply cannot be opened. Of course, a dog with a painful mouth may be unwilling to open his jaws and there is no way to ask the dog if simply hurts to open the mouth or if it is not possible to open the mouth. Here are the possible explanations: A foreign body may be stuck in the soft tissues of the mouth Masticatory Myositis Sorting these out will require general anesthesia to get the mouth open and check for painful oral conditions (broken teeth, oral foreign body, growths inside the mouth), and radiographs to assess the tempero-mandibular (jaw) joints and jaw bones themselves. General blood tests are also drawn including a special test for Masticatory Myositis (the “2M antibody” test). In more advanced cases of Masticatory myositis the patient’s jaws will not open even under general anesthesia.

The disorder presents as slow progressive weakness and withering away (atrophy) of the muscles (myositis), especially of the arms and legs. IBM is characterized by the gradual onset (over months or years) of muscle fatigue and weakness; a clear tendency to strike men more frequently than women; and affecting both the muscles closest to the body’s trunk (proximal) and those farthest from the trunk (distal). Onset is usually after age 50, although it may occur earlier.

The infection typically spreads from a discrete focus of clostridium within the body. The original source can actually be within the body, as clostridium bacteria normally live in the gastrointestinal tract. Alternatively, the infection can originate outside the body, for example, when an infection results from contamination of wounds during trauma such as a motor vehicle accident. Gas gangrene infection is severe and can advance quickly. Besides replicating and migrating, the organisms which cause gas gangrene produce poisons known as exotoxins. Exotoxins are capable of liquefying adjacent tissue and inhibiting local defense mechanisms which might normally contain a less virulent infection. As such, the advancing infection of gas gangrene may simply destroy healthy tissue in its path and spread over the course of hours. Clostridium bacteria are “anaerobic,” meaning that they prefer low oxygen concentrations to grow. If clostridium bacteria are exposed to high amounts of oxygen, their replication, migration and exotoxin production can be inhibited. This is why hyperbaric oxygen is used in the treatment of gas gangrene. Repeated treatment in the hyperbaric chamber has the potential to slow the progress of the infection while the two primary therapies, antibiotics and surgical removal of the infected tissue, control it.
