When I firsted started this site about a year and a half ago, I came up with the name ("The People's Republic of Seabrook") as an homage to the contentious nature of local politics. Seabrook may be a small town, but at times it seems, politically speaking, a smaller version of San Francisco. The late Herb Caen used to refer to San Francisco as "Baghdad by the Bay". At times you could say the same about Seabrook. The latest episode in our on-going political drama involves Seabrook City Councilwoman Margaret Kidd Duncan. Of course, I should begin by making my own personal bias clear. From where I sit, Duncan has proven herself to be a self-superior, inflexible, insensitive, racist, and intolerant troll, completely devoid of class and dignity. Other than that, she may well have many wonderful qualities, but if so they're well-disguised. Being a member of the Seabrook City Council does not exactly confer one with royal status. Duncan, however, seems not to have grasped this reality. Her term in office has been marked by anger, utter disdain for who elected her, and a complete inability to stomach those who disagree with, or worse, do not defer to her.

There is some variation in what has been described since in what has been called Meige's syndrome, but in all descriptions there is blinking and chin thrusting. Some patients have lip pursing or tongue movements and, for a few, the movements spread into the shoulders. The cause remains obscure, treatment less than ideal, and frustration is a major factor as it is with blepharospasm. Many physicians, even regarding the more common essential blepharospasm, may have had no experience with Meige's syndrome and the patient may be told the process is psychologic. It is not. There are some similar conditions related to medication effects, for example, the mouth movement seen when excessive levodopa is present in patients with Parkinson's disease. Tardive dyskinesia, the movements that can follow prolonged use of major tranquilizers, can be associated with mouth and tongue movements. Some normal, elderly individuals develop spontaneous mouthing or chewing movements. Meige is more than and different from any of these other mouth movements. Blinking forcefully is often a part of it as is grimacing and the chin thrusting forward. At times there is a joint interactive movement between the oral movements and the eye movements. The patients are more likely to be women than men and usually at middle age or beyond. There is, as in all neurologic conditions, some variation with stress, but the movement is present at rest and with activity and when with others or alone. As with almost all movement disorders, it disappears in sleep.

The term Meige's syndrome II/Brueghel's syndrome is used for blepharospasm and oromandibular dystonia occurring together. It is a disabling spasm of the facial musculature consisting of primary blepharospasm followed by abnormal facial movement. Squinting may begin unilaterally but soon becomes bilateral. In time, the lower facial muscles becomes involved with yawning, jaw opening, and abnormal tongue movements. The voice is often affected as well. The involuntary movements cease during sleep. The condition may be aggravated by eating or talking and is sometimes lessened by humming, singing, yawning or voluntary opening of the mouth.

The combination of upper and lower dystonia is sometimes called cranial-cervical dystonia. When oromandibular is combined with blepharospasm, it may be referred to as Meige's Syndrome named after Henry Meige, the French neurologist who first described the symptoms in detail in 1910. The symptoms usually begin between the ages of 40 and 70 years old and appear to be more common in women than in men. Oromandibular dystonia may be a continuous disorder that persists even during sleep, or it may be task-specific, occurring only during activities such as speaking or chewing. Difficulty in swallowing is a common aspect of oromandibular dystonia if the jaw if affected, and spasms in the tongue can also make it difficult to swallow.
