Lymphedema is the buildup of lymph (a fluid that helps fight infection and disease) in the fatty tissues just under the skin. The buildup of lymph causes swelling in specific areas of the body, usually an arm or leg, with an abnormally large amount of tissue proteins, chronic inflammation, and thickening and scarring of tissue under the skin. Lymphedema is a common complication of cancer and cancer treatment and can result in long-term physical, psychological, and social issues for patients. Extractions: Add your email address for updates on groups, lectures and Malecare events Lymphedema is an accumulation of lymphatic fluid in the interstitial tissue that causes swelling, most often in the arm(s) and/or leg(s), and occasionally in other parts of the body. Lymphedema can develop when lymphatic vessels are missing or impaired (primary), or when lymph vessels are damaged or lymph nodes removed (secondary). When the impairment becomes so great that the lymphatic fluid exceeds the lymphatic transport capacity, an abnormal amount of protein-rich fluid collects in the tissues of the affected area. Left untreated, this stagnant, protein-rich fluid not only causes tissue channels to increase in size and number, but also reduces oxygen availability in the transport system, interferes with wound healing, and provides a culture medium for bacteria that can result in lymphangitis (infection). Lymphedema should not be confused with edema resulting from venous insufficiency, which is not lymph-edema. However, untreated venous insufficiency can progress into a combined venous/lymphatic disorder which is treated in the same way as lymphedema.

Thompson announced today that Medicare will expedite coverage of pneumatic compression pumps to make it easier for Medicare beneficiaries with lymphedema to take advantage of the technology. Secretary Thompson announced the expanded Medicare coverage while working at the Baltimore offices of the Health Care Financing Administration (HCFA). The Secretary moved HHS headquarters to the Health Care Financing Administration's offices in Baltimore this week to give HHS and HCFA staff an opportunity to work together to build a stronger HCFA. Medicare's new coverage policy eliminates language that made these compression pumps the treatment "of last resort" for beneficiaries suffering from lymphedema, an accumulation of lymphatic fluid causing abnormal swelling of the arms, legs, breast, neck or head that often develops when lymph nodes are removed during surgery. Breast cancer surgery is the most common cause of the condition in the United States. "It's important to make effective technologies available to Medicare beneficiaries when it helps them the most," Secretary Thompson said. "This coverage decision simplifies Medicare policy to allow older Americans who need these pumps to get them more quickly and easily."

The skin of her legs is fibrotic and lichenified with elephantiasis verrucosa nostra changes. She has acute flares of the stasis dermatitis which produce bullae, erosions and ulcers. These have been treated successfully with Unna's boots. She cannot wear graduated pressure stockings because of the cone-like shape of her legs. I have ordered a Lymphapress pump for her to prevent leg edema/lymphatic edema and hopefully prevent ulceration. Does anyone have experience with these pumps? Is one better than another? Extractions: Annick D. Van den Abbeele, M.D. A 47 y.o. female, status post radiation therapy for pelvic tumor, presented with left lower extremity swelling. Imaging technique depends upon the study indication. Evaluation of extremity lymphedema - 0.05 cc injections of Tc-99m Sulfur Colloid (0.22 micrometer filtered, 0.5 mCi per injection) are performed intradermally. 2 injections are performed on the dorsum of the foot or hand. Injection of the contralateral extremity provides a useful comparison. Patient ambulates (~ 2 flights of stairs) to aid lymphatic return. For lower extremity lymphoscintigraphy, sweeps are performed, leaving the injection sites just out of the field of view, up through the chest, in anterior and posterior projections, at a rate of 8 cm per minute. Full body sweep images are obtained every 30 minutes x three, followed by another set of images at three hours, and then as needed. The end point of a normal exam occurs when the clinical question is answered and the liver is visualized.
