"For patients who have other significant medical problems or little support at home, we would recommend radiation alone. In all cases, patients should be followed closely during treatment by a head and neck surgeon, so that surgery can be performed if there is residual or recurrent cancer after treatment." This year, approximately 9,500 Americans will be diagnosed with laryngeal cancer and 3,800 will die from the disease. Experience with combined treatment, Forastiere adds, has reduced the need for complete removal of the larynx from 100 percent to about 15 percent. Removing the larynx leaves patients unable to speak with their natural voice and typically use speaking aids such as an electronic device. Other previously-studied treatment options included radiation therapy alone or several cycles of chemotherapy followed by radiation. Studies from a decade ago showed that the survival rate of patients treated with chemotherapy followed by radiation was just as good as those receiving surgery. This new study of 547 patients shows that giving chemotherapy and radiation together instead of sequentially is more effective in preserving the voice box. 88 percent of patients receiving chemotherapy and radiation together still had their voice box after two years as compared to 75 percent receiving chemotherapy followed by radiation and 70 percent receiving radiation alone. For each of these three treatment options, overall survival was similar at about 75 percent after two years.

Pressing the tracheal opening with a finger forces exhaled air through the shunt valve to the oesophagus, where soft tissue structures serve as a substitute voice. However, the voice is in many cases of poor quality and the process creates a mental barrier during speaking, as patients have to literally point at their handicap. Dr. Bart Verkerke, Project Manager of the Department of BioMedical Engineering at the main partner, the University of Groningen, describes how the partners initiated this work in a previous EUREKA project. Most of the NEWVOICE participants were involved in project E! 723 ARTIFICIAL LARYNX, during which we gained a lot of the expertise in medical devices to improve the rehabilitation process that will be used in this project. The new voice-producing element that was inspired by the lips of a trumpet player is working well technically, but poses a prototyping problem as it is too big to fit into an existing shunt valve. Work is progressing to try to develop a small voice-producing element that still has the proper frequency characteristics. At the same time, the project also aims to develop a new fixation technique, the so-called tissue connector, which allows for a larger shunt valve.

(Philadelphia, PA)- Researchers at the University of Pennsylvania Cancer Center have completed a study assessing both the oncologic outcome and quality life implications following a novel surgical technique to partially remove the larynx (voice box) of patients with cancer of the larynx, as opposed to a total removal of the larynx (TL). This organ preservation surgical procedure is known as the supracricoid partial laryngectomy (SCPL). The study which will be published in the February issue of Laryngoscope, demonstrates that the SPCL procedure provides excellent local control of the cancer, significantly improves a patients quality of life vis-a-vis the TL procedure, and suggests that many patients who underwent total voice box removal in the past could have been reasonable candidates for this partial removal procedure to save their voice box. The treatment of larynx cancer, the most common form of head and neck cancer, can have a major impact on many aspects of the quality of life of patients, including their ability to swallow and speak, as well as their ability to breathe normally. Depending on the size and extent of the cancer, several surgical and non-surgical procedures are available as treatment options. Historically, a total removal of the larynx has been the most common treatment method in cases where the cancer was very extensive and when non-surgical interventions such as chemotherapy and/or radiation have failed, resulting in a recurrence of the cancer. The main problem encountered after total removal of the voice box is that the patient no longer breathes through their nose and mouth, and must breathe through a stoma, or breathing hole in the neck.
