Extractions: Recently, the outside of my left heel has been hurting when I walk on it. With Pennie's foot issues, I was starting to wonder if I had bone spurs or something similarly icky. I mentioned it to Dave today and in a case of turnabout, he said, "you might have just bruised it or something." Well, that got me to thinking - as late night work sessions have a habit of making me do - and I just figured it out. Right after we checked in to the hotel last Friday night, I was setting my computer up on the table. As I backed away from the table, the outside of my left heel struck the leg of the chair with some serious force - after the obligitory rubbing the pain away, I had pretty much forgot about it. Upon palpitation of the area, it is pretty clear that there is a bump from hitting it and it is indeed bruised. Thanks Dave, for the memory jog.

Extractions: Login Daniel Dufala Melancholy was a term that described a condition of deep sadness and lethargic behaviors leading to experiences of delusion and sometimes accompanied by a temptation to suicide (Midelfort 302). It reigned as one of the most common diseases of sixteenth- and early seventeenth-century Europe and encouraged a great mass of literature that remains today a testimony to the extent of discontent in the human experience (145). Melancholy can easily be compared in contemporary terms to the mental illnesses defined as depression and manic depression and is considered by some to be a disease exclusively associated with sadness. Because of the widespread nature of the disease the term melancholy became generally applied to mental illnesses characterized by disturbances such as rage, anxiety, apathy or deep sadness (147). This led to the need for more accurate and reliable methods of diagnosis and separately defined symptoms and causes (221). The second type of melancholy, described as unnatural, was the result of a corruption from demons and spirits. The possession of an individual by such outside forces would lead to symptoms of sadness that in many cases graduated towards manic episodes and the other behaviors of rage and eventual absolute madness (154). This was considered in some instances when the patient could not be cured of the disease. In such circumstances the demonic possession was of such a nature that the spirits had taken their form in the manifestation of a disease with symptoms similar to that of melancholy. But as it was believed that the devil could create infinite disguises, it was assumed that the source of the ailments was not natural and must have come from without (386).

An opera singer and mother of two grown children, 57-year-old Sarah (not her real name) has suffered from hypochondria for more than 30 years. Throughout her life she would get symptoms, such as head and intestinal aches, and worry she was going to die. "I have this morbid dread and fear," she says. Sarah relied on her daughter, now 37 years old, to guide her about which complaints to seek medical attention for and which to ignore. "My daughter really tunes in on me," she says. Unlike other hypochondriacs, who go to many doctors and undergo numerous tests but doubt their reliability, Sarah fears doctors and even newspaper articles on health. Her life, she says, has been one of suffering and terror. But Sarah decided to take a risk, and she enrolled in a clinical trial run by Dr. Brian Fallon'85, assistant professor of clinical psychiatry. The trial tests whether Prozac, or fluoxetine-a drug currently approved by the FDA for depression and obsessive-compulsive disorder-could help hypochondriacs. This five-year, $500,000, blind, randomized, placebo-controlled trial of 100 patients is the first National Institute of Mental Health-sponsored controlled trial for the treatment of hypochondria. Started in July of 1993, the trial will determine whether Prozac can help hypochondriacs and if patients relapse after medication is withdrawn.

Where the patient sees physical illness, the doctor sees a psychological problem, and frustration rules on both sides of the examining room. Recently, however, there has been a break in the impasse. New treatment strategies are offering the first hope since the ancient Greeks recognized hypochondria 24 centuries ago. Cognitive therapy, researchers reported last week, helps hypochondriacal patients evaluate and change their distorted thoughts about illness. After six 90-minute therapy sessions, the study found, 55 percent of the 102 participants were better able to do errands, drive and engage in social activities. Antidepressant medications, other studies indicate, are also proving effective. The biggest challenge is sorting out their hypochondriacal complaints from real disease, because even a hypochondriac can get sick.
