Extractions: Hydrocephalus is described either as obstructive or noncommunicating and non-obstructive or communicating. This relates to the level of obstruction and whether CSF exits from the foramina in the roof of the 4th ventricle or is fully trapped within the ventricular system. Hydrocephalus is also classified as acquired when it is caused by acquired causes or congenital. In congenital hydrocephalus it is caused by either excessive production of CSF, blockage to the CSF drainage or reduced excretion into the venous channels. The CSF is clear colourless fluid which has minimal content of protein. It is contained in the ventricular system as well as the subarachnoid spaces surrounding the brain and the spinal cord. The CSF is in hydrostatic equilibrium with the interstitial tissue of the brain and can permeate across the brain tissue in both directions. It is expected that the brain tissue and the CSF would have the same hydrostatic pressure in any part of the brain. As much as the brain tissue is protected by a blood brain barrier from changes outside the central nervous system, the CSF has the same protection and does not change biochemically as a result of changes in the systemic circulation. These barriers are at the level of the endothelium of brain capillaries, at the level of the epithelium of the choroid plexuses and the outer layers of arachnoid matter. These barriers protect the brain and the subarachnoid spaces from damaging influences outside the brain.

Extractions: Hydrocephalus is that state in which something has occurred to prevent this orderly procession of events. If this occurs, there is a relative build up of spinal fluid in the brain which can lead to injury or even death if not treated. It is very important to realize that hydrocephalus is the result of some event on the nervous system and that a child's prognosis is not so much based on the hydrocephalus as the cause of the hydrocephalus. While many cases have no clear cause, the following have been associated with the development of hydrocephalus; bleeding, infection, trauma, tumors, vascular problems, and structural problems. Some occur during pregnancy and others after birth. In addition, a small number can be transmitted genetically. It is important to realize that while hydrocephalus is very treatable, the underlying cause may not be or may have caused irreversible damage to the brain. It is important for the doctors taking care of a child with hydrocephalus to determine if the problem is ongoing or if it has resolved but left enlarged but stable ventricles. This can sometimes be difficult to tell and often the decision will be based on many factors including symptoms, eye exams, changes in level of activity or school performance. In addition to this often the child will have repeated (serial) radiographic exams to look for changes in the ventricles.

Epstein, M.D. The use of shunting for hydrocephalus has a long history of improvements made through basic science, as well as clinical innovations and biomedical products. Shunting has dramatically changed the outlook of children with hydrocephalus, with many of them having normal life expectancies and attaining normal intelligence. The use of shunts, however, has created many unique problems of shunt dependence with frequent shunt revisions being the rule for most hydrocephalic children. Shunt problems assume a major amount of all neurosurgeons efforts. In addition, the slit ventricle syndrome has posed many new questions regarding what the post-shunting ventricular size should be, and what problems are created by having a slit-like ventricular system. This article reviews current concepts of childhood hydrocephalus with respect to the various hydrocephalic syndromes, and provides detailed descriptions of problems unique to various shunting systems. In addition, the slit ventricle syndrome is distinguished from asymptomatic post shunted slit-ventricles. The various forms of the slit-ventricle syndrome are described , and proposed guidelines for classifying, working up, and treating the slit ventricle syndromes are provided.

Hydrocephalus is an abnormal accumulation of cerebrospinal fluid (CSF) within cavities called ventricles inside the brain. CSF is produced in the ventricles, circulates through the ventricular system and is absorbed into the bloodstream. CSF is in constant circulation and has many important functions. It surrounds the brain and spinal cord and acts as a protective cushion against injury. CSF contains nutrients and proteins necessary for the nourishment and normal function of the brain. It also carries waste products away from surrounding tissues. Hydrocephalus occurs when there is an imbalance between the amount of CSF that is produced and the rate at which it is absorbed. As the CSF builds up, it causes the ventricles to enlarge and the pressure inside the head to increase. Hydrocephalus that is congenital (present at birth) is thought to be caused by a complex interaction of environmental and perhaps genetic factors. Aqueductal stenosis and spina bifida are two examples. Acquired hydrocephalus may result from intraventricular hemorrhage, meningitis, head trauma, tumors and cysts. Hydrocephalus is believed to occur in about 2 out of 1,000 births. The incidences of adult-onset hydrocephalus and acquired hydrocephalus are not known.

CSF usually circulates through channels of the brain known as ventricles, as well as flowing around the outside of the brain and through the spinal canal. When the circulation or absorption of this fluid is blocked, or excessive fluid is produced, the volume of fluid in the brain becomes higher than normal. The accumulation of fluid puts pressure on the brain, forcing it against the skull and damaging or destroying the tissues.
