Extractions: Hemangioma of infancy is the most common tumor in infancy. The incidence in the newborn nursery is between 1.0 percent and 2.6 percent in white, black, and Japanese infants. An incidence of 10-12 percent has been reported in white children by age 1 [ ]. It is characterized by an initial phase of rapid proliferation, followed by slow involution, often leading to complete regression. Although most of the tumors are small and innocuous, some may be life- or function-threatening, or have associated structural congenital anomalies [ ]. Vascular malformations are grouped as high- and low-flow malformations based on the flow dynamics within the lesion. If the lesion has arteriovenous shunting, it is considered to be a high-flow malformation; otherwise, it is considered to be a low-flow malformation. Most birthmarks are small and harmless, and require no additional testing or treatment. However, treatment may be imperative for some birthmarks. For many patients, particularly teenagers, discolorations of the skin or cosmetic disfigurations are a major concern. Many birthmarks may not be easily noticeable but may cause significant discomfort and pain despite their benign nature. Other birthmarks can grow rapidly and cause significant health problems (eg, heart failure), requiring intense medical care and therapeutic intervention such as embolization or sclerotherapy. Approximately 1 out of 100 newborns develop at least 1 hemangioma and approximately 25% of premature babies develop hemangiomas. Typically, hemangiomas exhibit a fairly predictable pattern of growth. They grow rapidly for the first 6 to 12 months of life, after which they begin a much slower process of shrinking, or involution, which may take from one to seven years. By the time a child reaches about 12 years of age, involution is almost always complete. For this reason, most hemangiomas require no treatment. However, some hemangiomas develop near the eyes or on vital organs, where, if they become large enough, they may pose a threat to the child's vision or life. These often require treatment with interferon alpha, a drug that was discovered by Bruce R. Zetter in Folkman's lab in the 1980s to be antiangiogenic and was the first antiangiogenic agent to be used in humans. However, interferon isn't universally effective. Joyce Bischoff and colleagues in the Vascular Biology Program are seeking to learn which factors stimulate the growth of endothelial cells and, more importantly, which factors inhibit growth. Identifying factors that inhibit abnormal endothelial cell proliferation in hemangiomas could suggest new treatments for the children who do not respond to current therapies.

It's free! Save it. Cavernous hemangiomas are benign vascular tumors most commonly seen in the head and neck region in childhood. They have been described rarely in the lungs. We describe a patient with incidental pulmonary nodules discovered at autopsy, which measured up to 0.9 cm and which were present in the lung parenchyma, as well as on the pleura. The nodules were composed of dilated vascular spaces lined by flattened bland cells. Immunohistochemical studies of the lining cells revealed CD34 and factor VIII immunoreactivity, consistent with a lesion of endothelial origin. Taken together, the gross, microscopic, and immunohistochemical findings support the diagnosis of multiple pulmonary cavernous hemangiomas.

The ultrasound of the liver showed a hyperechoic mass in the dome of the right lobe (seen a sagital and an oblique images; arrows show lesions on sagital and on transverse images). In addition there was a complex lession at the inferior tip of the right lobe of the liver ( shown by arrow First past images from the blood pool study (above) show no areas of increased flow. Planar delayed images show two areas of increase uptake (the lesion in the dome of the right lobe corresponding to the hyperechoic lesion is shown by arrow heads; the lesion at the tip of the right lobe is shown by arrows; also seen are the blood pools of the heart, H, and the spleen, S). Coronal SPECT images show these two areas of increased blood pool. Hemangiomas are the most common benign tumor of the liver, noted in up to 7% of patients on autopsy series. Up to 20% are multiple. Histology demonstrates an overgrowth of enlarged endothelial lined vascular spaces. While almost always asymptomatic, these tumors have been associated with abdominal pain, fullness, and belching. Complications are similarly rare, but have included bleeding, infarction, necrosis, and thrombocytopenia. The clinical importance of these incidentally discovered tumors relates to distinguishing them from malignancies.

Although only 1 to 2.6 percent of newborn infants have hemangiomas present at birth, the lesions are found in up to 10 percent of patients by 1 year of age, and appear most frequently during the first one to four weeks of life. Hemangiomas may have different appearances, depending upon the depth of the increased numbers of blood vessels. More superficial hemangiomas tend to be bright red and elevated with an uneven surface. Deeper hemangiomas tend to be smooth on the surface, but bluish in coloration. Many times, both red and blue components will be present. Not infrequently, the more superficial types of hemangiomas begin as flat, pink areas, but rapidly change into the elevated, bright red lesions. Hemangiomas typically tend to enlarge in size when the infant is between one month and approximately eight months of age. Then they often reach a plateau and begin to resolve most extensively when the child is between one and three years of age. Approximately 50 percent of hemangiomas are gone by the time the child reaches five years of age; more than 90 percent are totally gone by age nine. Occasionally, resolution can take up to as long as 12 years. In up to 50 percent of hemangiomas, some slight to more extensive skin changes may be noted when "clear." These changes include whiter skin, puckering, scarring, and persistent visible blood vessels.
