In an otherwise asymptomatic individual, recognition of oral lesions, such as oral candidiasis or oral hairy leukoplakia, may give the first indication of HIV disease. For patients who have documented HIV infection, NIH clinical guidelines suggest that oral examinations should be performed by the primary care provider during every physical examination. Routine oral examinations should include careful inspection of the soft tissues, with particular attention paid to the soft palate and lateral margins of the tongue. Oral hairy leukoplakia (OHL) is a benign oral lesion caused by Epstein Barr virus (EBV). Recognition of OHL in a previously healthy patient should prompt HIV testing. This lesion may be present in more than 50% of HIV-positive homosexuals and bisexuals in the United States. Often, the appearance of this lesion is a harbinger of progressive disease; the average CD4+ lymphocyte count is typically 400 at the time of diagnosis of OHL. Clinically, OHL appears as asymptomatic, hyperplastic, verrucous, white to grayish epithelial plaques on lateral tongue. The patient may report that the lesion changes from day to day, but does not rub off.

Walling and colleagues at the University of Texas report that in the majority of cases OHL was resolved and EBV replication was halted. OHL was cleared in 89% of subjects. OHL and EBV replication recurred in two subjects and EBV replication alone was detected in two more subjects. Sedano, DDS Back to Classification Relevant References to Oral Hairy Leukoplakia (OHL) Glick and co-workers (1) have demonstrated that the presence of OHL in patients infected with the AIDS virus is a trustable marker for the progression of the HIV infection. Greenspan and co-workers (2) as well as Moniaci and coworkers (3) have shown than 40 to 60% of patients infected with HIV convert to AIDS within 2 to 3 years after the development of OHL. Brandwein et al (7) using standard DNA in situ hybridization (ISH) to detect Epstein Barr virus (EBV) in lingual hairy leukoplakia, found EBV in all 11 specimens that were investigated. EBV was found in the basal and parabasal layers and strongly localized in the upper epithelial layers. Additionally these authors reported that in the same 11 specimes there was no evidence of human papilloma virus (HPV) or cytomegalovirus DNA. The authors concluded that there is no evidence to suggest that HPV is involved in the development of hairy leukoplakia. Miranda and Lozada-Nur (8) as well as other authors (9,10) have reported the presence of lingual hairy leukoplakia in HIV-negative immunosuppressed and in immunocompetent patients. These authors comment on the importance of a thorough examination of the oral tissues in patients who are undergoing immunosuppressive therapy.

There have been several studies which have demonstrated the importance of OHL as a determinant of the prognosis of AIDS. These studies have shown that patients with OHL have a shorter life span than those that do not present this lesion. This might implicate that the status of the immune system in those patients with OHL is at a lower level than those that do not have OHL. OHL is generally not treated unless it is requested by the patient because of aesthetics, masticatory or phonetic reasons. OHL resolve in patients which undergo treatment with AZT (zidovudine). This 24 year-old man presented these barely visible, vertical striations on both sides of his tongue. Note their almost parallel orientation. These lesions did not rub off. The clinical diagnosis of OHL was established. Additionally, the patient had a large tonsilar abscess and a weight loss of 15 pounds in the last 2 months. He attibuted the loss of weight to change in his diet. The patient denied belonging to any AIDS risk group but a Western blot and ELISA test, when performed, were positive and his CD4+ cell count was 280 cells per cubic millimeter. Then, the diagnosis of AIDS was established.
