A 62-year-old lady with right frozen shoulder, left-sided sciatica and peripheral neuropathy Presentation A 62-year-old housewife from New Delhi presented with pain in the right shoulder for the past 2 years and low back pain for 2 months prior to admission. She had a past history of hypertension for 5 years treated with atenolol 50 mg o.d. Initially, she had pain in the right shoulder only on exertion, associated with stiffness and difficulty in movement. The pain was maximal after activity and at the end of the day and was relieved with rest. There was no radiation of the pain, or increase in intensity on coughing, sneezing or straining. She was treated with analgesics and anti-inflammatory gels which afforded minimal improvement. For the past 2 months, she reported progressively increasing pain and marked limitation of movement of the right shoulder. Minimal movement caused severe lancinating pain. She was examined by various physicians and an orthopaedician and was thought to have a frozen shoulder. Other than osteoporosis, X-ray of the right shoulder did not reveal any abnormality. She was treated with analgesics, calcium supplements and advised physiotherapy. Despite this, her symptoms progressively worsened. For the past 2 months she also complained of low backache, which at times radiated to her left buttock and the back of the left thigh. The pain was catching in type but did not increase with coughing, sneezing or straining. There was no history of trauma, stiffness or heaviness of the lower limbs, difficulty in walking, or of bowel or bladder involvement.
