Extractions: FOREIGN ANIMAL DISEASES VENEZUELAN EQUINE ENCEPHALOMYELITIS (Peste loca, Venezuelan encephalitis, VEE, VE) Definition Venezuelan equine encephalomyelitis (VEE) is a zoonotic, mosquito-borne, viral disease affecting both Equidae and humans (6,8). In Equidae, infection may produce an acute, fulminating disease that terminates in death or recovery without development of encephalitic signs, or the more classical disease with progressive clinical encephalitis. In human beings, a flulike syndrome predominates with an accompanying high fever and frontal headache. Human deaths may occur in the young or the aged. A wide variety of hosts and vectors may be infected (5,10,13,15). Etiology The etiologic agent of VEE is an alphavirus of the family Togaviridae (formerly the group A arboviruses). . The virions are 60-75 nm in diameter and have an essential lipid membrane. Only minor antigenic variations exist among different VEE virus isolates (10,16). Six subtypes (I, II, III, IV, V, and VI) have been identified within the VEE complex. Within subtype I, only two (A/B and C) of the five variants (A/B through F) have been associated with epizootic activity in equines (5,10,13-15). The other variants (I-D through I-F) and subtypes (II through VI) have been associated with nonequine, sylvatic, or enzootic activity. Infection with one variant or vaccination with attenuated virus generally results in production of neutralizing antibodies and cross-protection of variable duration to infection with the other subtypes and variants (13-15).

Extractions: Acute disseminated encephalomyelitis (ADE) is a neurological disorder characterized by inflammation of the brain and spinal cord caused by damage to the myelin sheath. The myelin sheath is the fatty covering, which acts as an insulator, on nerve fibers in the brain. ADE may occur in association with a viral or bacterial infection, as a complication of inoculation or vaccination, or without a preceding cause. Onset of the disorder is sudden. Symptoms, which vary among individuals, may include headache, delirium, lethargy, coma, seizures, stiff neck, fever, ataxia, optic neuritis, transverse myelitis, vomiting, and weight loss. Other symptoms may include monoparesis (paralysis of a single limb) or hemiplegia (paralysis on one side of the body). The disorder occurs in children more often than in adults. One week later, severe neurologic signs developed, and the horse was humanely destroyed. A vaccine-related encephalomyelitis was later suspected. A final diagnosis of EEEV infection was established on the basis of acute onset of the neurologic signs, histopathologic and serologic testing, and isolation and molecular characterization of EEEV from brain tissue The vaccine was extensively tested for viral inactivation. Nucleotide sequences from the vaccine and the virus isolated in the affected horse were also compared. In California, arboviral encephalomyelitides are rarely reported, and EEEV infection has not previously been documented. This report describes the occurrence of EEEV infection in the horse and the investigation to determine the source of infection, which was not definitively identified.

( ) reports successful treatment of human herpesvirus 6 (HHV-6) encephalomyelitis. The patient was an immunocompetent young woman whose symptoms were fever, urinary retention, blurred vision, quadriparesis, bilateral papillitis, and optic neuritis. Magnetic resonance imaging (MRI) showed multiple lesions on the spinal cord white matter and the left thalamus, and the cerebral spinal fluid (CSF) showed inflammation. The patient was treated with acyclovir for 3 days, high-dose methylprednisolone for 5 days, cidofovir for 1 day, and ganciclovir for 15 days, starting on day 23 of hospitalization. By establishing a relationship between antiviral drug doses, serial determinations of HHV-6 DNA by polymerase chain reaction (PCR) in CSF, and neurologic improvement, Denes et al. concluded that antiherpesvirus drugs led to her recovery. This case fits well in the spectrum of acute disseminated encephalomyelitis (ADEM), an inflammatory demyelinating disease of the central nervous systems of children and young adults, which occur in close temporal relationship with several infectious illnesses and immunizations (

Extractions: Feedback chronic fatigue syndrome (CFS), collection of persistent, debilitating symptoms, the most notable of which is severe, lasting fatigue. In other countries it is known variously as myalgic encephalomyelitis, chronic fatigue and immune dysfunction syndrome, and postviral fatigue syndrome. It was first recognized as a syndrome in the 1860s by Dr. George Beard, who called it neurasthenia. He believed it to be a neurosis with a fatigue component. Definitions and theories of its cause have changed over the years; many cases have been misdiagnosed as imaginary because doctors could find no cause. In the mid-1980s it came to the public's attention, as affluent women in their thirties began to seek treatment. (For unknown reasons, more women than men seek treatment for the disease.) As currently defined, chronic fatigue syndrome is the presence of severe, disabling fatigue lasting for six or more consecutive months. The fatigue is persistent or relapsing, and is new (i.e., not lifelong), not relieved by rest, not the result of ongoing exertion, and interferes with normal work, social, educational, or personal activities. Diagnosis also requires at least four of the following symptoms, each persistent or recurring and not present before the fatigue: impairment of short-term memory or concentration, sore throat, tender lymph nodes in the neck or axillary region, muscle pain, joint pain, headaches peculiar to the syndrome, unrefreshing sleep, and malaise of more than one day's duration following exertion. Chronic fatigue that does not meet all these criteria is termed "idiopathic fatigue."

Abstract: Acute disseminated encephalomyelitis(ADEM) is an uncommon inflammatory demyelinating disease involving central nervous system white matter. A series of seven cases seen over a period of one year have been described. The patients presented with acute onset of focal neurological deficit (3 patients), alteration of sensorium and ataxia . CT scan showed non specific hypodensity in 4 cases and was normal in 3 patients . MRI showed characteristic radiological changes. Complete recovery was seen in 3 patients , one died and 3 were left with sequelae.

Extractions: Until the late 1980s, myalgic encephalomyelitis was thought to be a distinct, infectious disorder affecting the central, peripheral and autonomic nervous systems and the muscles. Its major symptom was fatigue to the point of extended periods of exhaustion. A group of experts studying the Epstein-Barr virus first published strict criteria for the symptoms and physical signs of chronic fatigue syndrome in 1988. This case definition was further refined in 1994. The Fact Sheet for CFS published by the National Institutes of Allergy and Infectious Diseases of the National Institutes of Health states that "[T]oday, CFS is also known as myalgic encephalomyelitis, postviral fatigue syndrome, and chronic fatigue and immune dysfunction syndrome." ME/CFS is now recognized as part of a range of illnesses that have fatigue as a major symptom.

Acute disseminated encephalomyelitis (ADEM) (post-infectious encephalomyelitis or post-vaccinal encephalomyelitis) is an acute inflammatory demyelinating disease of the central nervous system (CNS). Typically it follows infection by a virus, but it may also follow infection by other agents or may complicate vaccination. Sometimes it occurs without any obvious triggering factors. The clinical manifestations are diverse and include presentation with acute transverse myelitis. Acute haemhorrhagic leukoencephalitis is a rare and more severe form of ADEM with a high mortality and morbidity. There is good evidence that ADEM and acute haemhorrhagic leukoencephalitis are autoimmune diseases similar to acute experimental autoimmune encephalomyelitis (EAE) and hyperacute EAE repectively. Experimental Autoimmune Encephalomyelitis (EAE) is the prototype for cell-mediated autoimmune disease in general, and is the best animal model of human CNS inflammatory demyelinating disease. It has three forms, which vary in clinical course and neuropathology: acute EAE, hyperacute EAE and chronic relapsing EAE. Acute EAE and hyperacute EAE are monophasic diseases which resemble the human diseases, acute disseminated encephalomyelitis and acute haemhorrhagic leukoencephalitis, respectively. This disease not only affects horses, but can also be deadly for humans. The virus was isolated, characterized, and equine vaccines were developed in the 1930s. The two forms currently active in the United States are Eastern Equine Encephalomyelitis (EEE) and Western Equine Encephalomyelitis (WEE). The viruses responsible for causing these diseases are members of a family of viruses called the alphavirus. WEE is most active in Western United States and parts of the Midwest. EEE has an Eastern United States distribution. Historically, there have been isolated cases in Michigan and Wisconsin as well as outbreaks in the Caribbean, Dominican Republic, Haiti, Panama, and Central and South America.
