It is an inherited disorder; about half the family is usually affected. It often begins at two to six months of age, disappears for years, and returns in early adulthood. In children, food allergy may play a part. In Greek eczema means "boiling up." Eczema (also called atopic dermatitis) does seem to "erupt" as dry, red, scaly patches on the fold areas of the body such as the eyelids, neck, inner elbow, and behind the knees. At its worst, eczema will crust and ooze, usually indicating overgrowth of bacteria known as Staphylococcus aureus. Occasionally it affects only the hands, presumably triggered by occupational exposures, e.g., frequent hand washing. Research has shown that a specific subset of T-lymphocytes, known as TH2 cells, concentrates in eczematized skin. The byproducts of excited T cells, known as cytokines, are directly responsible for the itch and redness. Medication targeting these cells should theoretically lead to improvement. That is exactly the case with cortisone and immunosuppressive agents like tacrolimusand pimecrolimus.
