    Extractions: Over the course of treatment, Julie eventually identified 13 different alter personalities. Julie's experience is not uncommon for individuals with DID. Most people with DID have more than one alter, and some have reported up to 200 alter personalities! Usually, some of the alter personalities know about other alters, but only one alter (known in the psychology literature as the Internal Self Helper) knows about all the other alter personalities. This was true for Julie. She had some alters that knew about others, but only one knew about all the other alter personalities. Though effective at protecting the child in the immediacy of a traumatic and emotionally horrible situation, dissociation obviously creates a false understanding of the world that generally crumbles years later as the brokenness and stresses of life continue into adulthood. Even after the abuse is over and the child is separated from the threatening situation, the dissociation and lack of communication between the personality states continues. Ultimately, individuals with DID often seek counseling, not for the DID itself but because of depression, memory loss, confusion, or distress over forgetting particular interactions with people. For Julie, it was feelings of sadness and abandonment after the loss of a meaningful adult relationship that initially brought her into counseling.

In MPD, most recently known as Dissociative Identity Disorder (DID), a person experiences and displays at various times two or more distinct personalities. Often, these personalities are not aware of the existence of the other(s), which may result in some of the personalities having significant portions of time for which they have no memory. What "Causes" MPD? There is no proven single cause for MPD. However, many experts theorize that MPD is primarily caused by trauma or extreme stress. Many researchers have found a strong connection between MPD and the experience of severe childhood abuse. In fact, some researchers have argued that victims of severe trauma and abuse may develop MPD as a survival mechanism to escape from the event that is outside the range of normal human experience. How Do I Know If Someone Has MPD? MPD is very difficult to diagnose, even for experienced clinicians. Most people occasionally display at least some of the symptoms of MPD, even though they do not have MPD. However, if you suspect that you or someone you care for has MPD, you might consider a referral to a psychotherapist who specializes with this psychological disorder, as well as any additional specialties that might be useful to you or the one about whom you are concerned. A case of dissociative identity disorder presenting as gender dysphoria is presented. This individual, who was sexually traumatized as a little girl, had seven surgeries to complete gender reassignment before treating clinicians recognized that there were multiple male and female ego states. Psychological testing and treatment recommendations are suggested so that integration can be achieved or at least consensus of ego states can be reached before GRS surgery is done.

    Extractions: On Serendip Dissociative Identity Disorder (DID), formerly known as Multiple Personality Disorder (MPD) is considered by the American Psychiatric Association to be one of 4 main kinds of dissociative disorders (DSM-IV): "The essential feature of dissociative disorders is a disturbance or alteration in the normally integrative functions of identity, memory, or consciousness" . If identity is the main function affected, the person is said to have DID. Most non-DID people have one identity comprised of many parts that work harmoniously together. They have only one I-function which consists of a conglomeration of thoughts and feelings formed from connections between many different brain areas. People with MPD, however, have a decentralized, internal network of 2 or more I- functions or "alters," each with its own physiology, behavior, and cognitions. Dissociation often enables victims to maintain a relatively healthy level of functioning because traumatic memories are disconnected from other information in their minds. A multiple presents her view of DID: "We do not SUFFER FROM MPD. We SURVIVE because of MPD" . Living with this disorder is often an extremely lonely, confusing experience. Luckily, there has been much success in treating this disorder thanks to the extensive research conducted into DID etiology.
