This is one of the most alarming facts to come from all the research; depression is affecting younger and younger people - adolescents and teenagers. (1) Here we look at why, and what we can do about it. Twenty years ago depression in children was almost unknown. Now the fastest rate of increase in depression is among young people. Again, this backs up the fact that most depression is not caused by chemical imbalances, whether in adolescents, teenagers or adults. What we are seeing are changes in society where basic needs (see later in the Depression Learning Path ) for companionship, healthy goals, responsibility, connection to others and meaning are not automatically met. Children, adolescents and teens are fed a constant diet of images showing how we are meant to look, sound and be, and told that this is important in life. Meaning is attached to what they have, or look like, rather than what they do, or achieve. Regardless of our own affluence, we see what those at the 'top' have and are told we should have it too, without thought for the tools or strategies to go about achieving it. During childhood, teenage years and particularly adolescence, pressure to conform with peers can be almost intolerably strong. If children feel different, inadequate or deprived in some way, then depression may result, depending on how they deal with it.

Currently, there are no anti-depressant medications approved for use in children and adolescents. “Unfortunately, there is very little information available about treatment for depression in children and adolescents,” said Dr. Karen Dineen Wagner of the University of Texas Medical Branch. Wagner is a member of a team of researchers who conducted a multi-site clinical trial of the effectiveness of citalopram – a popular adult anti-depressant marketed as Celexa by Forest Pharmaceuticals, Inc. – with young patients. “The results of the study show that citalopram may help to relieve the suffering of youths with depression,” Wagner reported on Sunday, Dec. 9, at the annual meeting of the American College of Neuropsychopharmacology held in Waikoloa, Hawaii. Finding effective treatments for this childhood order is particularly critical. Children who suffer from clinical depression lose interest in activities and have low energy, poor concentration and sleeping problems. They may feel so hopeless that they want to kill themselves. Clinical depression can have a devastating impact on children’s school performance, friendships and family relationships. Children who are depressed are likely to experience depression in adulthood. Therefore, treatment of this childhood disorder is critical, Wagner said.

Children of different ages have differnet symptoms, often reflecting their cognitive and developmental maturity. Clinically depressed children are not experiencing normal sad mood; rather they are as affected as their parents can be, with school, friendships, family and even life itself at risk. Children who are depressed, especially young ones, do not have the vocabulary of adults. They don't know how to say "I am sad" or I am lonely" as well as they know how to say "my tummy hurts" or "I don't want to go to school".Sad mood is not present nearly as often in children, who tend to either somatize (get sick) or act out their depression. Children who are picked on a lot are almost always depressed. They get the bully to act out their own disregard for them selves. Some bullies are also depressed. Children who fight a lot, run away from home, get involved in delinquent activities,break windows etc. should be evaluated for depression. When Dr. Charles Kaiser and I developed the children's version of the Multiscore Depression Inventory, we knew that we had to have it written in the children's own language. Twelve children, all in all, wrote nearly all the items.The subscales allow a clinician to assess not only depression but which aspects of depression are most pronounced. Subscales measure guilt, low self esteem, anxiety, suicidal ideation, social introversion, oppositional defiant behavior, hopelessness (pessimism),instrumental helplessness, and sad mood.

With adults, depression symptoms tend to be straightforward and easily detected. Some children may have such obvious symptoms as constant sadness, lack of energy, lack of interest in previously pleasurable activities, inability to concentrate (not related to ADHD), being either unable to sleep, or sleeping too much, lack of appetite or overeating. Other children's symptoms may be very different. A child suffering from depression can be defiant, angry, aggressive, anxious, or withdrawn. She may have nightmares, be afraid to be away from her parents or withdraw from them, not want to go to school, and her grades may drop. She may complain that no one likes her, or that she doesn't have any friends. A child who begins giving away her possessions may be suicidal. Any time a child talks about dying or killing herself, take her seriously. If a child is having these symptoms, it is vital that her doctor evaluate her. There are physical problems that can cause depression. After a thorough medical evaluation, if she does not have any physical problems, talking to her teacher(s) can be helpful. Getting feedback from school about how she is behaving in class, and how she gets along with her peers can give additional perspective on her symptoms.
