It describes how the disease can affect people and how treatment may be provided. The information is based on the Epidemiological Survey of Dystonia (ESD), which arose from research carried out in North East England between 1993 and 1999. The database continues to be maintained and currently, there are over 1,600 people registered in the ESD. Definition Dystonia has been defined as a, previously little known, neurological disease of the bodys central nervous system and consists of a group of related movement disorders, characterised by involuntary and often painful, prolonged spasms of muscle contractions. It has also been classically defined as "a syndrome of sustained muscle contractions, frequently causing twisting and repetitive movements, or abnormal postures." It can affect a single part of the body, or the whole body, or any mid-way position between these two extremes. Despite this neurological disorder being named by Oppenheim in 1911, relatively little research had been undertaken until recently. The ESD was the first comprehensive and lengthy study of the disease, undertaken within a specified population, anywhere in the world.

Stephen G. Reich and Robert L. Ruff, Cleveland, OH retour vers les peintres What Pieter Brueghel the Elder had in mind in the mid-16th century, when he painted the face of a man showing the eyes tightly closed and the mouth widely open, may never be known. Historians of art contend that this painting depicts a huge yawn and have titled this work Yawning Man or De Gaper . Neurologists have offered alternative ideas; Marsden credited R.E. Kelly for noting the similarity of this painting to the syndrome of blepharospasm-oromandibular dystonia and suggested the eponym Brueghel's syndrome " (Marsden CD, J Neurol Neurosurg Psychiatry 1976;39:1204-9). Although Brueghel's works of art reveal that he had a fascination with human physiognomy, both normal and pathologic, Marsden's claim that Brueghel "clearly recognized the syndrome" may be an overinterpretation of this unsigned, undated, and untitled painting. DYSTONIA Meiges syndrome combines Blepharospasm and Oromandibular Dystonia, and is also known an brueghels syndrome, after the 16th century artist who painted http://www.dystonia.org.za/WhatIsDystonia.htm
Extractions: WHAT IS DYSTONIA? Dystonia is the name given to a group of movement disorders which are characterised by sustained muscle contractions causing twisting and repetitive movements or abnormal postures. Such dystonic spasms may affect one or more parts of the body, or the whole body. They are frequently painful, but disappear in sleep. Dystonias are often misunderstood by the public and may be misdiagnosed by medical professionals not familiar with the condition. Symptoms are often transient and can be mistaken for emotional or psychiatric disorders. Intellect, personality, emotions, sight, hearing, sensation and sexual function may be quite normal, but may be inhibited by the affliction. WHAT CAUSES DYSTONIA? The cause of these abnormal movements is thought to stem from the basal ganglia area of the brain which controls muscle activity. GENERALISED DYSTONIA This type usually starts in one part of the body, usually in a foot or a leg. After walking or other exercise, the foot may turn under, so that the person walks on the outer edge of the foot. It usually starts in childhood. It often spreads to other parts of the body, including the back, neck and arm. It can be inherited and is occasionally sporadic. FOCAL DYSTONIAS Blepharospasm is a focal dystonia of the muscles around the eye. Early symptoms may be uncontrolled blinking, especially in bright sunlight. In some cases, only one eye is affected initially, but in time, both eyes close. The spasms may render the person functionally blind, although the eyes and vision remain normal.
Part 4: Editing In this series of shots from The Stendhal syndrome (La Sindrome di 1996), a young woman becomes so absorbed by brueghel s The Fall of Icarus that she http://classes.yale.edu/film-analysis/htmfiles/editing.htm
Extractions: The shot is defined by editing but editing also works to join shots together. There are many ways of effecting that transition, some more evident than others. In the analytical tradition, editing serves to establish space and lead the viewer to the most salient aspects of a scene. In the classical continuity style, editing techniques avoid drawing attention to themselves. In a constructivist tradition such as Soviet Montage cinema, there is no such false modesty. Vertov's Man with the Movie Camera Chelovek s kinoapparatom, USSR, 1929) celebrates the power of the cinema to create a new reality out of disparate fragments. Cheat cut. In the continuity editing system, a cut which purports to show continuous time and space from shot to shot but which actually mismatches the position of figures or objects in the scene. In this sequence from Meet Me in St. Louis (Vincente Minelli, 1944) the editing sacrifices actual physical space for dramatic space. As we can see in the first shot, there is a wall behind the telephone. However, that wall magically disappears in the third shot in order to show both the telephone and the family seated around the dining table (an important element in the film) from an angle that would had been impossible in an actual room. Cheat cuts were also often used to disguise the relatively short stature of leading men in relation to their statuesque female co-stars.

Extractions: Before the development of sustained closure of the eyelids, about a third of the patients report increased frequency of blinking, suggesting that blepharospasm may be due to impairment of mechanisms associated with normal blinking. The increased blinking that precedes blepharospasm is commonly associated with a feeling of irritation in the eyes. It is usually progresses to clonic and later tonic (sustained) contractions of the orbicularis oculi (facial muscles surrounding the eyes) leading to forceful closure of eyelids. Up to 20% of patients have involvement of one eye initially but the opposite eye becomes involved later in all patients.

Blepharospasm For example, in the 16th century, the Flemish artist brueghel painted a woman with Blepharospasm and cranialcervical dystonia (Meige s syndrome) http://www.blepharospasm.org/1999jankovic.html
Extractions: Houston, Texas Involuntary facial movements have been recognized for long time and were depicted by artists who were fascinated how these movements distorted the facial expression. For example, in the 16th century, the Flemish artist Brueghel painted a woman with apparent blepharospasm and involuntary jaw opening. [Jankovic, 1988]. Although the eponym "Meige's syndrome" has been sometimes used to designate idiopathic cranial-cervical dystonia, this term is not appropriate because Talkow in Germany and Wood in the United States described blepharospasm and orofacial dystonia several decades before the 1910 publication by the French neurologist's report. It was not until the 1970's that blepharospasm was recognized as a form of focal dystonia. Blepharospasm may vary from only a slightly annoying condition to a disabling disorder which interferes with daily activities such a reading, watching television, and driving, Up to two thirds of patients are rendered functionally blind by their blepharospasm. Blepharospasm is usually exacerbated by bright fight and, as a result, many patients went sunglasses both outside and inside. The spasms may be transiently alleviated by pulling on an upper eyelid or an eyebrow, pinching the neck, talking, humming, yawning, singing, sleeping, relaxation, reading, concentration, looking down and other maneuvers or sensory tricks ("geste antagonistique") [Jankovic and Fahn, 1998]. Unlike idiopathic blepharospasm, which is most prominent when the patient is active, secondary blepharospasm often persists during rest. This distinction, however, is not reliable enough to differentiate primary from secondary blepharospasm.

There were rats, a fat mayor, a piper dressed like a jester with bells and a flute of some sort. I know my mother made her way through this story several times. It is the ending that I remember clearly. The piper comes to the mayor for payment and, when he is refused, leads all the children out of the village and into a secret entrance in a mountain. All the children save one. He has a crutch. He wants to accompany the children into the side of the mountain, but he is too slow. The last picture of the story is of a little boy who sits, leans on a crutch and cries. It is haunting. It is at this moment I feel the Pied Piper as evil as the mayor, as filthy as the rats. He allows someone to be left behind. To be left behind must be horrible. Approximately two weeks after Peter is born, I receive the confirmation that he has Down Syndrome. I take the call from a phone in the Neonatal Intensive Care Unit of Stormont Vail Hospital in Topeka where Peter has been relocated on his third day for a heart condition and fluid in his lungs. I ask for a copy of his karyotype, the picture of his chromosomes which confirms the diagnosis, a word recently added to my vocabulary. It will be sent to me. Tini and I met in college in the late 1980s. Then, he looked 15 at best. He has matured and now looks at least 20. At first I wondered what to make of him. He was lanky and thin, possessing an unusual amount of energy and talking in a rapid southern dialect. Long before we began dating he began walking with me at nights, around the brick lanes that circled the campus. I stared in disbelief as he repeatedly fell into hedges or tripped over curbs, once rolling down a hill. Later he would tell me how nervous he was during those years of walks. I should have been an indication to me of his attentiveness to a fault. From the very beginning he was willing to forfeit some awareness of his own two feet and various pedestrian obstacles for my sake. But back then I just thought him clumsy and told him so on several occasions.
