Information on the following diseases can be found in the Related Disorders section of this report: Meige Syndrome is a rare neurological movement disorder (dyskinesia) characterized by spasms of the muscles of the eyelids and associated loss of tone in these eyelid muscles. Symptoms may include excessive blinking (blepharospasm) or involuntary eyelid closure. On occasion, the muscles of the face may also be involved. The exact cause of Meige Syndrome is not known. This disorder generally affects people during late middle age. The symptoms of Meige Syndrome typically begin during late middle age and may include gradual functional impairment of the muscles of the eyelids (dyskinesia orbiculares oculi). Muscle spasms may cause excessive involuntary blinking (blepharospasm) or episodes of involuntary eyelid closure. In some people with Meige Syndrome, the muscles of the face are also affected, resulting in facial grimacing and uncontrollable squeezing movements of the facial muscles (facial dystonia). Other symptoms may include jaw grinding movements, repeated forceful opening of the jaw, and retraction of the lips into the mouth. Eyelid and facial muscle tone may gradually decline.

Involuntary movements of the face, tongue, palate, pharynx, vocal cords and neck are grouped into the broad classification of cranial-cervical dyskinesias (Table 1).(1) In many dyskinesias, there is evidence of pathology in the basal ganglia or the midbrain. Areas of the basal ganglia involved in orofacial control may be particularly susceptible to the aging process and to the effects of neuroleptic drugs, since the development of many cranial-cervical dyskinesias appears to relate to advancing age or drug or other toxin exposure. Dystonic movement disorders are characterized by muscle tone abnormalities, often manifested as increased muscle tone or muscle spasms. Meige syndrome is characterized by buccolingual, lower facial and eyelid spasms. The eyelid spasm alone is referred to as essential blepharospasm. Hemifacial spasm is pathophysiologically unrelated to the dystonias and is best classified under the broad heading of myoclonus.

Extractions: Dystonia is not a new disease. For example, the 15th century Flemish painter, Pieter Brueghel portrayed persons afflicted with one form of dystonia, blepharospasm (eyelid spasms). There is no known cause of dystonia, nor is there a definitive test for it. Although dystonia has no cure, there are successful treatments which greatly reduce the symptoms and restore patients to many daily living activities. The allegorical content of these works was always obvious, and the pieces of the series did not become separated. The still-life details enumerating many traditional symbols were painted by Jan Brueghel. The amazing increase in the commercial and agricultural product range caused a complete restructuring of people's perception - a change which can be seen very clearly in the numerous paintings and series of paintings on the subject of the Five Senses. Jan Brueghel the Elder's famous variations on this motif - now in the Prado, Madrid - show a number of settings, each of which is associated with the five senses. The dramatic 'unity of character' has been divided into components of the senses, thus reflecting an increasing compartmentalization of the world into functional spheres - a world which is defined in terms of luxury goods. In Jan Brueghel's paintings, the part of purely passive reception or physical consumption is played by an allegorical female figure. Interestingly, by depicting her naked or semi-naked breasts and sometimes her entire body, the painter emphasizes an element of eroticism. In this way the consumption of luxury goods and an emotional state of ecstasy are recognized as a syndrome.

The application of neural networks and innovative approaches to digital image processing is sure to be applied to other problems, such as analysis of planetary photos and particle decay images. The Modeling and Database Workgroup emphasized the need for a database of the face. There are parallels between the need to collect and maintain image databases for the analysis of facial expressions and the need to develop a database of utterances for speech recognition. The TIMIT database, a joint effort of Texas Instruments and MIT, provided the speech community with a research standard for speech records and a means for objectively comparing speech recognition systems. The proposed database of facial expressions could provide similar benefits for the vision community. As the Database Workgroup indicates, there are many challenges that the creation of such a huge multimedia database would bring to the investigators developing scientific databases.

Blepharospasm (Benign Essential Blepharospasm) Benign essential Blepharospasm (BEB) is a condition in which the muscles around the eyelids go into periodic blinking spasms that squeeze the lids shut. Each time a spasm occurs the lids may remain tightly closed for seconds to minutes. In the early stages, the blinking may appear to be merely a nervous twitch or a bad habit. Because of this, most patients have BEB for a long time before becoming aware of it. BEB usually comes on after the age of 50, is more common in women than men, and may be hereditary (though this is not been proven). Estimates of the number of affected individuals in the United States range up to about 150,000. In medical terms, essential means of unknown cause, blepharo means eyelid, and spasm means an involuntary, forceful contraction of muscles. Symptoms Early symptoms are winking, blinking, or squeezing together of the eyelids of one or both eyes or difficulty in keeping the eyes open. You may be especially sensitive to bright light. As the condition progresses, the spasms become more frequent until they are almost constant: both eyelids will clamp shut and the eyebrows will pull down.
