While good, population-based statistics that document the prevalence of pediatric bipolar disorder are lacking, every child mental health professional confronts the problem on a regular basis. A cursory search readily reveals more than a dozen books in the lay press dealing with early onset bipolar disorder, following the widely read descriptive book, The Bipolar Child. Clearly, in less than a decade what used to be an extremely rare illness in children and adolescents is now being diagnosed and treated with great regularity. The change in practice is astonishing to those of us whose professional careers have encompassed it. Even in the 1980's, bipolar disorder was not considered a childhood illness any more than was Alzheimer's disease. Our diagnostic Bible, the DSM III, essentially defined away the possibility of bipolar disorder occurring in prepubertal children and no one looked for it; not surprisingly, it wasn't found. A correction was needed, and the pendulum certainly has swung. Today reasonable estimates place the prevalence of bipolar disorder in adolescents at about 1% and in children, at about 0.3%. The most common age group for the initial diagnosis is between 20 - 30 years, but the second highest incidence occurs between ages 15 - 19 years. Once investigators started systematically asking adults with bipolar disorder when they had the onset of their first symptoms, it became clear that 20 - 30% were symptomatic during childhood or adolescence.

You've enriched our knowledge base about children with early-onset bipolar disorder. Your comments on the site also allow us to note emerging trends in prescribing practices. One of the most recent we've spotted is the treatment of bipolar disorder in children and adolescents without mood stabilizers, but rather with one of the newer, "atypical" antipsychotic drugs such as olanzapine (Zyprexa), quetiapine (Seroquel), or risperidone (Risperdal), often as a stand-alone therapy. In other words, the only medication prescribed is one of these three newer antipsychotic drugs, whose properties include antimanic effects, and so-far, unproved mood-stabilization.

Bipolar disorder, also known as manic-depression, is a type of mood disorder marked by extreme changes in mood, energy levels and behavior. Symptoms can begin in early childhood but more typically emerge in adolescence or adulthood. Until recently, young people were rarely diagnosed with this disorder. Yet up to one-third of the 3.4 million children and adolescents with depression in the United States may actually be experiencing the early onset of bipolar disorder according to the American Academy of Child and Adolescent Psychiatry. Doctors now recognize and treat the disorder in both children and adolescents, but it is still an under-recognized illness. Children with bipolar disorder usually alternate rapidly between extremely high moods (mania) and low moods (depression). These rapid mood shifts can produce irritability with periods of wellness between episodes, or the young person may feel both extremes at the same time. Parents who have children with the disorder often describe them as unpredictable, alternating between aggressive or silly and withdrawn. Children with bipolar disorder are at a greater risk for anxiety disorders and Attention-Deficit Hyperactivity Disorder (ADHD). These "co-occurring" disorders complicate diagnosis of bipolar disorder and contribute to the lack of recognition of the illness in children.

The clinical presentations of this disorder are broad and include mania, hypomania and psychosis. Frequently associated comorbid conditions include substance abuse and anxiety disorders. Patients with acute mania must be evaluated urgently. Effective mood stabilizers include lithium, valproic acid and carbamazepine. A comprehensive management program, including collaboration between the patient's family physician and psychiatrist, should be implemented to optimize medical care.
