SAN FRANCISCO, California (AP) An experimental AIDS vaccine tested in Thailand on some 2,500 drug users has failed, biotechnology company VaxGen Inc. announced Wednesday. The poor results were widely expected after the Brisbane, California-based company announced in February that a much larger experiment testing its vaccine in North America also had failed to prevent AIDS infections. Most AIDS researchers agree that vaccines will be the only effective way to control a pandemic that has killed 28 million people around the world and infected 42 million more, most of them in Africa. Now that VaxGen's vaccine has failed, attention has turned to the two dozen other experimental vaccines being tested on 12,000 human volunteers in experiments around the world. But none of those are as advanced as VaxGen's two failed experiments and any successful candidate is years away. The Thai results underscored again how notoriously wily AIDS is in beating the body's immune system, company officials said.

The Constitutional Court ruled on Thursday the government must make the anti-retroviral drug Nevirapine available in all state hospitals until an appeal against a similar order made in a lower court is heard. South Africa's health ministry has said the country's health infrastructure is insufficient to distribute Nevirapine nationwide and President Thabo Mbeki has questioned their effectiveness. The Constitutional Court is due to hear next month the government's appeal against the lower court's ruling that Nevairapine should be nationally availablae It will decide if the government has a constitutional duty to provide Nevirapine to all state hospitals on a permanent basis. Currently it is only available at 18 pilot projects.

Extractions: Self-esteem, employment and HIV can be everyday problems Ken Shigematsu remembers it well: When he came out as a gay man, it was a community of transgender people in his home state of Hawaii that embraced and helped him through a difficult time in his life. "What mattered to me was that they were interested in ensuring I was safe, nurtured and loved," says Shigematsu, now a social worker and team leader at the S.F. AIDS Foundation. His office is in San Francisco's Tenderloin District, home to a tightly knit community of transgender women, and where the same sense of community and support exists. HIV infection among San Francisco's self-identified transgender community is thought to be at a rate even higher than that which exists in the gay men's community. Because of the stigma that often accompanies the physical changes transgender women manage, it is very difficult for these women to find acceptance mainstream society as well as gaining meaningful and sustaining employment. As a result, many engage in basic survival activities such as exchanging sex or drugs for money, or becoming wholly dependent upon other individuals for support. "There are definitely issues of self-esteem, domestic violence, suicidal ideology and depression," says Shigematsu.
