Vulvodynia is a chronic vulvo-vaginal pain disorder in women. Women with vulvodynia experience chronic burning or sharp knifelike pain that occurs in the absence of infection or vaginal diseases. Often, the unexplained pain results in an inability to exercise, have intercourse, and in extreme cases can cause women to be bedridden. The causes of vulvodynia are still unknown. Current research studies are investigating possible causes of vulvodynia, including
an injury to, or irritation of, the nerves that innervate the vulva; an abnormal response of different cells in the vulva to environmental factors (such as infection or trauma); genetic factors associated with susceptibility to chronic vestibular inflammation; and a localized hypersensitivity to candida (yeast). There is no cure for vulvodynia. However, over 80% of women with vulvodynia achieve relief of their symptoms with a multidisciplinary approach that can include medications, topical therapies, biofeedback, proper diet and more.

Condition May Cause Excruciating Pain
UPDATED: 6:41 pm EDT July 27, 2005 Kristina Kozak is lucky to have her son Alexander. After she experienced some painful symptoms related to a bladder condition, she wasn't sure if she'd ever be able to have a child. "I was to the point where I couldn't cook, I couldn't stand for more than 10 minutes and I couldn't sit for more than two minutes," Kozak said. "It was painful me for me to even drive in the car. That's why I couldn't even work." Kozak's symptoms were due to a condition called vulvodynia, reported WNBC-TV in New York. "Basically what happens is the body gets used to expecting pain and the pelvic muscles tighten up. They get very, very tight, and they don't relax," said Dr. Jacques Moritz, director of the gynecology division at St. Luke's Roosevelt Hospital in New York. The pain can be excruciating, making sexual intercourse, and, in Kristina's case, even sitting down, impossible. "This is a very isolating experience. It's a very embarrassing experience, and it's difficult to talk about," said Amy Stein of Beyond Basics Physical Therapy in New York.

by Terry Miller Shannon, R.R.T. No one knows for sure how many women suffer from this mysterious, traumatic condition, but treatment and support are available. "I thought I was going crazy," says Jane (not her real name). "At one point, I tried to choose a suicide method." Jane's nightmare began with a burning sensation she assumed was a yeast infection. Over-the-counter medication seemed to help...for a while. But the searing agony returned, only to surge and ebb over a period of six years. "I was a happily divorced mother with a good job and a new romance," Jane says. "My future looked so promising. But then...the pain became my life." She was unable to have sex. Her relationship fell apart. Her energy diminished until her job and home life suffered. Desperate, Jane saw six different doctors. Treatments included laser surgery, injection therapy, and hysterectomy. Nothing helped.

Vulvodynia Possibly A Neuropathic Disorder Women with pain in the genital area have often been told it's all in their head but new research shows it may also be in the shins, arms and thumbs. Women with a condition called vulvodynia apparently process pain differently, and these women are more sensitive to pain at other points in their body, researchers at the University of Michigan found. The research appears in the journal Obstetrics and Gynecology The researchers tested 17 women with vulvodynia and 23 similar women without pain at 23 sites throughout the vulva and at the deltoid muscle, the shin and the thumbnail to assess pain tolerance. They found the women with vulvodynia had lower tolerance at all sites than the women without vulvodynia. "Vulvodynia used to be considered a psychological problem or a sexual disorder, and was not treated as a medical problem. Now we know vulvodynia is likely a neuropathic disorder in which the nerves in the area are hypersensitive. We found women with vulvodynia were hypersensitive at the vulva, not only in areas that were clinically tender, but in surrounding areas as well, and at areas not thought to be tender at all - the deltoid, shins and thumb. This study shows there are neurological differences that are not psychosomatic," says study author Barbara D. Reed at the U-M Medical School. Vulvodynia causes chronic and potentially severe pain at the outer genital region, or vulva. Currently, few doctors are familiar with vulvar pain disorders, and many women with intense pain are misdiagnosed for years with chronic yeast infections or psychological problems. Women with more mild pain or whose pain comes and goes often think some degree of pain in that area is normal and don't tell their doctors about it.

Psychology Department What is Vulvodynia? localized vulvodynia, in which pain is restricted to a portion of the vulva, such as the vestibule, as in vulvar vestibulitis syndrome (VVS). The second is generalized vulvodynia (GVD), in which the pain is more diffuse, involving the whole vulva.
1. What is vulvar vestibulitis syndrome (VVS)? VVS is the most common cause of dyspareunia (i.e., painful intercourse) in women of child-bearing age. A recent epidemiological study estimated that VVS affects 12% of pre-menopausal women in the general population. Women with VVS report experiencing a highly localized, burning and/or cutting pain at the entrance of the vagina (called the vulvar vestibule) during sexual intercourse, as well as during other activities that involve applying pressure to the vestibule (e.g., tampon insertion, gynecological exams). Although the pain of VVS typically disappears after pressure to the vestibule is removed, many women report lasting pain or discomfort after sexual intercourse or similar activities. Approximately 50% of women who suffer from VVS have what is called primary VVS, indicating that the pain has been present since their first intercourse attempt. The other half has

HOUSTON A new guideline for the diagnosis and treatment of vulvodynia offers multiple treatment options, including experimental and complementary therapies, for the controversial disorder. "We have oceans of lotions, potions, and notions out there for vulvodynia. There is not going to be one simple cure," Hope K. Haefner, M.D., the lead author of a paper detailing the guideline, said at a conference on vulvovaginal diseases sponsored by Baylor College of Medicine. What is clear, she said, is, "We have to do a lot more than say, 'Love burns.'"

In a general gynecologic practice population, the prevalence of this condition may be as high as 15 percent.1 Before the 1980s, very little was written about vulvodynia. Epidemiology Most patients consult several physicians before being diagnosed. Many are treated with multiple topical or systemic medications, with minimal relief. In some cases, inappropriate therapy may even make the symptoms worse.6 Since physical findings are few and cultures and biopsies are frequently negative, patients may be told that the problem is primarily psychologic, thus invalidating their pain and adding to their distress.7(pp1-15)
