Vulvodynia is characterized by chronic vulvar pain of unknown origin. The pain is usually a burning sensation but symptoms vary in each individual. The pain may come and go or be constant. This condition can seriously affect a woman's quality of life. It is estimated that more than 200,000 American women have vulvodynia.

... Consultants - advertisement - VULVODYNIA Victoria is a 36-year-old housewife living in Arizona, where her medical nightmare began. By all appearances she is the perfect model of the TV soccer mom, with a boy, 10, a girl, 7, a comfortable house in the suburbs and a 1998 Dodge 7-passenger minivan. Victoria also has a common, but relatively unknown disease, one that consumes her life. It is a disease with no cure - a disease that until recently had no name. It is a malady so personal, that Victoria won't discuss it with her closest friends or relatives, yet one afflicting 20 million or more American woman. Victoria has "Vulvodynia" - a constant burning and irritation in the mouth of her vagina. She cannot wear pantyhose or jeans. She is extremely uncomfortable sitting or even standing for long periods. Victoria describes it as "'like a particularly painful and irritating yeast infection that never goes away." She's been forced to live with the pain and discomfort for years, because doctors at first misdiagnosed her condition, a too typical occurance, and then could not find anything to relieve her symptoms. For Victoria, the symptoms of Vulvodynia first appeared in her late twenties, after the birth of her second child. But she thought these might be normal symptoms after giving birth.

    by Lauren Kunis
In September, 1999, I was diagnosed with Vulvodynia after suffering for 2 months with intense swelling, pinching, tingling, and ripping sensations, 24 hours a day, 7 days a week. Before I was finally diagnosed I saw - one family practitioner, my then - current gynecologist, and finally the gynecologist that diagnosed me. After hearing, "yes you look swollen but it probably is just your detergent", and, "well I don't see any signs of a yeast infection, but I am going to give you some suppository yeast medication anyway" and after trying every prescribed Herpes medicine (even though I tested negative for the disease), I decided to comb the Internet for help. Through an Internet referral from a STD site, I found the doctor who diagnosed me. After the initial shock of the diagnosis wore off I thought to myself, “ Okay, we live in the information age, I will go to the Internet”.  I typed the name of this disease in the search engines. I thought I would find doctors and medicines and whatever I needed to help me cope with this disease.  However, I learned there was little to no information available.  I remember the pain I was in, the flares I had almost everyday for hours with no relief.  I was taking a low dose of anti-depressants, (to help dull the nerves so that the pain would lessen,) nothing was helping the flares (severe swelling) and with what little energy I had left after battling the pain all day long, I would get on the Internet and try (in vain) to find information on possible treatments.  The process was exhausting!

Fibromyalgia and Vulvodynia Resources This page contains resources on two interrelated disorders, fibromyalgia syndrome (FMS) and vulvodynia. I first developed symptoms of both of these illnesses in the later half of 1994. I was finally diagnosed with FMS and chronic fatigue syndrome in July of 1996 and with vulvodynia in December of that same year. I was lucky, most people with fibromyalgia and/or vulvodynia suffer much longer before being appropriately diagnosed, and even longer before receiving effective treatment. Fibromyalgia and Chronic Fatigue Syndrome Symptoms of fibromyalgia include flu-like body aches, muscle pain and stiffness, fatigue, sleep disturbances, irritable bowel syndrome (alternating diarrhea and constipation), and difficulty with concentration and memory, among others. Effective treatments can include low doses of antidepressants (for their pain-reducing effect), pain medications of sufficient strength, physical/massage therapy, and the slow introduction of a daily exercise program. Many people with FMS have also found some relief through nutritional supplements such as malic acid . Fibromyalgia is similar to chronic fatigue syndrome and some experts believe they are the same disorder. People with fibromyalgia often fit the criteria for both diseases. The causes of fibromyalgia and chronic fatigue syndrome are not known. Many women with fibromyalgia also have vulvodynia. The vulva is the external area in women around the vagina. Vulvodynia is pronounced as (vul-vo-din-ee-a). Vulvodynia is a syndrome characterized by chronic vulvar discomfort, especially burning, stinging, irritation or rawness. Sexual intercourse, walking, sitting or exercising can make the pain worse. In study it is found that almost 20% of American women have suffered from chronic vulvovaginal pain at some point in their lives. Vulvodynia is more common in white women.
What are the Characteristics or Symptom of Vulvodynia ?
Vulvodynia is characterized by itching, burning, stinging or stabbing in the area around the opening of the vagina. Pain can be unprovoked, varying from constant to intermittent, or occur only on provocation such as attempted vaginal penetration with sexual intercourse, a condition also known as vulvar vestibulitis syndrome or vestibulodynia. Vulvar pain usually has an acute onset. The onset can be associated with vaginitis (yeast, bacterial), changes in sexual activity (new sexual partner), or medical procedures on the vulva (cryotherapy, laser). In most cases, the vulvar pain then becomes a chronic problem varying in length from months to years. The intensity of the pain can vary from mild to disabling. It can be burning, stinging, irritating or raw.

Treatment of Vulvodynia are directed towards providing relief to patients suffering from Vulvodynia (chronic vulvar pain). In case of Vulvodynia every women's symptoms may differ so remember that no single treatment works for every patient of Vulvodynia. Infact a combination of Vulvodynia treatment is used to get relief from vulvar pain.
How to get rid of Vulvodynia by proper treatment
Currently there is no specific cure for vulvodynia. Instead, treatments are provided to aid in symptom relief. These treatments include drug therapies such as tricyclic antidepressants or anticonvulsants, nerve blocks, interferon, biofeedback, diet modification, and topical corticosteroids. The antidepressants are for women who become depressed because the vulvodynia has interfered with their quality and way of life. Interferon is a protein designed to protect against viral infection. Biofeedback is a training program designed to develop the individual's ability to control the involuntary nervous system. Using this technique, the woman may be able to relax the muscles in the pelvic organs that may be having spasms and causing pain.
Treatment of vulvodynia usually requires a multidisciplinary approach that may include:
Psychological therapy / Biofeedback therapy Treatment for Vulvodynia
This therapy can help reduce pain by teaching you how to control specific body responses. The goal of biofeedback is to help you enter a relaxed state in order to decrease pain sensation. To cope with vulvodynia, biofeedback can teach you to relax your pelvic muscles, which can sometimes contract in anticipation of pain and actually cause chronic pain itself.

The cause of Vulvodynia is still unknown and is speculated by physicians to be caused by an injury or irritation of the nerves that supply and receive signals from the vulva, a localized super sensitivity to yeast, an allergic reaction to environmental irritants, high levels of oxalate crystals in the urine, and spasms or irritation of the pelvic floor muscles. At this time, there is no evidence that Vulvodynia is caused from any type of infection or a sexually transmitted disease. Vulvodynia has different symptoms but the most common is a burning sensation. The type and severity of the symptoms differ from each woman. Some complain of stinging, irritation or rawness. It can be constant or intermittent, localized or diffused. There are two major subsets of Vulvodynia and are very much different. One is Dysesthetic Vulvodynia which can be present in the labia majora and or the labia minora. It can affect the clitoris, perineum, mons pubis and even the inner thighs. The pain can either be constant or intermittent and is not always associated by touch or pressure to the area. Sometimes, the vulvar area may show signs of inflammation but normally there are no visible signs. The other subset is Vulvar Vestibulitis and is associated with pain when touched or pressure is applied to the area around the opening of the vagina. Pain is noted with intercourse, tampon insertion, gynecologic exam, bicycle riding, horseback riding and even the wearing of tight clothing such as pantyhose or jeans. Usually the area around the vagina is inflamed and red.

Vulvodynia interferes with daily functioning, such as sitting walking and most other physical activity. It often affects one's ability to engage in sexual activity. It can impair one's ability to work, and participate in a social life. These limitations often lead to depression and thoughts of suicide.
It is common for a woman to consult up to 12 doctors before obtaining an accurate diagnosis.

Dilated, inflamed blood vessels in the underlying connective tissue: Severe inflammation of various glands within the vulvar vestibule.
Three different groups of mucous secreting glands are located around the vulvar vestibule (i.e. the vaginal opening): * The Minor Vestibular glands - shallow (5mm or less) mucous secreting pouches located within the groove at the base of the hymen (vaginal opening).
    - shallow (10mm) mucous secreting pouches on either side of the urethral opening.
In patients with severe or longstanding vulvodynia these various mucous secreting glands can become intensely inflamed, usually providing an identifiable physical reason for it. PHYSICAL FINDINGS Even though the causes for vulvodynia remains unknown diagnostic features are relatively constant from person to person. Careful physical examination will show some or all of the following signs: An irritated dry change to the skin of the vulvar:
This irritative change is usually the first problem to appear and tends to be most striking in women with light skin and red complexion. Patients can generally locate affected areas because of exquisitely painful, sometimes ulcerated red spots at the vaginal entrance.
