Vertigo, strictly defined, refers to an hallucination of movement. When the symptom complex is of spinning or rotation, the cause is almost always the inner ear or peripheral vestibular system. Although it is true that some patients experience a definite sense of environmental spin or self-rotation, the majority do not present solely with true spinning vertigo. The most common complaint is one of dizziness , a term that represents a variety of symptoms (Table 1):

Is it a spinning sensation that could be characterized as vertigo, pointing to the peripheral vestibular apparatus? Is it a sensation of falling without rotation? Is it a sensation of unsteadiness or imbalance? Is there a particular direction in which the patient tends to fall? When the patient's complaint is actually of incoordination or clumsiness, the cause may be cerebellar dysfunction or peripheral neuropathy. When the symptom complex is of "lightheadedness" or "swimmy-headedness," the examiner should think of presyncope or syncope and consider systemic factors such as postural hypotension, vasodepressor syncope, or cardiac arrhythmia.

Introduction: Disability has several meanings. Here we are speaking of it in the legal sense. Persons who have a medical impairment, may be able to obtain financial support from an insurance company or the government. Evaluation and documentation of hearing or vestibular disability is somewhat of a formal process, involving a thorough examination, a set of tests prescribed by regulations, and regular (typically every 3 month) followups during the evaluation process. In the United States, the government rules set the general standard for disability evaluations. Another useful source is the "AMA guide" for evaluation of impairment and disability.

Timothy C. Hain, MD Please read our Return to Index Page last modified: August 6, 2005 Goto Spanish Version Bilateral vestibulopathy occurs when the balance portions of both inner ears are damaged. The symptoms typically include imbalance and visual disturbance. The imbalance is worse in the dark, or in situations where footing is uncertain. Spinning vertigo is unusual. The visual symptoms, called "oscillopsia", only occur when the head is moving (J.C., 1952). The illustration to the right shows what a person with bilateral vestibulopathy may see when driving over a bumpy road. Oscillopsia often occurs during walking (Freyss et al, 1988). Quick movements of the head are associated with transient visual blurring.
What Causes Bilateral Vestibulopathy ?
About 1% of all dizziness in the author's practice is due to bilateral vestibulopathy. Click here for a more detailed discussion. In many instances, bilateral vestibulopathy is due to exposure to an ototoxic medication (about 50%). Gentamicin is an antibiotic medication and gentamicin toxicity is the most common single known cause of bilateral vestibulopathy, accounting for 15-50% of all cases. Ototoxicity can also be due to infection (meningitis, about 10%)

VESTIBULAR DISORDERS Causes and Effects of A Hidden Problem Mr. Wood was concerned about Lori, one of his second-grade pupils. Over the last few months, he had observed several changes in her behavior. A formerly eager, bright and attentive child, Lori was having difficulty concentrating and coping with even minor conflicts and decisions. She rubbed her eyes and tilted her head, and she seemed clumsy at times, holding her body, and especially her head, stiffly. Lori had lost weight - she complained that she felt "sick at her stomach" - and instead of playing at recess, she spent the time resting with her head on her desk. Although her vision had tested normally just a few months earlier, she now squinted a lot. Once she even had a panic attack on the school bus and now avoided riding it. Mr. Wood recalled that Lori had been in an automobile accident the previous summer, but she wasn't seriously injured - just a slight bump on the head and a few bruises.

Comprehensive management of vestibular disorders
Functional Anatomy History: Vestibular disorders (commonly known as dizziness) affect about 20% of the general population. Their prevalence increases with age: approximately 1/4 to 1/3 of the elderly complain of some form of dizziness. Dizziness can be related to a vast number of causal factors, but in most patients one or several of a handful of etiologies can easily be identified. The causes of dizziness are rarely secondary to a serious ailment. New facts: Traditionally, vestibular disorders have been evaluated and managed by various medical specialists, including otologists. Over the last three decades, otoneurology (or neuro-otology) has started to emerge as a new subspecialty within otolaryngology and, most recently, within neurology. Currently, there are only about a dozen neurologists in this country who specialize in the evaluation of vestibular disorders. The shift in specialty reflects the realization that many cases of dizziness don't require ear surgery and are actually neurological. Another factor prompting specialization in vestibular disorders is their presentation, which consists of a vague, evasive, and rather complicated set of symptoms. Dizziness can mean various sensations to different patients. It can present as vertigo (spinning), lightheadedness, near-fainting, disequilibrium, or a combination of these. Most commonly, dizziness is used by patients to mean any and all of the above. It is a subjective feeling, difficult to describe, and it can be caused by many classes of diseases. Almost half of patients with vestibular problems indeed have more than one type of dizziness.

Collaborator University of Pittsburgh The primary objective of this research is to increase our understanding of the combined effects of aging and vestibular disease on postural control in an effort to reduce the risk of falling in older adults. Patients with vestibular dysfunction commonly report a feeling of imbalance in situations with complex and rapidly changing visual stimuli such as in supermarkets. It is possible that reduced sensory function in older adults, or a slowing in central processing, may render the central integration of such information more difficult, resulting in greater imbalance. The proposed research will examine the postural sway response of older and younger adults, with and without vestibular disorders, as they view a moving visual scene. Understanding how older subjects with vestibular disorders integrate sensory information during these conditions will enhance our ability to develop balance rehabilitation programs in an effort to reduce the risk of falls in older adults.
