Ureterocele eversion with vesicoureteral reflux in duplex kidneys: findings at voiding cystourethrography

Ureterocele eversion refers to the sudden appearance of a bladder diverticulum at the site of ureterocele compression during voiding cystourethrography (VCUG). The radiologic appearance closely resembles a congenital bladder (paraureteral) diverticulum. Distinguishing ureterocele eversion with vesicoureteral reflux in duplex kidneys from congenital bladder diverticula with reflux is important for preoperative planning. This study describes the findings of ureterocele eversion and lower pole vesicoureteral reflux in duplex kidneys on VCUG and demonstrates how its appearance can be misleading. MATERIALS AND METHODS. Abstract Objective : We prospectively compared echocontrast cystosonography (ECS) using 'agitated saline' with micturating cystourethrography (MCU) for the detection of vesicoureteral reflux (VUR). Subjects and methods : Fifty-one children (39 boys and 12 girls) ranging from nine days to twelve years underwent ECS followed by MCU for the evaluation of VUR. Both procedures were performed on the same day and by the same radiologist throughout the study. After confirming the absence of urinary infection, children were catheterized under strict aseptic conditions. Baseline renal and bladder sonograms were obtained. The bladder was filled to capacity by slow injection of normal saline. Five to ten ml of air was then injected to generate microbubbles 'in vivo'. The sonographic criteria used for the diagnosis of reflux were (1) increase in dilatation and (II) visualization of moving microbubbles within the collecting system. No attempt at grading the reflux was made. The findings were compared with the 'gold standard' micturating cystourethrogram.

Vesicoureteral Reflux
What is vesicoureteral reflux (VUR)?
Vesicoureteral reflux occurs when urine in the bladder flows back into the ureters and often back into the kidneys. The bladder is the hollow, muscular organ that stores urine before urination occurs. The bladder has three small openings: two connect the ureters where urine is drained down from the kidneys, and one connects the bladder to the urethra where urine exits the body. The ureters are funnel-shaped tubes that carry urine from the kidneys. Ureters enter the bladder at a diagonal angle and have a special one-way valve system that normally prevents urine from flowing back up the ureters in the direction of the kidneys. When a child has vesicoureteral reflux, as this condition is most frequently diagnosed in infancy and childhood, the mechanism that prevents the back-flow of urine does not work, allowing urine to flow in both directions. A child who has vesicoureteral reflux is at risk for developing recurrent kidney infections, which, over time, can cause damage and scarring to the kidneys.
