Many years ago I worked with an infant with VATER syndrome. As with most syndromes, however, there are wide variations in the number of characteristics of the syndrome and the severity of each symptom. The little boy I worked with had anal atresia and general airway problems, along with a unilateral vocal fold paralysis. He clearly had fewer and less severe symptoms than your colleague's nephew. He was always an oral feeder, but required extensive therapy guidance to prevent aspiration and give him the needed caloric intake orally. I worked with him from the age of 7 months to 3 years. What is VACTERL association?
VACTERL or VATER association is an acronym used to describe a series of characteristics which have been found to occur together. V stands for vertebrae, which are the bones of the spinal column. A stands for imperforate anus or anal atresia, or an anus that does not open to the outside of the body. C is added to the acronym to denote cardiac anomalies. TE stands for tracheoesophageal fistula, which is a persistent connection between the trachea (the windpipe) and the esophagus (the feeding tube). R stands for renal or kidney anomalies and radial or arm anomalies. L is often added to stand for limb anomalies. Babies who have been diagnosed as having VACTERL association usually have at least three or more of these individual anomalies. There is a wide range of manifestation of VACTERL association so that the exact incidence within the population is not exactly known.

VATER Association is not a diagnosis. It is a combination of birth defects that are associated. This means that they tend to occur together. If a doctor sees two of the birth defects, he/she knows to check the other symptoms of VATER Association. VATER is an acronym in which each letter stands for a body part that may be affected. Anyone who has abnormalities in three or more of these body parts is said to fit in the category of VATER Association. V - Vertebrae A - Anus (and sometimes rectum) T E R Occasionally the following are also affected, resulting in acronyms such as VACTERLS, VATERS, and VACTERL. C L S Causes of VATER Association Most cases of VATER Association seem to occur sporadically throughout the population, although a few cases appear to run in families. The cause or causes for this group of symptoms are unknown at this time. No known environmental factor (drug, chemical, radiation) has been shown to cause VATER Association. Braddock syndrome is a new syndrome, described in sibs, associating VACTERL-like defects, pulmonary hypertension, abnormal ears, blue sclerae, laryngeal webs, and persistent growth deficiency. Patients show normal intellect, no tracheo-oesophageal fistula, and no anal atresia. Vital prognosis depends on the cardiac and pulmonary functions. This new syndrome has a probable autosomal recessive inheritance. March 2004.

Small-cell carcinoma of the ampulla of Vater and a paraneoplastic syndrome

Small-cell (oat-cell) carcinoma of the ampulla of Vater is rare. To our knowledge, no paraneoplastic syndromes have been associated with small-cell carcinoma at this anatomical location. In this report the clinical, pathological and biochemical findings in a patient with small-cell carcinoma of ampulla, associated with syndrome of inappropriate antidiuretic hormone (SIADH), are presented.

A firm, nodular tumour measuring 2 cm in size was found, which involved the ampulla and part of the pancreatic head. There was no lymphadenopathy and no evidence of hepatic metastases. Intra-operative fluids consisted of 4.5 litres of crystalloids (Plasmolyte 148 4 l, and normal saline (0.9% sodium chloride solution) 500 ml), 1 litre of colloid (Gelofusin, with sodium content of 77 mmol/500 ml) and 5 units of packed red cells.

I'm new to this but want to see if anyone out there has gone through this. My son had congenital scoliosis that we have been following since birth (Vater Syndrome). True to the Doctors word when he went through puberty the curves got very large very quickly. He had to have 12 vertebrae fused with 15 inch rods. He had this surgery on June 27th and did incredibly well. We were home from the hospital in five days.
However after the surgery he had very little appetite-we thought from the pain meds. Then on his fourth day home he began vomiting profusely. They ran a bunch of tests-upper GI, CatScans, etc-and they think due to the surgery and his weight loss he has Superior Mesenteric Artery Syndrome (where the artery creates a blockage at the doudenum).
We have been hospitalized twice in the past 3 weeks since the surgery, his weight at 5'7' has gone from 105 ot 88 pounds and now he is home with an NJ tube and we are feeding him via pump at night to get a weight gain. He still can handle very little by mouth (popsicles, gatorade).
Kevin has been a healthy kid other than his scoliosis since he was four (prior to that he had a tracheostomy).
