Prof. Strandness passed away on January 7, 2002, at the age of 73 years. However, his legacy as a vascular surgeon and clinical researcher remains alive through numerous scientific papers and books, particularly the book Duplex Scanning in Vascular Disorders, whose translation into Portuguese is the object of this review. The book Ecodoppler Colorido nas Doenças Vasculares , published by Guanabara-Koogan in 2004 is the translation of the third American issue of Duplex Scanning in Vascular Disorders
The list of the book authors reflects a characteristic of vascular laboratories in the USA. In that country, vascular technologists are responsible for performing Doppler ultrasound examinations. Such technologists, who have a high level of training, perform the exams, choose the proper images, write and sign the reports, which are also signed by a physician, who is technically responsible for the laboratory. In Brazil, on the contrary, it is a physician who almost always performs the examination. Due to these American characteristics, all authors of the book are vascular technologists or nurses, except Prof. Strandness and Dr. Kirk, his right-hand man.

There are numerous causes of vascular disorders of the upper extremity and hand, and to understand them it is valuable to classify vascular disorders into acute or chronic categories.
    Acute Vascular Disorders
    Acute injury due to trauma is covered extensively in the replantation section of this site and in the free flap section where examples of severe vascular injury are presented. A subcategory of acute injury is iatrogenic disease due to radial artery catheterization or drug injection. Both are treated with the same principles as major vascular injury. Compartment Syndrome is a condition of increased pressure in the hand or forearm that causes acute loss of blood flow and nerve injury. Treatment is directed toward treating the increase pressure which causes loss of blood flow, and often requires fasciotomy.

    Chronic vascular disorders tend to develop more slowly and have a tendency, but not always, to be seen in an older population.

    Warren E. Morgan, M.D. Bleeding disorders may complicate many routine otolaryngology procedures. This may result from technical errors, but bleeding associated with defects in the coagulation system may be difficult to control. All otolaryngologists should be aware of common bleeding disorders and their treatment. Normal coagulation may be divided into three phases: vascular response, platelet activation, and the coagulation cascade. The initial vascular response of vasoconstriction occurs immediately after the injury. Platelet adherence occurs in response to collagen exposure by endothelial damage. Further platelet adhesion is mediated through specific platelet receptors and mediators creating an unstable hemostatic plug. Coincident with the platelet activation, the coagulation cascade is initiated by the release of tissue thromboplastin and contact-activating factors. The coagulation cascade forms a fibrin matrix that reinforces the platelet plug.

          Yale-New Haven Hospital and the Yale University Department of Radiology offer three one-year fellowships in this subspecialty. Experience at both Yale-New Haven Hospital and the West Haven Veterans Administration Hospital is included during this year. Fellows receive training in all areas of diagnostic angiography other than the coronary or intracranial distribution. Vascular interventional techniques include arterial and venous revascularizations (balloon angioplasty, thrombolysis, and stenting), transcatheter embolotherapy, and percutaneous inferior vena cava filter placement. Additional vascular experience includes the management of stenotic and occluded hemodialysis accesses, percutaneous creation of intrahepatic portosystemic shunts, and the placement of a variety of venous access devices, including tunneled and implantable ones. Nonvascular procedures include the percutaneous management of abscesses, gastrostomies, radiofrequency ablation of tumors, and interventions in the biliary and urinary systems, such as external drainage, internal stenting, dilatation, and lithotripsy. The section also has two endoscopes for percutaneous diagnosis and intervention, principally in the biliary system.
