Introduction This information sheet is for people with vascular dementia and their carers. It includes medical information about the diagnosis and treatment of vascular dementia, and practical information for carers. If you require information about other issues regarding dementia call the freephone Dementia Helpline on 0808 808 3000 , or search this website. What is vascular dementia? Dementia is the general term for a gradual progressive decline in a person’s memory and other mental abilities. Vascular dementia is the second most common cause of dementia and recently its incidence has been found to be higher than previously expected. The most common type of vascular dementia is multi-infarct dementia (MID) where the brain has been damaged by repeated small strokes. However, vascular dementia can be caused by a number of other conditions including high blood pressure (hypertension), irregular heart rhythms (arrhythmias) and diseases which cause damage to the arteries in the brain. Indeed, any condition which causes the circulation to the brain to be impaired or damaged carries a risk to mental abilities.

CADASIL is the English acronym for Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy. Recognized more than a decade ago as a new cerebral vasculopathy , and maybe reported 40 years ago by Van Bogaert as an aggressive form of Binswanger’s disease , CADASIL has been continously diagnosed in the last years, with more than 50 affected families reported
Pathogenesis
CADASIL still has an unknown pathogenesis but alterations in the q12 region of chromosome 19 have been detected in some cases . Histopathologically, a characteristic eosinophilic granular deposit of an unknown composition has been present in the media of the small penetrating cerebral arteries. This seems to cause vascular obliteration and, consequently, multiple infarcts, mainly in the white matter, basal ganglia and leptomeninges . Extracerebral vascular abnormalities have also been noted

Classification of vascular dementia in the Cardiovascular Health Study Cognition Study
Lopez OL, Kuller LH, Becker JT, Jagust WJ, DeKosky ST, Fitzpatrick A, Breitner J, et al.; Neurology 2005; 64 (9); 1539-1547 Objective:  To describe the diagnostic classification of subjects with incident vascular dementia (VaD) participating in the Cardiovascular Health Study (CHS) Cognition Study. Methods:  The CHS classified 480 incident cases between 1994 and 1999 among 3,608 CHS participants who had brain MRI in 1992 through 1994 and in 1997 through 1998. The patients were diagnosed before and after reviewing the brain MRI. Results:  The pre-MRI classification showed that 52 participants had VaD and 76 had both Alzheimer disease (AD) and VaD. The post-MRI classification showed that the Diagnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV) criteria classified 61 subjects as having VaD, the National Institute of Neurological Disorders and Stroke-Association Internationale pour la Recherche et l’Enseignement en Neurosciences (NINDS-AIREN) criteria classified 43 subjects as having probable VaD and 10 as possible VaD, and the State of California Alzheimer’s Disease Diagnostic and Treatment Center (ADDTC) criteria classified 117 as having probable VaD and 96 as possible.

NINDS - AIREN criteria for the diagnosis of vascular dementia I. The criteria for the clinical diagnosis of probable vascular dementia include all of the following:
  • Dementia defined by cognitive decline from a previously higher level of functioning and manifested by impairment of memory and of two or more cognitive domains (orientation, attention, language, visuospatial functions, executive functions, motor control, and praxis), preferable established by clinical examination and documented by neuropsychological testing; deficits should be severe enough to interfere with activities of daily living not due to physical effects of stroke alone.

Practice Pointers Donepezil, a cholinesterase inhibitor, is somewhat effective in the treatment of mild to moderate cognitive impairment caused by Alzheimer's disease. Malouf and Birks examined the evidence from two similar manufacturer-sponsored studies of donepezil in a total of 1,219 patients with vascular cognitive impairment (sometimes called "multi-infarct dementia" or "vascular dementia"). The prevalence of dementia in those aged 65 years or over in China is 3.9%, of which vascular dementia accounted for 69% in 1989 [1]. A subsequent study has confirmed that multi-infarct dementia occurs more frequently than primary degenerative dementia, with a ratio of 3:2 [2]. Apart from a few individual case studies, there were few reports of attempts to investigate the treatment of vascular dementia until the early 1990s [3], when a clinical research programme to explore the use of Chinese herbal medicine for this condition was started at Beijing Dongzhimen Hospital.

    kcrary This is a difficult choice but there are some medical facts to help you. The surgery itself should have no effect at all on the dementia or Alzheimers.
    The question about surgery is much more difficult to respond to because it is urological problem rather than a psychological problem. One thing you can do is ask for a second opinion from another urologist.

    Last Updated: 2004-10-19 17:42:42 -0400 (Reuters Health) NEW YORK (Reuters Health) - Diabetes increases the risk of dementia, especially vascular dementia, in the elderly, according to new data from the Kungsholmen Project, a community-based longitudinal study on aging and dementia. "The risk for dementia and vascular dementia is especially high when diabetes mellitus occurs together with severe systolic hypertension or heart disease," investigators note in the October 12th issue of the journal Neurology. Among 1301 individuals 75 years of age or older who were free of dementia at baseline, 350 developed dementia, including 260 cases of Alzheimer's disease and 49 cases of vascular dementia, over 5584 person-years of follow up (6 years). According to Cox proportional modeling, diabetes mellitus increased the risk of dementia, particularly vascular dementia, independent of other vascular factors, with hazard ratios of 1.5 and 2.6, respectively. The hazard ratios for dementia and vascular dementia were even higher in patients taking oral antidiabetic agents, 1.7 and 3.6, respectively. Dr. Chengxuan Qiu from the Stockholm Gerontology Research Center and co-authors of the report "failed to find a relevant association between diabetes mellitus and Alzheimer's disease risk."
