    Tissue Engineered Heart Valves: A Potential Cure for Valvular Heart Disease
    Home Innovation Centers Valve Technology Since the advent of surgery for valvular heart disease, scientists and surgeons have sought the ideal valve prosthesis when replacement is required. Over the span of four decades, significant advancements in valve design and construction include increased use of pyrolytic carbon and other inert materials with increased durability, less obstructive designs, better processing of tissue valves for added longevity, and stentless constructs including homografts, autografts, and xenograft tissue bioprostheses. None of these advancements, however, can be regarded as truly revolutionary. All valve replacement options have advantages and disadvantages which are discussed with patients on a daily basis.
    The ideal valve substitute in the future may arise from tissue engineering, a science that combines the principles of bioengineering, cell transplantation, hematology, and material science. This field is in its infancy, with many more dreams than reality, but with incredible potential to revolutionize treatment for heart disease. A small step in the application of tissue engineering for heart valves has been the creation of the Synergraft decellularized homograft and xenograft aortic prosthesis. Cryopreserved and completely decellularized, these valves have the theoretical advantage of allowing for repopulation of the matrix by host cells which provide the functions of a living structure: growing, healing (regeneration), resistance to infection (immune function), and creation of extracellular substances such as matrix and biologically active compounds. While histologic studies have demonstrated recipient cells within these types of grafts [

Background
The serotoninergic drugs fenfluramine and its d-isomer dexfenfluramine as well as the adrenergic agonist phentermine have been increasingly prescribed for the treatment of obesity. All of these medications have been implicated in the development of pulmonary hypertension, possibly through a serotonin-related mechanism. Serotonin has also been implicated in the development of specific cardiac valvular lesions seen in the carcinoid syndrome and in patients taking ergotamine preparations. This is the first report of similar cardiac valvular problems in patients taking the combination of fenfluramine and phentermine.
Methods
Identification of cases This was not a case-control study, and there was no clearly defined methodology for case finding. In May, 1996 a 41 year old woman underwent mitral valve repair at the Mayo Clinic for severe mitral regurgitation. At surgery, the valve had a glistening white appearance, suggestive of ergotamine-induced damage, but there was no history to support this. Two months later the same patient was found to have severe tricuspid regurgitation and thickened leaflets; at this time a history was obtained indicating fenfluramine-phentermine use for 25 months prior to her surgery.

There are four major valves in a person's heart that direct blood flow forward through the  chambers of the heart . Heart valve disease can occur with any one or a combination of the valves, and it will often lead to heart failure if left untreated.
Diseases of the mitral or aortic valves (the valves of the left side of the heart) are most common affecting more than 5 percent of the population. Heart valve disease implies that a valve either fails to open properly (stenosis) or fails to close properly allowing backward flow of blood (regurgitation). Heart valves can have both malfunctions at the same time (regurgitation and stenosis). When heart valves fail to open and close properly, the implications for the heart can be serious, possibly hampering the heart's ability to pump blood adequately through the body. Heart valve problems are one cause of heart failure.
At Stanford, we repair rather than replace heart valves whenever possible to preserve a patient's own tissues so that blood thinning (anticoagulation) medications are not required. Repaired valves do not deteriorate like other valve replacements. When patients have combined disease of both the aortic valve and the ascending aorta (the large vessel that houses the aortic valve and takes blood out of the heart), we strive again to replace the diseased aorta and repair the aortic valve using a technique known as "valve-sparing aortic root replacement."

What Causes Valvular Heart Disease? Valve disease can be congenital, and congenital valve disease most often affects the aortic or pulmonic valve. The disease can also develop after birth and during adulthood. In some instances, the cause of valve disease is unknown, but some of the known causes include: coronary artery disease, heart attack, cardiomyopathy, rheumatic fever, syphilis, hypertension, aortic aneurysms, connective tissue diseases and endocarditis (a bacterial infection that enters the blood and attacks the heart valves). Heart valve disease can reduce the pumping efficiency of the heart and eventually cause heart failure. The valvular heart disease found in these patients was similar to the damage found in patients who took the diet drugs Redux and Pondimin ( fen-phen Redux and Pondimin were withdrawn from the market following reports of serious valvular heart damage. Valvular heart disease is a condition which impairs heart function. The heart functions as a one-way pump to move blood through the lungs and the body. Within the heart, one-way valves work to prevent the backward flow of blood in the heart. In patients with valvular heart disease, one or more of these valves fails to seal properly and allows significant amounts of blood to flow backwards, or regurgitate. Valvular regurgitation increases the workload on the heart and, in sufficiently severe cases, can require heart valve replacement surgery. Left unchecked and untreated, some types of valvular heart disease can progress to the point where the patient is at risk of death from heart failure.
