Peer Review Status: Internally Peer Reviewed (1) Introduction Urticaria, described by the patient as itchy welts, can appear as 2 to 10mm raised papules surrounded by erythema that involve the superficial portion of the dermis. When the mediator release (primarily histamine) from mast cells occurs deeper in the dermis and subcutaneous tissue, the result is angioedema or swelling. (2) Acute Urticaria and Angioedema Acute urticaria (and/or angioedema) is very common, occurring at some point during the lifetime of about a quarter of the population. In about a third of the cases, the cause is a food or drug, and the patient makes the obvious connection. One example would be the presentation of hives immediately after eating shrimp or beginning an antibiotic. In the remainder of the cases, no cause is ever found. The problem is usually self-limiting and lasts less than 4 weeks.
  • Atarax or Benadryl are the standard symptomatic treatment for urticaria. Unfortunately, sedation often is a side effect with these medications.

Hives (Urticaria)
A friend told him about National Jewish so he decided to fly out and become a patient. After a thorough history and physical examination, the doctors there immediately put him on a restricted diet of a non-allergenic protein drink. Within forty-eight hours, his hives were gone. Further tests and diet modifications identified the source of his chronic hives: an allergy to wheat. With the help of nutritionists who were part of his comprehensive treatment team, the man modified his diet and resumed a normal, happy life without itchy, debilitating hives. Although this scenario is fairly straightforward and ends happily, finding the cause of chronic urticaria can be very challenging. in the Southwest Hives (urticaria) and angioedema Urticaria is an itchy rash consisting of inflamed wheals or welts that are raised above the level of the surrounding skin. The wheals vary in size and shape, and tend to come and go within a period of less than 24 hours. The whole lip or eyelid may swell when wheals involve them. When this process involves the tissues under the skin, a more diffuse and extensive swelling known as angioedema may occur. If this causes airway obstruction or is associated with a fall in blood pressure, the condition is termed anaphylaxis . Eczema or atopic dermatitis is a different condition in which affected skin is usually rough and inflamed, remaining unchanged in appearance over several days. Short-lived attacks of urticaria and/or angioedema may be caused by allergy to a food or a medication, and in young children may be provoked by infection. In some cases the problem may be provoked by physical factors such as cold, scratching, pressure on the skin or sweating. More rarely, exercise, heat, sunlight or mechanical vibration may provoke attacks. Treatment for angioedema associated with urticaria is usually the same as the treatment for urticaria. When the condition lasts for months it is the classified as chronic : causes that are external to the body are almost never found in these patients. Approximately one half of patients with chronic urticaria have been found to have an antibody in the blood that attacks

Relevant Fundamentals of Skin Structure and Function
The skin is not an absolutely impermeable membrane. It consists of a layer of horny dead cells overlying living (and very vulnerable) ones. It is fairly impermeable to water splashes but continuous immersion in water with friction will macerate it. Organic fat solvents can go through the skin fairly quickly. Inorganic caustics (strongly alkaline compounds) will damage the skin very readily by sparifying fat and hydrolysing protein. The skin has a very good capacity to regenerate provided the deepest layers are not irretrievably damaged. However while it is still damaged and not yet fully healed it remains vulnerable to damage and can be made worse by many agents which the normal skin could easily have withstood.

Detta är den vanligaste formen. Klåda och rodnade eller ljusa upphöjda kvaddlar i huden, kommer då på några minuter eller timmar, efter det att något utlösande ämne har tillförts kroppen. Ibland kan även lätt feber, svullnadskänsla i handflator och fotsulor, ledvärk eller magbesvär förekomma. I svårare fall kan svullnader i ansiktet uppkomma. Exempel på orsaker till akut urticaria är födoämnen (skaldjur, musslor, nötter, jordgubbar, kiwi, mögelost m m), färgämnestillsatser i sprit och godsaker, konserveringsmedel i läsk, dressing och såser, läkemedel (antibiotika, värktabletter med acetylsalicylsyra eller kodein, B-vitaminer, röntgenkontrastmedel m m), eller insektsstick (bi, geting). Akut urticaria visar ofta kraftiga symptom med klåda och utslag, men klingar sedan vanligen av efter någon eller några veckor. Kronisk urticaria
Urticaria som varar mer än 6 veckor kallas ”kronisk” och kan pågå i flera månader, eller i sällsynta fall ett eller flera år. Den kan starta akut på grund av någon ovan nämnd orsak, och sedan fortsätta trots att den utlösande faktorn undvikes.
