Urethritis - chronic
Definition
Chronic urethritis is an inflammation (irritation with swelling and presence of extra immune cells) of the urethra (the tube that carries urine from the body) that continues for weeks to months.
Alternative Names
Urethral syndrome
Causes
Chronic urethritis is usually caused either by a bacterial infection or structural problem (narrowing of the urethra). It may also be associated with a variety of systemic diseases, including emotional disorders. Urethritis may occur in either women or men. The major symptoms of urethritis

Non-Specific Urethritis
What is it?
Only men get this. It is an inflammation of the tube (urethra) which carries the urine from the bladder to the tip of the penis.
How does it show itself?
Many men notice a discharge coming from the end of the penis with pain, irritation or discomfort on passing urine.
Where does it come from?
It is called "non-specific" because there are a range of different causes.
  • Chlamydia. This is a common cause. It is a germ passed between sexual partners but may take weeks, months or even years in some cases to show up. Most women with it may never know they have it. For both men and women it can cause a lot of damage if it is not treated i.e fertility problems. Other sexually transmitted infections. Reaction to vaginal infections. Some men develop NSU as a response to a simple infection in their partner who may not even know they have one. Injury. The urethra is delicate and may be damaged during vigorous sexual activity leading to NSU. Squeezing the penis or using disinfectants etc if worried about having an infection might cause NSU. Urine/bladder infection. This is quite an unusual cause of NSU in younger men

Non-Gonococcal Urethritis (NGU)
If you suspect that you have non-gonococcal urethritis (NGU), see a doctor even if symptoms go without treatment. Antibiotics usually clear the infection. Your sexual partner should also be treated, even if they have no symptoms. You should not have sex until you have finished treatment.
What is urethritis? The urethra is the tube between the bladder and the end of the penis. Urethritis means inflammation of the urethra.
  • Gonococcal urethritis is caused by a bacterium (bug) called gonorrhoea. This is one type of sexually transmitted infection. A separate leaflet describes gonorrhoea in more detail.
  • Non-gonococcal urethritis (NGU) is due to causes other than gonorrhoea. This is sometimes called non-specific urethritis (NSU). This leaflet describes NGU further.
  • Some men have both gonococcal and non-gonococcal urethritis at the same time.
What causes non-gonococcal urethritis?
  • Infection with chlamydia causes about half of cases of NGU. Chlamydia is a a sexually transmitted infection. You can pass chlamydia on during vaginal, anal or oral sex.
  • Various other bacteria or viruses which are sexually transmitted can cause NGU.

Urethritis and Urethral Discharge in Men
If you have a urethral discharge you are likely to have a sexually transmitted infection which you can pass on to others. See a doctor, even if symptoms go.
What is urethritis and what causes it? Urethritis means inflammation of the urethra. The urethra is the tube between the bladder and the end of the penis. Urethritis is usually caused by a sexually transmitted infection, but not always. It can occur within a stable relationship.
  • Gonorrhoea is one type of sexually transmitted infection which can cause urethritis. A separate leaflet describes gonorrhoea in more detail.
  • Non-gonococcal urethritis (NGU) is the term used to describe urethritis caused by anything other than gonorrhoea. A bacteria called chlamydia is the common cause of NGU. This is one type of sexually transmitted infection. NGU can also be caused by a variety of other bacteria or viruses (bugs). NGU may rarely have a non-infective cause. For example, injury or surgery to the urethra can cause inflammation. A separate leaflet describes NGU in more detail.
Some men have a combination of gonococcal and non-gonococcal urethritis at the same time.

    Non-specific urethritis
This little-known male STI could be to blame if you're experiencing penis discharge and pain when you pee.
    What is it?
Non-specific Urethritis (NSU) is a common STD found in males. It is thought to be caused by bacteria including chlamydia and ureaplasmas, and rarely by the herpes virus. It is not a very well understood condition, and the exact cause is often never found.
    What are the symptoms?
    • A slight discharge from the end of the penis, usually clear or greyish Sometimes pain or irritation during urination
    Tests and treatment
At the clinic, a swab of the discharge is taken and the genital area is examined by a doctor or nurse. Treatment is with a short course of antibiotics, such as erythromycin or tetracycline. Avoid sex and alcohol during the treatment.
    Complications
Rarely, NSU can flare up again without warning, and a different course of antibiotics is needed. If left untreated, it can lead to boils or abscesses around the scrotum and testicles, inflammation of the prostate gland, or narrowing of the urethra (urine tube inside the penis). If you think you might have a sexually transmitted infection, sort it out straight away. Go to a genitourinary medicine (GUM) clinic, available at most large hospitals. GUM clinics give free and confidential advice and treatment. Or you can make an appointment to see your GP.

What is urethritis?
What is proctitis?
Proctitis means inflammation of the rectum. It is caused by the same germs which cause urethritis (see above) - with the addition of herpes simplex and various intestinal parasites such as amoebae. Also, any substances including chemicals can be irritating to the anal lining.
How are the germs transmitted?
What are the symptoms?
Proctitis - again, symptoms can be very mild or totally absent, especially if you have had the problem for a long time. Symptoms to look out for are diarrhoea, mucus streaking of stools, anal pain and a sensation of needing to go the the toilet again shortly after your last visit.
How do I know if I have urethritis or proctitis?
The test for proctitis involves the doctor gently looking at the appearance of the anal lining through a small instrument called a proctoscope. This is the best way to diagnose proctitis, but if you prefer you may simply have some swabs taken from the entrance to the anus - these can screen for gonorrhoea and chlamydia.
What is the treatment?

Urethritis
Non specific urethritis is the term for a sexually transmitted urethritis not caused by N. gonorrhoeae gonorrhoea ]. In developed countries, Chlamydia trachomatis serovars D to K is the dominant pathogen, being thought to be the cause of around 50% of cases of symptomatic non specific urethritis in men, with various other organisms including Mycoplasma genitalium and Ureaplasma species [ Horner et al Taylor-Robinson, 2002 ] and Trichomonas vaginalis Pepin et al ] and perhaps the anaerobic bacteria of bacterial vaginosis in women [ Keane et al ] also involved. In Africa , by contrast, M. genitalium appears to be much more commonly associated with non specific urethritis, particularly in Central Africa [ Morency et al ]. Fortunately, M. genitalium is sensitive to similar antibiotics to C. trachomatis The incubation period for chlamydial urethritis is approximately 7 - 14 days [ Stamm et al ], whereas gonococcal urethritis has a shorter incubation period of the order of 4 days.
