Definition Urethritis is an inflammation (irritation with presence of extra immune cells) of the urethra.
Female Urinary Tract
Male Urinary Tract Urethritis may be caused by the same organisms that cause urinary tract infections (for example, E. coli or klebsiella) and by some sexually transmitted diseases (for example, chlamydia gonorrhea , and Ureaplasma urealyticum infections). Viral causes of urethritis include herpes simplex virus and cytomegalovirus. Urethritis may also develop because of a chemical irritation (such as spermacide in condoms or contraceptive jelly, cream, or foam). Trauma may also cause urethritis. Men between the ages of 20 and 35, those with multiple sexual partners, and those who engage in high-risk sexual behavior (such as lack of condom use, particularly during active anal intercourse) are at higher risk.

Abstract: A retrospective population-based case-control study of sexually transmitted urethritis was conducted at a large military base over a 21-month period. During the study, 9514 patients were seen for sexually transmitted disease. The analysis was restricted to active duty males and showed that Blacks had 14.8 times the incidence rate for gonococcal urethritis (GCU) and 4.7 times the rate of nongonococcal urethritis (NGU) compared to Whites. There were slightly fewer cases of NGU than GCU. A case-control study of active duty soldiers were 1.65 times as likely to have NGU as uncircumcised subjects (95% CI): 1:37-2.00). However, circumcision was not associated with an increased incidence of GCU. What is NGU? NGU (NonGonococcal Urethritis) is an infection of the urethra caused by pathogens (germs) other than gonorrhea. How common is NGU? Several types of germs cause NGU, the most common and serious is chlamydia. Chlamydia is very common in both males and females. The diagnosis of NGU is more commonly made in males than in females, mainly due to the anatomical differences. Germs that can cause NGU include but are not limited to:
  • Chlamydia trachomatis (most common) Ureaplasma urealyticum Trichomonas vaginalis (rare) Herpes simplex virus (rare) Adenovirus Haemophilus vaginalis Mycoplasm genitalium
How can I get NGU?
  • Sexual: Most germs that cause NGU can be passed during sex (vaginal, anal or oral) that involves direct mucous membrane contact with an infected person. These germs can be passed even if the penis or tongue does not go all the way into the vagina, mouth or rectum, and even if body fluids are not exchanged.

Nongonococcal Urethritis (NGU)
What is Nongonococcal Urethritis?
Urethritis is an infection of the male urethra, or the tube that carries urine from the bladder to the outside of the body. Nongonococcal means it is not caused by gonorrhea. To find out if you have nongonococcal urethritis (NGU), you must be examined by a doctor or nurse and have lab tests done. NGU is most commonly caused by a bacterial infection, but it may also be caused by a virus. One of the causes, chlamydia, can lead to complications in men and women. See BC HealthFile #08l Chlamydia for more information.
How is it spread?
What are the symptoms?
Symptoms in men may include a slight burning feeling when urinating, an itching inside the penis, or a clear to creamy white fluid from the urethra. Some men with NGU show no symptoms, or mild symptoms that may be unnoticeable. Usually, the symptoms are seen or experienced more in the morning.

Urethritis, an inflammation of the urethra with or without discharge, is most commonly caused by chlamydia or gonorrhea, but can also be caused by Ureaplasma urealyticum or Trichomonas vaginalis . Urethritis as a result of chlamydia or other infections tends to be more subtle in presentation than gonococcal urethritis. This patient presented with urethral itching and a clear urethral discharge - 10 days after unprotected intercourse. Symptoms presenting more than one week after unprotected sex tend to be non-gonococcal in nature. photo reproduced with the permission of Dr. Marc Steben

NGU refers to an infection of the urethra (the tube running from the bladder through which urine passes) caused by germs other than those that cause gonorrhea. Infection can be caused by several different organisms, although the most frequent cause of NGU is a germ called Chlamydia, (Kla-mid-ee-ya), which is a sexually transmitted disease (STD). Urethritis What is Urethritis? Urethritis is an inflammation of the the urethra (the tube that carries urine out of the body). What causes Urethritis? Urethritis can be caused by a mechanical injury, an irritating chemical (antiseptic or spermacide), or by an infection. Doctors usually classify infectious Urethritis under one of two categories: Gonococcal urethritis - this is caused by gonorrhea bacteria. Non-gonococcal urethritis - this is caused by chlamydia. Other possible causes of this are Mitoplasma genitalium, Trichomonas vaginalis, Ureplasmic urealyticum, and Herpes simplex virus. What are the symptoms of Urethritis? 75% of women and half of men with Non-gonococcal urethritis infection have no symptoms. Due to this, a person could spread around chlamydia and not know it. When Non-gonococcal urethritis does have symptoms, they tend to be like gonococcal urethritis. These include: redness around the urethra, frequent urination, a discharge from the end of the penis, and a burning sensation when urinating. How do I prevent myself from getting Gonococcal Urethritis?

Nongonococcal urethritis
Definition
Any inflammation of the urethra not due to gonorrhea , almost always contracted through sexual intercourse and found far more often in men. Description
Men between the ages of 15 and 30 who have multiple sex partners are most at risk for nongonococcal urethritis (NGU), which is believed to be the most common sexually transmitted disease in the United States.
NGU is spread almost exclusively via sexual contact, and appears most often in men because a woman's urethra is less easily infected during sex. The infection is most often due to Chlamydia trachomatis , the organism that causes chlamydia. Those that aren't caused by Chlamydia trachomatis are usually due to another bacterium, Ureaplasma urealyticum . In 10% to 20% of NGU cases, the cause is unknown. Symptoms appear within one to five weeks after infection, and include a slight clear discharge (the color of the discharge can vary from one patient to the next), and itching or burning during or after urination.

Urethritis
Definition
Urethritis is an inflammation of the urethra that is usually caused by an infection. Description
The urethra is the canal that moves urine from the bladder to the outside of the body. When this canal becomes infected, inflammation occurs due to the accumulation of white blood cells in the area. When this occurs, it is called urethritis. Besides the urethra, the urinary tract consists of the bladder, ureters, and kidneys. Inflammation can move up the urethra, causing cystitis in the bladder, or nephritis in the kidneys. Collectively, these inflammations are called urinary tract infections or UTIs. Urinary tract infections are much more common in women than in men, probably due to anatomy. Infections are especially more common in older women, due to bladder problems.
Uncomplicated urethritis usually results from infection by the bacteria Escherichia coli , commonly found in the bowel. Complicated urethritis can occur when other problems exist, such as kidney stones , malformations of the urinary tract, spinal cord injury , or a compromised immune system. People with diabetes tend to have more urinary tract infections, as well as hospitalized patients. Urinary tract infections can also be sexually transmitted. Some people seem to be susceptible to urinary tract infections, having them recurrently.

Nonsexual causes of NGU may include urinary tract infections, an inflamed prostate gland due to bacteria (bacterial prostatitis), a narrowing or closing of the tube in the penis (urethral stricture), a tightening of the foreskin so that it cannot be pulled back from the head of the penis (phimosa), or as the result of a process such as inserting a tube into the penis (catheterization), During birth, infants may be exposed to the germs causing NGU in passage through the birth canal. This may cause the baby to have infections in the eyes (conjunctivitis), ears and lungs (pneumonia).
