Diagnosis
Evidence of urethritis, i.e. 5 or more polymorphs (white blood cells involved in the inflammatory process) per high power field on microscopy, and the absence of gonorrhoea and chlamydia.

DEFINITION In genito-urinary medicine, urethritis is broadly divided into gonococcal and non-gonococcal. Non-gonococcal urethritis (NGU) in male is referred to as inflammation of the urethra not caused by Neisseria Gonorrhoeae but some specific identifiable pathogens e.g. Chlamydia and ureaplasma or some non-specific unidentificable or unknown pathogens. Non-specific urethritis (NSU) refers to urethritis in which no identifiable pathogens have been found. Post-gonococcal urethritis refers to urethritis occurred after curative therapy for urethra gonorrhoea. In female, non-specific genital infection (NSGI) is a more preferred term as other than the urethra, vagina and cervix are the common sites for genital infections.

It can be caused by a number of different organisms, the most significant one being chlamydia. NGU not due to chlamydia may be due to exposure to normal vaginal or throat organisms.
    What are the symptoms?
    Men may experience a urethral discharge, pain when urinating, or urethral irritation. These symptoms may be more noticeable in the morning. Symptoms may develop between one and five weeks after the likely acquisition of infection. Some men are unaware of any signs or symptoms.

    What is NGU?
    NGU (non-gonococcal urethritis) also known as non-specific urethritis (NSU) is one of the most common diseases men can catch from sex. It can be caused by a number of different bacteria including a bacteria called chlamydia. NGU not due to chlamydia may be due to normal vaginal or throat organisms as well.
    How does someone get infected?
    NGU is passed on, in most cases by vaginal, anal or oral sex without a condom with someone who has the infection. How do I know if I have NGU? Men may have a discharge coming out of the penis, pain when weeing, or irritation inside the penis. These symptoms may be more noticeable in the morning. Symptoms may come on between one and five weeks after being infected. Some men have no symptoms at all.

    Urethritis What is urethritis?
    Urethritis refers to irritation or inflammation of the urethra. The urethra is the tube that empties urine from the bladder. In men the urethra extends the full length of the penis. In women the urethra is much shorter, about an inch long, and the opening is just above the vagina. Because the opening of a woman's urethra is so close to the vagina and anus, bacteria can easily enter it from these areas. How does it occur?
    Most often, especially in males, urethritis results from a sexually transmitted disease (STD). The two most common STDs associated with urethritis in males are chlamydia and gonorrhea . However, other STDs can cause these symptoms as well, including herpes simplex virus and the human papilloma virus. Less serious STDs such as trichomoniasis can cause symptoms also. In most older women, the tissues of the urethra and bladder become thinner and drier after menopause. This is the usual cause of urethritis in older women. Urethritis may at times simply be an irritation caused by rubbing or pressure on the groin area. For example, tight clothing can cause urethritis. It may also result from an irritant such as soap, powder, or spermicides. Some people can't use spermicides for this reason. Sometimes vaginal infections (for example, a yeast infection) cause women to have symptoms of urethritis.

    Presentation
    A male complains of dysuria, a burning discomfort along the urethra, or a urethral discharge. A copious, thick, yellow- green discharge which stains underwear is characteristic of gonorrhea, whereas a thin, white, scant discharge with milder symptoms is characteristic of chlamydia. Urethritis in a female may be asymptomatic or indistinguishable from cystitis or vaginitis, or may be manifest as UTI symptoms with a low concentration of bacteria on urine culture, or tenderness localized to the anterior vaginal wall. In addition to increased vaginal discharge, women may have intermenstrual bleeding, especially postcoital spotting and cervical friability.
    What to do:
    • Gram stain any urethral discharge, looking for gram- negative diplococci inside white cells, which imply gonococcal infection.
    • Order a serologic test for established syphilis. Further antibiotic treatment is required if the RPR or VDRL is positive .
    • Examine the urine sediment for swimming protozoa, implying infection with Trichomonas vaginalis, best treated with metronidazole (Flagyl) 250mg qid x 7d, or 2gm po once.

I. Etiology Gonococcal urethritis: Neisseria gonorrhoeae. Nongonococcal urethritis: Chlamydia trachomatis Ureaplasma urealyticum Trichomonas vaginalis Herpes Simplex Virus Candida albicans Miscellaneous bacteria Mycoplasma genitalium Bacteroides urealyticum Other ( E. Coli , Haemophilus species) Unknown II. Epidemiology Urethritis affects an estimated 4 million American men each year. NGU is more common than GU in the U.S. and in much of the developed world; GU may account for up to 80% of the cases of acute urethritis in some undeveloped areas. The ratio of NGU to GU is greater among groups of higher socioeconomic status in the U.S. For example, comparisons of men with NGU versus GU show that those with NGU more often are white, with higher education, higher socioeconomic status, and fewer number of sexual partners. On college campuses, more than 85% of urethritis is nongonococcal.
