Peptic ulcer
Definition: Ulcers are erosions in the lining of the stomach or duodenum (the first part of the small intestines). An ulcer in the stomach is called a gastric ulcer . An ulcer in the duodenum is called a duodenal ulcer. Together, ulcers of the stomach and duodenum are referred to as peptic ulcers. Most ulcers are erosions of the first layer of the inner lining. If the hole goes all the way through, this is called a perforation of the intestinal lining. This can cause shock and is a medical emergency.

Causes, incidence, and risk factors: Normally, the lining of the stomach and small intestines have protection against the irritating acids produced in your stomach. For a variety of reasons, the protective mechanisms may become faulty, leading to a breakdown of the lining. This results in inflammation ( gastritis ) or an ulcer.

It is thought that the pathogenesis involves complex interplay between stomach acid secretion and chronic inflammation induced by Helicobacter pylori infection. It is not known to be caused by spicy foods (i.e., capsasin) or stress. Incidence

Symptoms
  • Dull, gnawing ache (often occurs 2-3 hrs after meal and in the middle of the night) Weight loss Poor appetite Bloody or black stools Bloody vomit or "coffee ground" like vomit
Pathophysiology
  • H. pylori infection is thought to be responsible for the majority of peptic ulcers. Not all individuals infected with H. pylori eventually develop ulcers. H. pylori weakens the protective mucous coating of the stomach and duodenum, resulting in acid penetration into the underlying sensitive lining. Interestingly, H. pylori secretes enzymes that neutralize stomach acid, hence prolonging its survival.

Most ulcers can be easily treated. However, when complications develop they can pose a serious threat to health. Patients can be treated with drugs or through surgery, but there is no firm evidence that this works. Bleeding ulcers can be a sign of general poor health. What is an ulcer? An ulcer is a sore or a hole that forms in the lining of the stomach or small intestine (duodenum). Approximately one in ten people will suffer from an ulcer. Each year about ten per cent of sufferers required surgery because of persistent symptoms. In a small minority of cases patients will die of ulcer-related complications. Who is at risk? Ulcers can develop at any age, but they are rare among teenagers and even more uncommon in children. Duodenal ulcers occur for the first time usually between the ages of 30 and 50. Stomach ulcers are more likely to develop in people over age 60. Duodenal ulcers occur more frequently in men than women; stomach ulcers develop more often in women than men.
