Ulcers are erosions in the lining of the stomach or duodenum (the first part of the small intestines). An ulcer in the stomach is called a gastric ulcer . An ulcer in the duodenum is called a duodenal ulcer. Together, ulcers of the stomach and duodenum are referred to as peptic ulcers. Most ulcers are erosions of the first layer of the inner lining. If the hole goes all the way through, this is called a perforation of the intestinal lining. This can cause shock and is a medical emergency.

Left untreated, peptic ulcers can cause internal bleeding (commonly called a bleeding ulcer). Peptic ulcers may even perforate the wall of your stomach or small intestine. Peptic ulcers usually cause pain from stomach acid washing over the open sore. But the pain may decrease or even disappear when bleeding occurs because the blood buffers the acid. About 10 percent of people with bleeding peptic ulcers do not experience pain. Peptic ulcers are common. Five to 10 percent of Americans experience a peptic ulcer at some point in their lives. The good news is that the rate of bacterial infections that can cause some peptic ulcers seems to be dropping in the United States. You should not treat an ulcer on your own, without seeing a doctor. Over-the-counter antacids and acid blockers may relieve the frequent pain, but the relief is always short-lived. With a doctor's help, you can find prompt relief from ulcer pain as well as a lifelong cure from the disease.

If they're found in the duodenum, they're called duodenal ulcers. You can have more than one ulcer. *Many terms are defined in the glossary near the bottom of the page. Peptic Ulcer Disease and Non Ulcer Dyspepsia Diet Purpose
A diet that avoids stomach irritants is for those patients who suffer from symptoms of peptic ulcer disease or non-ulcer dyspepsia. In peptic ulcer disease, the patient has one or more ulcers in the stomach or duodenum (the first part of the intestine beyond the stomach). Non-ulcer dyspepsia refers to these same symptoms but without the presence of an ulcer. These symptoms, which include discomfort or burning in the upper abdomen, often occur an hour or so after eating and may be relieved by milk, food, or antacids. In the past, diet was considered very important in treating ulcers. Now physicians know that foods do not actually cause an ulcer. There are a few foods, however, that can aggravate ulcer symptoms or delay healing. Nutrition Facts
Depending on individual food selection, this diet meets the National Research Council's Recommended Dietary Allowances (RDA). Special Considerations
  • Items most frequently associated with gastric discomfort include the following: black pepper, red or hot pepper, chili powder, caffeine, regular and decaffeinated coffee or tea, alcohol, cocoa, chocolate, cola beverages, citrus fruits and juices, fatty and fried foods, tomato products, and peppermint. These should be eliminated from the diet unless specifically allowed by the physician and/or dietitian. Additional adjustments may be made according to individual tolerances.

    Causes, incidence, and risk factors: A duodenal ulcer is a type of peptic disease that is caused by an imbalance between acid and pepsin (an enzyme ) secretion and the defenses of the mucosal lining. The inflammation may be precipitated by aspirin and nonsteroidal anti-inflammatory medications (NSAIDs).
    Duodenal ulcers are commonly associated with the presence of the bacteria Helicobacter pylori in the stomach. Risk factors are aspirin and NSAID use, cigarette smoking , and older age. Duodenal ulcer has historically occurred more frequently in men, but more recent data suggest similar rates in both men and women. The lifetime prevalence of a peptic ulcer 5 to 10% and approaches 10 to 20% in patients who are Helicobacter pylori positive.

    Peptic ulcer refers to an area of the stomach or duodenal lining which becomes eroded by the stomach acid. These are known as stomach and duodenal ulcers, collectively known as peptic ulcers.
    Symptoms
    You may notice no symptoms although you have an ulcer, but frequently they are associated with indigestion or pain in the upper abdomen or even lower chest. This pain may be worse before or after eating and frequently wakes you in the early hours of the morning. The pain may go to the back or sometimes be perceived elsewhere. If an ulcer bleeds you may vomit blood or partially digested blood (which looks like coffee grounds) or pass black stools (melaena), which contain changed blood, when you go to the toilet. If any of these happen call the doctor immediately (even out of hours).
    Causes
    Many factors make you more likely to have an ulcer.
    • It often seems to run in families.
    • Smoking increases the risk.
    • Heavy drinking.
    • Certain drugs eg aspirin and other anti-inflammatory drugs
    • We now know that a large percentage of people who have ulcers have a germ (Helicobacter pylori) in their stomach which they probably picked up many years previously, which makes the ulcer more likely to occur and more difficult to heal.

A peptic ulcer is a sore on the lining of the stomach or duodenum. Peptic indicates it is related to digestion. It occurs in tissues which are bathed by secretions containing acid and pepsin. The stomach is a unique organ which has the ability to hold very concentrated solutions of acid, as well as the protein digestive enzyme pepsin, without damage to itself. This unique ability is due to a highly specialised protective barrier made up of a continuous mucous layer which maintains a near alkaline pH at the cell surface in spite of the concentrated acid in the stomach sac, very tight junctions between the cells, and a very good blood supply.
