This is a video that illustrates what view the doctor has during a colonoscopy examination in a patient with ulcerative colitis . This 17 year old high school student came to our office with a complaint of 6 weeks of worsening diarrhea which was a new symptom for her. Recently, she noticed some blood and mucus in her bowel movements and crampy abdominal pain. When she began missing school, her mother made an appointment for a gastroenterology evaluation. When this colonoscopy was performed, she was found to have ulcerative colitis. Ulcerative colitis is a disease of the colon which causes inflammation and ulcers on the inner lining of the colon. This usually causes symptoms of bloody diarrhea and abdominal pain. The cause is not known and there is no medical cure as yet, but treatment is available.
In this short video clip, you can see the inflamed irregular colon lining with some red bleeding. The white material called exudate is a response to the inflammation. Compare this to the video of a normal colon lining . With treatment, her symptoms improved and her diarrhea and rectal bleeding ceased. What is Crohn's disease and ulcerative colitis?
Crohn's disease and ulcerative colitis are two forms of inflammatory bowel disease with similar symptoms, including abdominal pain (cramping), diarrhea, constipation, and fever. Crohn's disease causes inflammation and ulceration in any part of the digestive tract from the mouth to the anus. Because many areas may be affected, further symptoms can include fatigue, weight loss, joint pain, eye problems, and skin rash. Deep ulcerations in the digestive tract occur, causing narrowing of the bowels or bowel obstructions, or fistulas, which are abnormal connections or openings in the intestines. Many complications can develop from Crohn's disease, including nutritional deficiences as the intestines are unable to absorb nutrients from food.

ABSTRACT Background Selective blockade of interactions between leukocytes and vascular endothelium in the gut is a promising strategy for the treatment of inflammatory bowel diseases. Methods We conducted a multicenter, double-blind, placebo-controlled trial of MLN02, a humanized antibody to the integrin, in patients with active ulcerative colitis. We randomly assigned 181 patients to receive 0.5 mg of MLN02 per kilogram of body weight, 2.0 mg per kilogram, or an identical-appearing placebo intravenously on day 1 and day 29. Eligible patients also received concomitant mesalamine or no other treatment for colitis. Ulcerative colitis clinical scores and sigmoidoscopic assessments were evaluated six weeks after randomization. Results Clinical remission rates at week 6 were 33 percent

Ulcerative colitis is a chronic inflammation of the large intestine (colon). The colon is the part of the digestive system where waste material is stored. The rectum is the end of the colon adjacent to the anus. In patients with ulcerative colitis, ulcers and inflammation of the inner lining of the colon lead to symptoms of abdominal pain, diarrhea , and rectal bleeding. Ulcerative colitis is closely related to another condition of inflammation of the intestines called Crohn's disease. Together, they are frequently referred to as inflammatory bowel disease (IBD). Ulcerative colitis and Crohn's diseases are chronic conditions that can last years to decades. They affect approximately 500,000 to 2 million people In the United States. Men and women are affected equally. They most commonly begin during adolescence and early adulthood, but they also can begin during childhood and later in life. It is found worldwide, but is most common in the United States, England, and northern Europe. It is especially common in people of Jewish descent. Ulcerative colitis is rarely seen in Eastern Europe, Asia, and South America, and is rare in the black population. For unknown reasons, an increased frequency of this condition has been recently observed in developing nations.

Ulcerative colitis
Ulcerative colitis is a long-term inflammatory disorder that causes ulceration of the rectum and the colon. It may only affect the rectum or may spread from the rectum to affect part of, or the entire, colon.

What are the symptoms?
Symptoms are intermittent, often not occurring for months or years. During mild attacks symptoms may develop over a few days and include diarrhoea (sometimes with blood and mucous), tiredness, abdominal pain and poor appetite. In severe attacks the symptoms come on suddenly and may include severe diarrhoea many times a day, passing of blood and mucous, fever, abdominal pain and swelling, and weight loss. A potentially fatal complication of ulcerative colitis is inflammation and distension of the colon, called toxic megacolon, which may result in bowel perforation. Ulcerative colitis affecting most or all of the colon for several years increases a person's risk of bowel cancer. Those with ulcerative colitis may also have arthritis, eye disorders (uveitis) and erythema nodosum.

CLEVELAND, April 8-Ulcerative colitis patients can reduce steroid doses yet achieve good symptom control by drinking a nutritionally balanced cocktail of fish oil, soluble fiber, and antioxidants daily, researchers here report. Patients who took the nutritional supplement daily for six months were able to control ulcerative colitis symptoms while reducing daily prednisone use by 1.27 mg/day from baseline. Patients randomized to placebo had a 0.92 mg/day decline (P <0.001), said Douglas Seidner, MD, a gastroenterologist at the Cleveland Clinic.

This protocol does not involve any experimental treatments. Patients between the ages of 8 and 75 years of age with Crohn's disease or ulcerative colitis or symptoms of inflammatory bowel disease may be eligible for this study. Screening tests may include the following: medical history and physical examination, routine blood tests, examination of stool specimens, X-rays such as barium enema or upper GI series, proctosigmoidoscopy, colonoscopy, gastroduodenoscopy, and small bowel biopsy. Participants will receive medical treatment according to the best generally accepted measures for treating Crohn's disease or ulcerative colitis. This may include anti-inflammatory drugs, immunosuppressive drugs, and antibiotics to treat infections. A surgical consultation may be recommended for patients whose disease does not respond to medical treatment. If surgery to remove intestinal tissue is recommended, a qualified gastrointestinal surgeon will perform the procedure.

Colitis Overview
The inflammation makes the colon empty frequently, causing diarrhea. Ulcers form in places where the inflammation has killed colon lining cells. The ulcers bleed and produce pus and mucus. You may have abdominal pain, diarrhea, rectal bleeding, painful spasms (tenesmus), lack of appetite, fever, and fatigue.
  • Ulcerative colitis is an inflammatory bowel disease (IBD), the general name for diseases that cause inflammation in the intestines.
    • Crohn disease differs from ulcerative colitis because it causes inflammation deeper within the intestinal wall. Crohn disease usually occurs in the small intestine, but it can also occur in the mouth, esophagus, stomach, duodenum, large intestine, appendix, and anus. Ulcerative colitis affects only the colon.
      Another confusing condition called irritable bowel syndrome is not like either ulcerative colitis or Crohn disease. Irritable bowel syndrome is a common disorder of the intestine that leads to cramps, excessive production of gas, bloating, and changes in bowel habits.

    Up to 2 million people in the US are estimated to have either ulcerative colitis or Crohn disease. Ulcerative colitis is generally found in younger people, before they reach age 30. But the disease can occur in people in their 60s and later in life. It affects both men and women equally and tends to run in families. Jewish people tend to have more incidence of ulcerative colitis than non-Jewish people.

Tiny open sores or ulcers, form on the surface of the lining where they bleed and produce pus and mucus. Inflammation causes the colon to empty frequently, so symptoms typically include diarrhea and crampy abdominal pain. Inflammation usually begins in the rectum, but it may involve the entire colon. When ulcerative colitis affects only the rectum, it is called ulcerative proctitis. If the disease affects only the left side of the colon, it is called limited or distal colitis, and when it involves the entire colon, it is called pancolitis. There are no areas of normal intestine between the areas of diseased intestine, and it affects only the innermost lining of the colon.
