What is inflammatory bowel disease?
Inflammatory bowel disease is the name of a group of disorders that cause the intestines to become inflamed (red and swollen). The inflammation lasts a long time and usually comes back over and over again. More than 600,000 Americans have some kind of inflammatory bowel disease every year. If you have inflammatory bowel disease, you may have abdominal cramps and pain, diarrhea, weight loss and bleeding from your intestines. Two kinds of inflammatory bowel disease are Crohn's disease and ulcerative colitis. Crohn's disease usually causes ulcers (open sores) along the length of the small and large intestines. Crohn's disease either spares the rectum, or causes inflammation or infection with drainage around the rectum. Causes, incidence, and risk factors: The cause is of ulcerative colitis is unknown. It may affect any age group, although there are peaks at ages 15 to 30 and then again at ages 50 to 70. The disease usually begins in the rectal area and may eventually extend through the entire large intestine. Repeated episodes of inflammation lead to thickening of the wall of the intestine and rectum with scar tissue. Death of colon tissue or sepsis may occur with severe disease.

What is ulcerative colitis (UC)? Ulcerative colitis (UC) is a disease of the large intestine (the colon and rectum).
  • Colitis means 'inflammation of the colon'.
  • Ulcerative means that ulcers tend to develop. An ulcer is a raw area on the lining of the intestine which may bleed.
  • The inflammation and ulcers in the large intestine cause the common symptoms of diarrhoea, and passing blood and mucus.
Understanding the gut The gut (gastrointestinal tract) is the long tube that starts at the mouth and ends at the anus. Food passes down the oesophagus (gullet), into the stomach, then into the small intestine. The small intestine has three sections - the duodenum, jejunum and ileum. The small intestine is where food is digested and absorbed into the bloodstream. The structure of the gut then changes to become the large intestine (colon and rectum). The colon absorbs water, and contains food that has not been digested, such as fibre. This is passed into the last part of the large intestine, the rectum, where it is stored as faeces. Faeces (motions or stools) are then passed out into the toilet.

The inflammation usually occurs in the rectum and lower part of the colon (large intestine), although it may affect the entire organ. Ulcerative colitis may also be called colitis, ileitis or proctitis. This disease is often difficult to diagnose because symptoms are very similar to inflammatory bowel disease and Crohn's disease. Ulcerative Colitis Symptoms About half of all patients have mild symptoms, including:
  • Abdominal pain and bloody diarrhea (most common symptoms) Fatigue Weight loss Loss of appetite Rectal bleeding Loss of body fluids and nutrients

Crohn's disease and ulcerative colitis are the best known forms of IBD, and both fall into the category of "idiopathic" inflammatory bowel disease because the etiology for them is unknown. Pathologic findings are generally not specific, although they may suggest a particular form of IBD. "Active" IBD is characterized by acute inflammation. "Chronic" IBD is characterized by architectural changes of crypt distortion and scarring. Crypt abscesses (active IBD consisting of neutrophils in crypt lumens) can occur in many forms of IBD, not just ulcerative colitis.
    Ulcerative Colitis
    Ulcerative colitis (UC) involves the colon as a diffuse mucosal disease with distal predominance. The rectum is virtually always involved, and additional portions of colon may be involved extending proximally from the rectum in a continuous pattern. The etiology for UC is unknown. UC is more common in persons of Caucasian race, in women, and in young persons (peak incidence at ages 20 - 25 years). Clinical findings can include diarrhea, but the amount of diarrheal stool is not great, and is often accompanied by tenesmus. Patients with prolonged UC are at increased risk for developing colon cancer. Colonic biopsy can be used to detect dysplasia, a neoplastic change in the mucosa which implies an increased probability of malignancy. Patients with UC are also at risk for development of liver diseases including sclerosing cholangitis and bile duct carcinoma.

    What is ulcerative colitis?
    Ulcerative colitis is an inflammation in the lining of the large intestine (colon) and rectum.
    How does it occur?
    The cause of ulcerative colitis is not known. Sometimes the disease worsens with stress, but stress is not the cause.
    What are the symptoms?
    The symptoms usually come and go. Flare-ups of symptoms alternate with times when you have no symptoms (remissions). Symptoms include:
    • diarrhea, usually with blood or mucus crampy pain in your lower abdomen dehydration fever weight loss fatigue.
    How is it diagnosed?
    Your health care provider will ask about your symptoms and examine you. You may have blood tests to check for anemia and inflammation.

    Ulcerative colitis is a form of inflammatory bowel disease (IBD). It causes swelling, ulcerations, and loss of function of the large intestine. Description
    The primary problem in IBD is inflammation, as the name suggests. Inflammation is a process that often occurs in order to fight off foreign invaders in the body, including viruses, bacteria, and fungi. In response to such organisms, the body's immune system begins to produce a variety of cells and chemicals intended to stop the invasion. These immune cells and chemicals, however, also have direct effects on the body's tissues, resulting in heat, redness, swelling, and loss of function. No one knows what starts the cycle of inflammation in IBD, but the result is a swollen, boggy intestine. In ulcerative colitis, the inflammation affects the lining of the rectum and large intestine. It is thought that the inflammation begins in the last segment of large intestine, which empties into the rectum (sigmoid colon). This inflammation may spread through the entire large intestine, but only rarely affects the very last section of the small intestine (ileum). The rest of the small intestine remains normal. Ulcerative colitis differs from Crohn's disease , which is a form of IBD that affects both the small and large intestines. The inflammation of ulcerative colitis occurs only in the lining of the intestine (unlike Crohn's disease which affects all of the layers of the intestinal wall). As the inflammation continues, the tissue of the intestine begins to slough off, leaving pits (ulcerations) which often become infected.
