Ulcerative colitis
Published by BUPA's Health Information Team
February 2004 Ulcerative colitis is an inflammatory bowel diseases. It can cause frequent bouts of diarrhoea that may contain blood and mucus. It can also cause abdominal pain. Ulcerative colitis tends to have flare-ups and then settle down again for variable amounts of time, although it is possible to have a single attack of the condition.
What is it?
Ulcerative colitis affects the rectum and variable amounts of the rest of the colon (the large bowel or intestine). The other main inflammatory bowel disease, Crohn's disease, can affect any part of the digestive tract from the mouth to the anus. In around 10% of cases, it is not possible for doctors to distinguish between ulcerative colitis and Crohn's disease. For more information, please see the separate BUPA factsheet titled Crohn's disease. Approximately 1 in 100 people are affected by ulcerative colitis. The disease affects men and women equally and can develop at any age, but most commonly affects adults aged 20-40. It is not an infectious illness.

Ulcerative colitis is a disease that causes inflammation and sores, called ulcers, in the top layers of the lining of the large intestine. The inflammation usually occurs in the rectum and lower part of the colon, but it may affect the entire colon. Ulcerative colitis rarely affects the small intestine except for the lower section, called the ileum. Ulcerative colitis may also be called colitis, ileitis, or proctitis. The inflammation makes the colon empty frequently, causing diarrhea. Ulcers form in places where the inflammation has killed colon lining cells; the ulcers bleed and produce pus and mucus. Ulcerative colitis is an inflammatory bowel disease (IBD), the general name for diseases that cause inflammation in the intestines. Ulcerative colitis can be difficult to diagnose because its symptoms are similar to other intestinal disorders such as irritable bowel syndrome and to another type of IBD called Crohn's disease. Crohn's disease differs from ulcerative colitis because it causes inflammation deeper within the intestinal wall. Crohn's disease usually occurs in the small intestine, but it can also occur in the mouth, esophagus, stomach, duodenum, large intestine, appendix, and anus.

Jeffrey's IBD Story..
In February of 1997 a chapter of my life started that would forever change the way I viewed my world and the health so many of us in life take for granted..I passed some blood while using the bathroom one day and as you would expect It scared me a little but I dismissed it as something bad I ate or some rawness down there..you know..nothing major..forget about it right We'll it started appearing more and more frequently however I never really felt bad other than the blood making me a little weak.. I passed it off as nothing and figured it was just a normal part of my movements now..Life goes on and ready for the next day...We'll I finally mentioned it to my wife and well she did not think it was so normal and urged me to make an appt with our family doc..so I did with much resistance..C'mon you know how us guys are with doctors and hospitals..Were invincible!! I went and saw our family P.A. who dismissed it as probably a innocent Roid and recomended I see a Colorectal Doctor to confirm this..Well after about another month of the symptoms now combined with some kind of mucus I finally did make that appt with a colorectal doc..Were up to April 97 now and still bleeding and also starting to take a toll on my health!! The Colon Rectal doc told me it was roids and performed three small banding procedures that hurt like hell to remove these supposed hemmies!! To this day I doubt he ever did do anything significant because guess what The bleeding and mucus got worse and now more symptoms began such as Severe cramping and Bloody Diareha. I started going 10,15,20 times a day and it was all blood..I got so weak that I could barely lift my head some days let alone get out of bed!!

IBD is depressing, maddening and unfair, but it's real. We can't wish it away, so we have deal with it as best we can. Here I offer some things I've written over the years of living with Crohn's Disease . They're arranged in chronological order, so they're sort of a diary of coping and healing. Many deal specifically with IBD, but many deal with the stuff of life in general. Since I don't believe the body and mind and spirit function separately from one another, it all seems pertinent. I hope they bless you, amuse you and/or inspire you. I am happy to link this page to similar sites, as well as to responsible IBD and IBS informational sites. Index Home

Colonoscopy Digestive system Ulcerative colitis Definition: Ulcerative colitis is a chronic , episodic, inflammatory disease of the large intestine and rectum characterized by bloody diarrhea Causes, incidence, and risk factors: The cause is of ulcerative colitis is unknown. It may affect any age group, although there are peaks at ages 15 to 30 and then again at ages 50 to 70. The disease usually begins in the rectal area and may eventually extend through the entire large intestine. Repeated episodes of inflammation lead to thickening of the wall of the intestine and rectum with scar tissue. Death of colon tissue or sepsis may occur with severe disease. The symptoms vary in severity and their onset may be gradual or sudden. Attacks may be provoked by many factors, including respiratory infections or stress Risk factors include a family history of ulcerative colitis or Jewish ancestry. The incidence is 10 to 15 out of 100,000 people. Symptoms:
  • Diarrhea, from only a few episodes to very frequently throughout the day (blood and mucus may be present)

A s you read, i t might seem sad, but I want to emphasise how important it is to manage your own health and to believe that there is a solution. Who would have believed that removing my colon would be such a godsend — despite the stoma. My experience is provided for information purposes only and should not be considered medical advice. Always check with your personal physician when you have a question about your health. You must satisfy yourself that this risk is worth taking. In my opinion the doctors will let you go on as long as you don't complain too much. And just to reemphasise the point, I elected to undergo even more invasive and risky surgery 10 years later (in 2005) to remove the residual rectum and anus as they showed precancerous cell development. And I should never have waited so long because a tumour was discovered in the rectum removed (see an ongoing log at the end of this page for the latest position).

Ulcerative Colitis
Case Study, by Leonid M. Katkovsky, MD
Abstract : Active ulcerative colitis may preclude issuance of a medical certificate. An authorization for special issuance can be granted in well-controlled, inactive ulcerative colitis. The medical certificate would be time-limited for a period of 1 year and require that the airman give the FAA a current status of his medical condition at that time.
Background
A year-old first officer of a major commercial airline sub-mitted a new application for a first-class medical certificate. He had a long history of ulcerative colitis, which was quiescent until early January 2000. While in flight training, he developed shortness of breath and tachycardia. His local physician noted a hematocrit of 27 and iron level of 5. The applicant was immediately started on Asacol, Prednisone, Ciprofloxacin, and oral iron. There was a rapid improvement of symptoms. He did not lose any weight. After 2 months of treatment his hematocrit returned to normal. History and physical exam.
