NIH State-of-the-Science Conference on Manifestation and Management of Chronic Insomnia in Adults The conference (free and open to the public) will be held at the NIH Natcher Conference Center on June 13-15, 2005, and is sponsored by the National Institute of Mental Health and the NIH Office of Medical Applications of Research. http://www.consensus.nih.gov/ The Behavior Therapy and Research Lab (BTRL) at the University of Wisconsin-Milwaukee is a very active research lab. We present a good deal of research at national conferences such as the Association for the Advancement of Behavior Therapy (AABT) or the Association for Behavior Analysis. We also publish a number of papers per year in various journals. Below, you can see the lab productivity over the previous 5 years. Nearly all of these articles and chapters were co-authored by BTRL graduate students. Research in the BTRL has two related foci. One focus is on understanding and developing treatments for OC-Spectrum disorders in children and adults. A second focus is on evaluating the efficacy of Acceptance and Commitment Therapy and trying to understand the related mechanisms of change.

SYMPTOMS
Recurrent pulling out of one's hair resulting in noticeable hair loss. An increasing sense of tension immediately before pulling out the hair or when attempting to resist the behavior. Pleasure, gratification, or relief when pulling out the hair. The disturbance is not better accounted for by another mental disorder and is not due to a general medical condition (e.g., a dermatological condition). The disturbance causes clinically significant distress or impairment in social, occupational, or other important areas of functioning.

Just what is Trichotillomania
The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) (American Psychiatric Association, Washington, DC, 1994) defines Trichotillomania as:
Recurrent pulling out of one's hair resulting in noticeable hair loss.
Increasing sense of tension immediately before pulling out the hair.
Pleasure, gratification, or relief when pulling out the hair.
Not better accounted for by another mental disorder and not due to a general medical condition (e.g., a dermatological disorder).
Results in substantial distress and/or interferes with functioning.
Not all patients who pull out their hair meet these strict criteria. In fact, many do not experience a growing sense of tension before pulling. Nonetheless, they frequently have a distressing problem that might very well benefit from treatment.
Finding out I wasn't alone was one of the greatest days in my life. Barry can feel the tension building, sometimes for hours, before he yields to his impulse. Although he knows he's affecting his dark good looks, the anxious feeling doesn't fade until he's yanked out another fistful of his own curly black hair. He immediately vows he'll never do it again, but will readily admit he probably won't be able to keep that promise. For Belinda, the tension reliever is pulling out her now-stubby eyelashes. One at a time, slowly, ritualistically she removes the remaining hairs that line her upper and lower lids. She began removing them this way as a child, although she can't recall exactly why or when. Despite numerous medical exams first by her pediatrician and later by her internist and a dermatologist no physical problems have been found. Her upper and lower eyelids are the only sign of this persistent habit. They have scarcely any hairs at all. Louisa also tugs at her hair, pulling out a single strand at a time, dozens of times each day. Then she places the strands she's removed from her scalp into her mouth. She's seldom seen without a scarf that covers the bald spots this practice has created, but she denies that the patchy appearance of her hair is the result of her own behavior.

TRICHOTILLOMANIA The defining characteristic of Trichotillomania is the recurrent, compulsive pulling out of one's own hair, resulting in observable hair loss. Usually, but not always, the scalp and/or face are the primary locations for hair pulling. However Trichotillomania may involve any part of the body with hair. The most common hair pulling sites are the scalp, eyebrows, and eyelashes. Less common locations for hair pulling include the pubic area, perirectal region, or any other body region. An individual with Trichotillomania may use his or her fingernails, as well as tweezers, pins or other mechanical devices. This hair pulling can result in permanent skin damage. Usually, but not always, hair pulling is preceded by a high level of tension and a strong "urge". Likewise, hair pulling is usually, but not always, followed by a sensation of relief or pleasure. Hair pulling is usually done alone, often while watching TV, reading, talking on the phone, driving or while grooming in the bathroom. An episode may be triggered by a negative mood state or occur in response to stress, but may also occur while an individual is calm and relaxed. Sometimes hair pulling is done as a conscious behavior, but it is frequently done as an unconscious habit. Individuals with Trichotillomania often attempt to camouflage the hair loss that accompanies the disorder. Common camouflaging techniques include the use of hats, scarves, and false eyelashes. Some may even resort to having false eyebrows permanently tattooed. In extreme cases, individuals with Trichotillomania may avoid social situations in an effort to prevent others from seeing the hair loss that results from hair pulling.

In a moment of extreme frustration I have been known to whine that I was about to “pull my hair” out. While my quip would always illicit a laugh from my companions, in reality, hair pulling is no laughing matter. According to Dr. Howard Rosenthal, Ed.D, famous author of the best selling book, Encyclopedia Of Counseling “it is estimated that more than 8 million Americans currently suffer from a form of compulsive hair pulling known as Trichotillomania (pronounced trick-o-till-o-mania It is now believed females suffer more from trichotillomania (TTM) than males. Coined by a French dermatologist in 1889, the term described individuals who suffer from a " dis-tressing condition.” The word is derived from the Greek words thrix (hair) and tillein (to pull) and mania (madness). Although TTM involves a compulsive pulling , and sometimes eating, of hair from the scalp, and other parts of the body, it really is not about the individual’s hair or how they feel about it.

To learn more about "The OCD Workbook" and order a copy, click here Trichotillomania (TTM) is characterized by chronic, repetitive pulling of bodily hair. The sites of hair pulling include the scalp, eyelashes, eyebrows, axillary, body, and pubic area. Hair pulling tends to occur in episodes, exacerbated by stress, or sometimes by relaxation (when reading a book or watching television, for example). All other causes of hair loss, including medical and dermatological problems, must be considered and ruled out before the diagnosis of trichotillomania is confirmed. People with TTM experience an increasing sense of tension immediately before pulling out a hair or when attempting to resist hair-pulling. When the hair is pulled, they experience immediate feelings of pleasure, gratification, and relief. Once thought to be rare with a prevalence of only .05 percent to .6 percent, recent studies estimate a prevalence of 2 percent to 3 percent of the population (Keuthen, 1998). As with OCD, people with TTM are often highly secretive about their symptoms. They may hide their symptoms by pulling hair from areas not easily visible or wear wigs or hair styles that conceal the areas where their hair is pulled.
