    Congenital muscular torticollis is a one-sided tightness of the sternocleidomastoic muscle located on either side of the neck. Tightness of this muscle results in a slightly tilted position of the head and limited neck movements. This condition can also lead to skull and facial asymmetries, limited and asymmetrical neck and trunk movements, and altered gross motor skills. While some children simply regain movement on their own, most require some type of intervention. Physical therapy treatment with conservative measures as early as possible has been shown to be extremely effective in dealing with torticollis, and critical in obtaining the best possible outcomes for the child. Evaluation Services Upon referral from your child's primary care physician or specialist, a pediatric physical therapist will evaluate your child's status in the following areas:
    • Range of motion
    • Flexibility
    • Strength
    • Resting posture
    • Performance in age appropriate gross motor skills
    Treatment Strategies
    • Stretches to improve muscle excursion and joint flexibility
    • Myofascial techniques to increase muscle range

Torticollis
Over 45,000 infants are born with congenital torticollis in the US each year. This twisted or tilted neck, the so-called "fixed wry neck" is often ascribed to problems during birth. The condition causes painful, involuntary spasms; impedes normal growth and development and disturbs vision. The medical approach to this condition is often surgery on neck muscles, nerves and tendons, and intensive physical therapy and drug therapy.

Abstract: This paper describes the successful treatment of an infant with wry neck associated with birth trauma using low force, relatively long-duration cranial adjusting, and soft-tissue techniques to the whole body with special attention to the cervical region, and parental management of home care procedures. Wry neck, or congenital muscular torticollis (CMT), has been a well - recognized condition for centuries. CMT is often associated with plagiocephaly, which has long-term adverse effects on physical and mental functions. A review of some of the literature relating to this condition is provided.

Our job here is twofold. First there is a common entity which is fairly straightforward in its nature. But there are also look-alikes which need to be differentiated. The typical case is a baby with a slightly turned and tilted head. Most commonly, it isn't even noticed until two to three weeks of age. Sometimes a horizontal red line is seen at the base of the neck. All that is is skin reddened from sweat trapped because of the constancy of the head to one side. Air makes that go away. Some of the more tilted and more rigid cases also have a lump on the neck muscle. It can feel hard. It isn't at all uncommon (sadly) to see parents with a sheaf of x-rays and studies trying to figure out the nature of this "tumor". It begins with a fetal posture that has the neck tilted. If that way long enough, the muscle called the "SCM" shortens. Birth gives little time for gradual stretching and that SCM muscle may very well tear as the head goes through the twists of getting born. Because the muscle tears, the head is NOT tilted. The SCM muscle sheath fills with blood and presents a fairly normal contour (not a dent). As the blood absorbs, scar tissue infiltrates the tear area and vessels come in to take away the debris - that's the swelling. Scar is nature's suture, it bridges defects then CONTRACTS. It pulls the torn ends back together. So at about two to three weeks we see the tilt for the first time, but it is really the second time as the initial tilt was out of view in the womb. The lump may be prominent or already gone.

My story begins with a surprise pregnancy. My husband and I found out we were going to be starting a family earlier than we had planned. In nursing school, I had learned that human milk was superior to infant formula, so we knew I would be breastfeeding our baby. I found myself anticipating nursing my baby, and had several dreams during my pregnancy in which I nursed a baby boy. I really wasn't worried about being able to breastfeed. After all, women have been doing it for thousands of years. I would roll my eyes behind the back of mothers who said, "I tried to nurse my baby but I just couldn't do it." I thought to myself, "What on earth did they mean? What's so difficult about breastfeeding? Your body produces the milk, you put your nipple near your baby's mouth and he drinks." My sister- and mother-in-law encouraged me to attend La Leche League Group meetings prior to the birth of our baby. I eventually went to a meeting when I was eight months pregnant. I thought LLL was a breastfeeding support group, kind of like Alcoholics Anonymous or something. I thought it was a place to go where mothers who suffer social persecution because they breastfed go for support from other breastfeeding mothers who have great tactics for dealing with society. I didn't realize that LLL was so much more until after our baby was born.
