West Bloomfield, Michigan 48323 USA; www.drbrownstein.com; 1-888-647-5616; Softcover, 2002, US/$15.00 272pp. In Chapter 2, Dr. Brownstein succinctly explains why thyroid disorders are so prevalent today. He makes the case that hypothyroidism may be the most under-diagnosed condition today. Dr. Brownstein feels that up to 40% of the population may have an underactive thyroid condition, which is being missed by the common blood tests used today. This book builds on Dr. Broda O. Barnes' groundbreaking work on hypothyroidism. Dr. Brownstein illustrates his belief that a more holistic approach, which takes into account the clinical picture in addition to the blood tests and the basal body temperatures, is a comprehensive way to diagnose thyroid disorders. Dr. Brownstein explains his ideas and supports much of what he says with numerous citations.

It has a lobe on each side of the Adam's apple with a thin ridge of tissue between the lobes. The two major thyroid hormones are thyroxine (T4) and triiodothyronine (T3). The thyroid produces T4 when the pituitary gland in the brain releases thyroid-stimulating hormone (TSH). Almost all of T3 is made from T4 after it is released by the thyroid gland, but a small amount of T3 is produced directly by the thyroid gland. T3 and T4 help control how the body uses food for energy (metabolism). In children, these hormones are needed for normal growth and development. In adults, they are vital in maintaining many normal body functions.

Women more likely to suffer from thyroid disorders
More than 13 million Americans have thyroid disorders and more than half of them aren't even aware of it. Are you one of them? Women are five to six times more likely to suffer from thyroid disorders and approximately one in eight women will develop a thyroid problem during her lifetime. Thyroid disorders fall into two categories:
Hypothyroidismthe underactive thyroid
This is the most common form of thyroid problems, affecting four to eight percent of the general U.S. population. UI Health Care endocrinologist Thomas O'Dorisio, M.D., says symptoms include droopy eyelids, intolerance to cold, difficulty swallowing, forgetfulness, decreased concentration, and feelings of sluggishness and fatigue. You may feel depressed and experience mood swings. Your hair may become dry and brittle, your skin may become dry and itchy, your voice may become deep and raspy, you may be constipated and may experience muscle cramps. O'Dorisio says you may have trouble losing weight, no matter how closely or strictly you follow a diet. "Hypothyroidism is easily treated with thyroid hormone replacement therapy using levothyroxine sodium," O'Dorisio says.

Thyroid Disorders
Are you experiencing unexplained fatigue, forgetfulness, irritability, nervousness, mood swings, heat or cold intolerance, muscle weakness or tremors, weight gain, weight loss, or sleep disturbances? Do you have dry, coarse skin and hair, a hoarse voice, vision problems or eye irritation? Are you a woman experiencing irregular menstrual problems that seem to have no explanation? If you can answer yes to just a handful of these symptoms, you may have a thyroid disorder. In fact, "an estimated 13 million Americans have thyroid disorders, and more than half of them remain undiagnosed (American Medical Women's Association, 1999a)." Among the most common types of thyroid disorders are hypothyroidism (underactive thyroid), hyperthyroidism (overactive thyroid), benign nodules, inflammations of the thyroid, and malignant cancers (The Endocrine Society, 2000a), which we will discuss in this issue of HealthHints.

Questions and Answers About Thyroid Disorders
and The Kelly G. Ripken Program
What is a thyroid? The thyroid is a butterfly-shaped gland located in the neck in the front of the windpipe. In healthy people, the thyroid makes just the right amounts of two hormones, T4 and T3, which perform important actions throughout the body. These hormones regulate many aspects of our metabolism, affecting everything from our temperature, mood, muscle strength and appetite to heart rate. What are thyroid disorders? When something goes wrong with the thyroid, it is often because the hormones produced by the gland are off balance. Hyperthyroidism is a condition that occurs when the thyroid gland produces too much thyroid hormone in an uncontrolled manner. The gland also may become enlarged. Because the body's metabolism is increased, patients may feel hotter than other people and often lose weight even though they may be eating more. Patients usually experience fatigue, but have trouble sleeping. Trembling of the hands and hard or irregular heart beats may develop. Patients may become irritable and easily upset. Hypothyroidism is a condition that occurs when the thyroid gland is unable to produce enough thyroid hormone to meet the body's needs. It often results from thyroid gland inflammation or from previous treatment for an overactive thyroid gland. Patients may have fatigue, constipation, memory loss and depression. They may be intolerant to cold and may have a slower heart rate. Dry hair and skin are seen in some patients. Others have no symptoms at all.

Home Issues Email alerts Classifieds ... Paediatrics Editorials Iodine intake and prevention of thyroid disorders: surveillance is needed Peter Laurberg and Susanne B MJA There is a need for national monitoring and adjustment of iodine intake as part of a program of prevention of thyroid disorders and their complications The widespread application is a response to the paucity of iodine in the natural diet in many regions of the world and the severe public health consequences of iodine deficiency. However, in some countries, the tendency to low iodine intake has mostly been corrected by haphazard increases in the iodine content of certain parts of the diet. In the United Kingdom, dairy products may contain extra iodine as a result of adding iodine to cow feed to increase the animals' reproductive performance or of using iodine-containing cleansing agents in the dairy industry. As reviewed by Phillips, g per day) in 1964. Subsequently, they discovered that bakers used iodine-containing conditioners in bread and that this caused high levels of iodine intake. Obviously, such unplanned variation in dietary iodine is a hazardous way of providing a population with an adequate intake of iodine, as mechanisms unrelated to disease prevention can profoundly alter iodine intake. To some extent, Australia may be a country where factors other than disease prevention have modulated the intake of iodine, and iodine intake may now be in an unplanned phase of decrease.

Undergoing fine needle aspiration cytology for thyroid cancer. THE thyroid gland is a small endocrine organ situated in the neck. It produces hormones, which have a profound impact on the body as a whole. The hormones produced are three in number: tri-iodo thyronine, thyroxine and calcitonin. Of these, the first two are of daily relevance. The dynamic release of these two hormones is controlled by the pituitary gland, located in the brain.

Nicotine studied as treatment for brain disorders
By Carey Goldberg, Globe Staff, 11/12/2003 Scientists reported yesterday that nicotine seems to diminish mental impairment stemming from stress or an underactive thyroid the latest in a growing body of evidence that the long vilified substance may help people with brain disorders ranging from Alzheimer's disease to schizophrenia. ADVERTISEMENT Nicotine researchers caution that the findings by no means offset the health ravages of smoking. But for years, a growing number of them have been exploring the aid nicotine may offer people with dementia, depression, attention deficit hyperactivity disorder, and more. "Yesterday's poison can be today's therapy," said Dr. Jerry J. Buccafusco, a pharmacologist and researcher at the Medical College of Georgia. Pharmaceutical researchers are racing to synthesize drugs that could offer the benefits of nicotine without its negative side effects, such as addiction and narrowed blood vessels.
