    Background
    There's not much difference between T3 and T4. The numbers refer to the amount of atoms of iodine contained in the hormones. T3 is the more powerful while T4 is released by the thyroid in larger amounts, but is mostly converted to T3 in the liver and kidneys. The effect of T3 and T4 is to
    • increase the basal metabolic rate of almost all the cells in the body increase the fat and carbohydrate metabolism boost protein synthesis increase heart rate and blood flow to other organs.
    Thyroid hormones are also needed for normal development of organs such as the heart and the brain in children and for normal reproductive functioning. The thyroid gland also contains some other structures that are important in bone and calcium metabolism. top

    When the gland is healthy, it releases as much thyroid hormone as we need to keep our metabolism on an even keel. It knows just how much to release because of the feedback mechanisms between a gland in the base of the brain, called the pituitary, and a small part of the brain above the pituitary, called the hypothalamus.

Small and butterfly-shaped, it rests unassumingly at the base of the throat, producing hormones that regulate the body's metabolism, temperature and heart rate. When it's functioning properly, we hardly know it's there. But this unpretentious gland often makes its presence known in a big way. Like the ice maker in your freezer, it can go on the fritz, either pumping out hormones at a frantic pace or reducing its production to barely a trickle. When this happens, the over- or underactive thyroid can dramatically rev up or slow down your body's metabolic activity.     Disorders Affecting the Thyroid
    The thyroid gland, located in the front of the neck and below the larynx (voice box), plays an important role in regulating the body's metabolism and calcium balance. The small, two-inch gland consists of two lobes, one on each side of the windpipe, connected by tissue called the isthmus.
    The thyroid tissue is made up of two types of cells: follicular cells and parafollicular cells. Most of the thyroid tissue consists of the follicular cells, which secrete iodine-containing hormones called thyroxine (T4) and triiodothyronine, or T3 (both hormones secreted by the thyroid which regulate metabolism). The parafollicular cells secrete the hormone calcitonin. The thyroid needs iodine to produce the hormones. Levels of hormones secreted by the thyroid are controlled by the pituitary gland's thyroid-stimulating hormone, which, in turn, is controlled by the hypothalamus.
    A thyroid disorder may be congenital (present at birth), or develop later in childhood. With proper treatment, which usually involves hormone replacement therapy, most thyroid disorders can be successfully managed in children.

    Study
    Consecutive patients to an outpatient clinic were examined over one year. For every patient newly diagnosed with thyroid disease, three age and sex-matched controls were chosen from the same outpatient list, but with no biochemical or clinical evidence of thyroid disorder. In face-to-face interviews diagnosis of RLS symptoms was made using current diagnostic criteria.
    Results
    There were 146 thyroid patients and 464 controls. The mean age of thyroid patients and controls was about 46 years, and about 70% were women. Twelve thyroid patients (8%) responded positively to the first three questions on RLS (urge to move the legs associated with sensory symptoms, motor restlessness, relief on moving the legs), but not the fourth question on symptoms being worse in the evening or at night. In controls, only 0.2% responded in this way. Eleven of the thyroid patients had Graves' disease or hyperthyroidism. Four patients had complete resolution of symptoms of RLS after treatment. The remainder reported some improvement.

    Research - Areas of Study THYROID DISORDERS
    Thyroid disease is the most common endocrine disorder. The thyroid is a gland located in the neck that produces several essential iodine-containing hormones. Thyroid hormones are metabolic regulators that affect many things including heart rate, cholesterol level, body weight, energy level, muscle strength, skin condition, vision, menstruation, and mental state. These hormones are transported from the thyroid gland to other tissues by specific serum proteins, such as transthyretin, that protect the hormones from being metabolized before they reach their target cells. Disorders of the thyroid can range from goiter (enlarged thyroid) to cancer. OUR WORK
    For over 25 years, Dr. Vivian Cody has studied the structure and function of thyroid hormones. Currently, her research involves the structural analysis of human transthyretin. These studies will help to explain how other substances also can bind to transthyretin and displace the hormone molecules. This nonspecific binding can result in cross reactivity among drugs and induce other unwanted side effects. In addition, mutations in the normal amino-acid sequence of transthyretin are the cause of the genetic disorder, familial amyloid polyneuropathy (FAP), that results in the formation of fibrils whose presence in the blood results in damage to major organs and, ultimately, in death. Dr. Cody is attempting to understand how these mutations affect the stability of transthyretin and to design agents to stabilize the protein.

    A previously healthy 14 year old female complains of a fast heart rate, weight loss, and fatigue over the past 2 months. Her family history is significant for a grandmother and aunt with Hashimoto thyroiditis. Exam: VS T 37, HR 110, R 22, BP 120/50. On exam, she is comfortable without diaphoresis. She has a mild tachycardia without murmurs or gallop. She is found to have a smooth goiter with a bruit, a mild tremor, and exaggerated DTRs. Labs: Elevated T4, undetectable TSH. Thyroid stimulating immunoglobulin assay is positive. She is diagnosed with Graves' disease. She is treated with PTU (propylthiouracil) after which her thyroid function normalizes. Clinically, there is resolution of her tachycardia, weight loss, and fatigue, and her goiter decreases in size. Her thyroid function is monitored routinely and the dose of PTU adjusted as indicated to maintain a euthyroid state. Two years after diagnosis, she goes into remission and PTU is discontinued. Only small amounts of T4 and T3 are free or unbound. This free hormone is available to tissues. T4 serves largely as a prohormone and is deiodinated in peripheral tissues by several iodothyronine monodeiodinase enzymes to active T3 or biologically inactive reverse T3 (rT3). The major source of circulating T3 is peripheral conversion from T4, largely by the liver. Only small amounts of T3 are secreted by the thyroid gland in euthyroid subjects ingesting adequate iodine. Normally, T4 is deiodinated to both T3 and rT3. The T3 mediates the predominant effects of thyroid hormones via binding to the 50-kD nuclear protein receptors, which function as transcription factors modulating thyroid hormone-dependent gene expression (1,2).

Thyroid hormone affects a person's physical energy, temperature, weight and mood. Thyroid diseases generally fall into two broad groups of disorders: abnormal function and abnormal growth (nodules) in the gland. These problems are common in the general population, especially among older people and women. Most thyroid problems can be detected and treated. Functional disorders are usually related to the gland producing too little thyroid hormone (hypothyroidism) or too much thyroid hormone (hyperthyroidism). Benign nodules in the thyroid gland are common and do not usually cause serious health problems. These nodules occur when the cell growth within the nodule is abnormal. Nodules can occasionally put pressure on the neck and cause trouble with swallowing, breathing or speaking if they are too large. The thyroid usually functions normally even when nodules are present.
