Summary of an address to the Kitchener-Waterloo Area Chapter Top Detection of the thyroid problem is complicated by the fact that everyone feels anxiety and tension to some degree, that many thyroid symptoms are similar to those of other diseases, and that hypothyroidism in particular often develops insidiously over a considerable time. But the results of overlooking the thyroid can be serious. It is very important for the physician to explore fully and give the tests for thyroid dysfunction, which today are relatively simple. When effective thyroid treatment is begun, the general response is quite favorable. Vitality returns and the mental processes become efficient again. If there is a residue of emotional difficulties, it may be related not to the thyroid gland but to other aspects of life. The question arises: since thyroid hormone therapy is so rewarding for patients who have depression associated with a malfunctioning thyroid, would it also benefit those who have normal thyroid function? The answer is not yet clear, though it has helped some who did not respond to anti-depressants.

Thyroid disorders: The thyroid gland produces various hormones used by the body and thyroid disorders cause various symptoms. The two main types are hypothyroidism (too little hormone) such as Hashimoto's thyroiditis , and hyperthyroidism (too much hormone) such as Graves' disease . Both conditions can be difficult to diagnose because they product relatively mild symptoms . Your thyroid gland tells every cell in your body the rate at which it should function. Having too little hormone, called hypothyroidism, puts you into a dragging slowdown. This is a problem for more than 10 million Americans-of whom 8 million don't know it. Having too much hormone, called hyperthyroidism, races your engine, so you feel all revved up-as if you were burning out. This problem affects some 4.5 million Americans, but at least 600,000 of them have yet to be diagnosed. Thyroid underactivity (Hypothyroidism)
Underactivity of the thyroid gland (hypothyroidism) can arise for a number of reasons. Hypothyroidism can occur in newborn infants (Congenital Hypothyroidism, CHT) as well as later on in childhood (acquired Hypothyroidism, AHT.) Affected newborns look, and usually act, normal for the first few days and the national screening programme for CHT, by blood spot collection, ensures that affected infants are identified and treated as early as possible. The commonest cause of CHT is a poorly developed or absent thyroid gland; the underlying basis to this abnormality is not understood. CHT can also occur if there is a defect in thyroxine production within the gland. These defects may be permanent or transient. Although every child with CHT should be started on thyroxine promptly, they should be evaluated at some time later to exclude a transient problem. The commonest causes of acquired hypothyroidism include an immune reaction against the thyroid gland by the body itself (Autoimmune Hypothyroidism, Hashimoto's disease) or damage to the thyroid gland or the part of the brain which controls the thyroid.

Introduction Related Articles Babies who are born with underactive thyroid function have a disorder known as congenital hypothyroidism. The usual cause of this condition is the failure of the thyroid gland to develop during pregnancy. At birth the infants look normal and then slowly over a period of weeks the clinical features of hypothyroidism appear. Because there are no conspicuous signs or symptoms, the diagnosis of congenital hypothyroidism is seldom made at birth by the examining physician. If the condition goes unrecognized, which is usually the case, then signs and symptoms such as constipation, dry skin, hoarse cry, large tongue, swelling around the eyes, failure to suckle well, and prolonged periods of sleep will appear within a few weeks. The Screening Process Every child born in North America has blood collected on filter paper by heel stick before discharge from the hospital or birthing center. The dried blood specimens are forwarded to a central laboratory where a one-eighth inch paper blood spot is tested for the amount of T4 and/or thyroid stimulating hormone (TSH). A low T4 and elevated TSH indicate that an infant lacks normal thyroid function. Although an elevated TSH is a more sensitive and specific marker of hypothyroidism, the majority of North American screening programs use T4 as the initial test and confirm the diagnosis with the measurement of TSH.

Where is it?
The thyroid gland consists of two lobes located in front of the neck just above the larynx, near the trachea (windpipe). What does it do? The thyroid produces THYROID HORMONE. The two most important hormones are Tetraiodothyronine (thyroxin or T4) and Triiodothyronine (T3). It's primary function is the production of thyroxin, a hormone that allows the dog to maintain an adequate and consistent level of metabolism.
How is the Thyroid Hormone Produced and WHAT does it Do In the Body? These hormones are essential for life and have many effects on body metabolism, growth and development. The Thyroid gland is influenced by hormones produced by two other organs:
  • The Pituitary gland, located at the base of the skull produces thyroid stimulating hormone, (TSH).
  • The Hypothalamus, a small part of the brain above the pituitary, produces Thyrotropin releasing hormone (TRH). In the healthy dog, thyroid hormone is secreted into the blood stream in steady amounts and it regulates the dog's metabolism. A deficiency of thyroid hormone, ( called Hypothyroidism) can lead to a slow metabolism while an over abundance of thyroid hormone (called Hyperthyroidism) can lead to an overactive metabolism. Hyperthyroidism is not common, usually occurs in dogs with tumors of the thyroid gland. The T4 will be above normal and the TSH is below normal in this situation.

    General Introduction
    Most thyroid disease seen in adults also occur in children. Although there are some differences in management, the principles remain the same. This pamphlet is to be used together with the other Health Guides on Thyroid Disease, which outline the causes and treatment of similar conditions in adults. It is very important to explain to the child, according to their level of understanding, where the thyroid is and what it does. A good way to describe the shape of the thyroid is to compare it to a butterfly sitting in the centre of the front of the neck over the windpipe and just above the collar bone. The outline can be seen in a child by raising the chin and tilting the head slightly back, especially if the thyroid is larger than normal. Its function, or job, can be compared to that of a furnace. If the thyroid is overactive (hyperthyroid), it can be considered as turned up too high; if underactive (hypothyroid), as turned down too low; or if the thyroid, no matter what its size, is making the right amount of thyroid hormone, then the thermostat is set just right. The thyroid is a different type of gland from the small round lymph glands which are easily felt on the sides of every child's neck. The lymph glands are there to protect against infection. The thyroid gland is there to make thyroid hormone, a body chemical needed by all cells so that they will work properly and at the right speed. The hypothalamus and pituitary gland (see
