OVERVIEW:
Occlusion of small and medium sized arteries and veins caused by inflammatory changes in these vessels. It primarily occurs in men who smoke.
System(s) affected: Cardiovascular
Genetics: Greater prevalence of HLA-A54, HLA-A9 and HLA-B5. Familial cases reported rarely.
Incidence/Prevalence in USA:
Predominant age:
20 to 40 years
Predominant sex: Male > Female (3:1). Increasing numbers of women are being diagnosed, presumably due to increased smoking.
SIGNS AND SYMPTOMS:
Symptoms tend to wax and wane in early disease and are often asymmetric. Symptoms may be gradual or have a sudden onset related to impaired vasculature.

Summary

With the exception of a single lady, all the patients were smokers, and 40 (83.5%) of 48 smokers for whom the relevant information was available persisted with their habit after the onset of the disease. 8 (16.6%) of the 48 patients claimed to have stopped smoking during the course of their disease. Among the patients who continued to smoke, 65% required an amputation, which was almost twice that seen in those who stopped smoking.
Conclusion: The long-term investigation shows that TAO is associated with frequent hospitalisations and surgical procedures. Continued consumption of tobacco in particular was found to be associated with a multiplication of the amputation rate. An influence of patient age at disease onset on the progression of TAO was excluded. In 49 (71 %) of the patients, the disease resulted in termination of the working life by either dismissal or premature retirement.

A case for cerebral thromboangiitis obliterans

The existence of cerebral thromboangiitis obliterans (CTAO) has been controversial. The clinical, laboratory and angiographic features of a young woman with recurrent thrombophlebitis, digital gangrene and a bilateral anterior opercular syndrome (Foix-Chavany-Marie) are reported. The cerebral angiogram demonstrated significant narrowing of fronto-opercular branches of both middle cerebral arteries. Histology of small digital muscular arteries revealed segmental adventitial fibrosis, narrowing or occlusion of lumen and mild lymphocitic infiltrates; occasional veins showed phlebitis. Vascular Complications of Cocaine Use
Department of Dermatology, Centre Hospitalier Universitaire Vaudois, Lausanne, Switzerland To the Editor: In their article on the neurovascular complications of cocaine use, Conway and Tamargo concluded that vessels narrowing and delayed clinical deficit after aneurysmal subarachnoid hemorrhage were due to cocaine-induced vasospasm. However, cerebral thromboangiitis obliterans (TAO), which is often unrecognized, may have the same clinical and angiographic presentation. Many cases of peripheral arteritis very similar to TAO have recently been reported with cocaine use. The same type of vascular lesions can be observed in cerebral and coronary arteries of patients exposed to cocaine.

subsection 196B(2) 1. Being of the view that there is sound medical-scientific evidence that indicates that Buerger's disease and death from Buerger's disease can be related to operational service rendered by veterans, peacekeeping service rendered by members of Peacekeeping forces and hazardous service rendered by members of the Forces, the Repatriation Medical Authority determines, under subsection 196B(2) of the Veterans' Entitlements Act , that the factors that must as a minimum exist before it can be said that a reasonable hypothesis has been raised connecting Buerger's disease or death from Buerger's disease with the circumstances of that service, are:
(a) smoking 20 cigarettes per day or its equivalent in tobacco products, for at least three years before the clinical onset of Buerger's disease; or (b) smoking for three pack years before the clinical worsening of Buerger's disease; or

Abstract
Questions assertions that cigarette smoking causes Buerger's Disease. Notes that even though a large percentage of those with the disease also smoke cigarettes, not enough is known about the disease to claim causation. Argues that claims that Buerger's Disease rates are rising with the prevalence of smoking are "not consistent with the observation of a professor of medicine that 'when the total number of heavy smokers in the general population is considered, the actual number in whom [Buerger's Disease] develops is exceedingly small." Adds that the rates of the disease in women are extremely low even though their smoking rates have increased. Mentions that many diseases have similar symptoms as Buerger's Disease and the increased rates may be due to misdiagnosis. Argues that other factors may be involved in developing the disease, such as genetics or the environment. Includes bibliographic references.

Incidence
It affects 6 in every 10,000 people, 95% of them men. It affects mostly men between the ages of 20 and 40. Recently it has been seen more in people over the age of 50. It is more common in those of oriental race, including those from South-east Asia, India and the Middle East but it is less common in those of African origin. Prevalence Prevalence ranges from about 12% in the USA to 63% in India and even 80% among Jews of Ashkenazi ancestry living in Israel. Part of this difference in disease incidence may be due to variability in diagnostic criteria Risk Factors Rarely it affects children with auto-immune phenomena and a strong level of inflammation.
